PURPOSE EXHIBIT A The purpose of this Performance Work Statement (PWS) is to provide the general and specific requirements for Pest Control Services at the Housing Authority of the City of Durham. 2. GENERAL REQUIREMENTS The contractor shall furnish all labor, supervision, equipment and materials to provide pest control services in accordance with the requirements specified herein. All services performed by the contractor shall be in accordance with all Federal, State, and Local pest control requirements and regulations. State regulatory and licensing Agency rules and standards shall have priority over any standard set forth in these specifications. The contractor must inspect and discuss with property manager personnel the problems and challenges required to develop customized treatment programs for each property. Customized treatment programs should take into consideration all aspects of each property that requires a tailored treatment program for optimum efficiency. The DHA shall reserve the right for Callback Treatment regardless of the customized treatment program for each property. Callback treatments to previously treated units where infestation is still evident may be initiated by Property Management staff and/or the contractor. All callback treatments shall be completed within fortyeight (48) hours of the request. It shall be the responsibility of the contractor to maintain a record of callback treatments and provide monthly reports to the DHA regarding the activity and recommended actions necessary by the DHA. Unlimited callbacks at all sites shall be included in the contract price. 3. REQUIRED METHOD OF TREATMENT Treatment must treat all existing and potential harborage sites through the use of diversified techniques, tools and materials to increase the likelihood of a successful treatment. The treatment program may be a spraying program, using approved sprays, applied to all required areas. Extreme care shall be used during the application of spray. The contractor is to insure that spray is not applied to occupant s personal property including personal belongings, such as tables, nightstands, dressers and other furniture. The contractor shall be liable for any and all damage, i.e., spotting or staining, to occupant s personal property due to inadvertent over spray of pesticide. Treatment methods may also include boric acid, bait and repellants, etc. Spillage and overruns shall be wiped clean. Dusts shall be used in treatment of cracks and crevices where there is not a possibility of contamination and shall be used exclusively in treatment between floors and ceilings except where such areas are not accessible. Nests and breeding areas shall be destroyed and removed where possible and the areas thoroughly treated. Pest control services for every structure shall also include perimeter treatment not less than ten 1

2 (10) feet from the structure. This perimeter treatment shall not only consist of pest control for all of the requirements in this proposal, but also for such activities as bee s nests, hornets/wasps nests, and fire ants. 4. GENERAL CONDITIONS Contracts for the services requested herein will be awarded to pest control operators for the period from November 1, 2010 through October 31, 2011 subject to continued availability of funds for these services. DHA is interested in contracting with a firm, to provide pest control services for a contract duration period of one year(s) with two one year options to extend the contract. All service provided under this contract shall be in accordance with all federal, state and local pest control requirements and regulations. Contractor shall hold all necessary licenses and all service technicians must be certified applicators or registered technicians as defined by the NC Dept. of Agricultural & Consumer Services and shall be thoroughly trained according to existing state requirements for effectiveness and safety. State regulatory and licensing Agency rules and standards shall have priority over any standard set forth in these specifications. The contractor must furnish to the Housing Authority of the City of Durham a photocopy of the valid structural pest control license under which pest control services will be performed. Contractor shall assure maximum precautions for resident safety in use of chemical or any products or equipment which might be hazardous if improperly handled. Safety precautions shall preclude unlimited or unprotected use of such chemicals, products, or equipment. The safety of residents shall be a priority over pest eradication. Service provided under this Contract shall not include any structural repairs of damage existing prior to commencement of service or not determined to be the fault of the Contractor. Any structural damage or repairs needed and so noted by the Contractor shall be corrected by the Contractor within seven (7) working days after receipt of notice from the DHA. Contractor shall provide Material Safety Data sheets to the DHA on all chemicals used. Contractor shall vacuum unit to remove dead pests before and after treatment. 5. TYPES OF PEST TO BE CONTROLLED Specifications for the eradication of cockroaches, ants, to include fire ants, silver fish, spiders, bees wasps, fleas, mice, rats and bats, bed bugs etc. Control of cockroaches, ants, spiders and silverfish shall include inspection, identification of and treatment to eliminate existing infestations and to prevent future infestations of but not necessarily limited to the following species: 1. American Periplaneta Americana (Linnaeus) 2. German Blatella germanica (Linnaeus) 3. Oriental Blatta orientalis (Linnaeus) 2

3 4. Brown-banded Supella longipalpis (Serville) 5. Smokey-brown Perilaneta fiugliginosa (Serville) 6. Australian Periplaneta austrlasiae (Fabricius) 7. Brown Periplaneta brunnea (Burmeister) 8. Woods Parcoblatta spp. 9. Other species of cockroaches which might occur in isolated 10. All ants 11. Silverfish Lepisma saccarina. (Linnaeus) 12. All spiders 13. Fleas Treatment to be provided upon request: Rodent control within units, Termite treatment and bed bugs. 6. AREAS OF TREATMENT Contractor shall locate nests and breeding/harborage sites, routes of travel, and points of entry. Where possible, these nests and breeding areas shall be destroyed and removed and the areas properly treated. The contractor shall treat those areas as necessary, as well as door sills, window frames, and other potential points of entry, to include all exterior walls, doors, windows, etc. Fire ants, rats, bats and bed bugs are treated, eradicated and/or removed from all DHA properties and sites. Contractor or sub-contractor must be experienced and certified in wildlife preservation and comply with all local, state, and federal guidelines, rules and regulations. All treatments under this paragraph shall be in accordance with label directions except where these areas prove inaccessible. Contractor shall treat other areas including, but not limited to: a. cracks behind baseboards, door frames, and window casings b. crevices between furniture or appliances and walls c. leg contacts floors d. cracks behind wall outlets e. false-bottom cabinets f. circuit boxes g. hollow legs of appliances h. shelf brackets i. under table tops j. behind sinks k. cracks at pipes and wiring through walls and floors l. around baseboards m. in all closets n. in all basements o. inside cabinets p. motor compartments in appliances q. around and above heating ducts whether exposed of unexposed r. in all hollow or drop ceilings s. exterior walls, doors, windows, etc. 3

4 t. office and common areas (i.e. community centers, kitchens, bathrooms, etc.) u. trash chutes v. public restrooms w. behind appliances (i.e. stoves/refrigerators) x. pantry shelves y. compactor rooms z. electrical rooms (interior & exterior) 7. UNIT PREPARATION The Housing Authority of the City of Durham shall provide access to treatment areas at prearranged service times. The Contractor shall provide the DHA with written instructions for residents detailing the resident requirements for unit preparation of the entire unit. Residents will be informed that they will be charged an administrative fee and additional charges as necessary, if their unit is not prepared in accordance with these instructions and that non-preparation after adequate notice of treatment may constitute a lease violation. Beyond these measures to be taken by the DHA, the Contractor will bear all costs associated with adequate unit preparation for initial and callback treatment. 8. MATERIALS STANDARDS AND SPECIFICATIONS Chemicals and equipment used shall be approved for domestic pest control use and shall have current registration number from the Environmental Protection Agency (EPA) and the North Carolina Department of Agriculture. Chemicals shall be used in strict accordance with manufacture s recommended procedures for safety and maximum effectiveness. Below is a suggested list of approved products but not required General Material Standards Pesticides and equipment shall be approved for domestic pest control use and shall have a current registration number from the Environmental Protection Agency and registered for use in North Carolina. Chemicals shall be used in strict accordance with manufacturer s recommended procedures for safety and maximum effectiveness as specified on label. Material standards for the control of cockroaches, ants, and silverfish. The following shall be used in combination or alone according to the particular infestation: Organic Phosphates Carbamates Inorganics Pyrethrins and Synthetic Pyrethroids Baits Insect growth regulators Approval Material MRF2000 Roach Bair Boric Acid crack and crevices only PT240, PT230, PT250 Whitmire Systems of Aerosols 4

5 Killmaster II Avert Roach Bait Baygon Roach Bait Gencor Pyretheum, Diazinon, Dursban, Silica Gel and Drione Dust Maxforce Ant and Roach Baits BP 100, 300 ULV fog Commodore WP, EC Demon WP, EC All materials to be applied in connection with an approved application plan shall be included in the contractor s Bid and in accordance with label instructions. CONTRACTOR MUST SPECIFY AND PROVIDE AN UP TO DATE LABEL OF ALL PRODUCTS TO BE USED IN THE BID FOR EACH SITE/LOCATION APPLICATION 9. INSPECTION Service provided shall be available for inspection by any authorized personnel of the Housing Authority of the City of Durham during period of treatment. Further inspection may be requested by The Housing Authority of the City of Durham from the State Pest Control Licensing Agency and will be accepted by the Contractor. 10. REPORTING Contractor shall report to management officials any condition that hinders adequate results in providing service under this contract. Within five (5) working days after the completion of the treatment in any unit, the contractor will deliver to the DHA a written report for that unit documenting the level, types and locations of infestations; treatment methods and materials used; and, degree of unit preparation by resident. The Contractor shall be required to report to management any conditions he may discover while providing treatment under this Contract which might prove hazardous to public health and safety or which might affect the structural integrity of the property being serviced, or might enhance conditions for pest investigation. Contractor shall be required to provide the following items to each facility for which it provides services prior to any payments being made: a. The pest control technician s name b. The specific areas serviced at the facility c. The types, concentrations, and approximate amounts of pesticides and bait stations applied d. The date e. The time of arrival f. The time of departure g. The pest control technician s signature 5

6 h. Any recommendations for maintenance or sanitation, which, in the opinion of the pest control operator, would improve any pest problems. i. Note whether occupant is home or not j. Note condition of pest infestation in unit and if service was allowed (L) light; (LM) light/moderate; (h) heavy; (R) refused; (RM) refused medical; (P) poor housekeeping. 11. WORK PERFORMANCE EVALUATION Should the pest control operator fail to satisfactorily perform this contract during any month of the contract period, The Housing Authority of the City of Durham may terminate the contract upon written notice to the pest control firm. The Housing Authority of the city of Durham may terminate this contract by giving the Contractor ten (10) days notice. The pest control firm shall be deemed to have failed to satisfactorily perform this contract under any of the following circumstances: A. If a satisfactory service call to handle a complaint is not handled within five (5) business days after being contacted by the contract coordinator. B. If it becomes evident in a sanitary environment that the pest control services being provided does not satisfactorily eradicate the facility of pests. C. If the firm fails to obtain renewal of its N.C. Structural Pest Control License at each renewal date during the contract period. A designated representative of the Housing Authority Site Management Team of the City of Durham MUST countersign this report, with an additional copy to the facilities contract coordinator along with a monthly summary of treatment concern(s) or corrective action(s) needed. 6
