Please be advised that the changes below are applicable to the original specifications of the above referenced solicitation. Added or new language to the ITB is highlighted in yellow, while deleted language has been struck. This Addendum includes the Department s answers to written questions received. 2 Responses to Written Questions FDC ITB Question Number Question 1 What is the frequency of sample courier service required at each facility? 2 Page 26, Apalachee CI, Disinfection Byproducts when we see quantities that are the same for each section, e.g. 6 TTHMs and 6 HAA5 per year, are these samples submitted at the same time? Answer The frequency of sampling, and information relating to ground water monitoring wells are based on the permits at each facility. A copy of each permit is being provided as part of this Addendum. The frequency, size and parameters of sampling are based on the permits at each facility. 3 Question Number Question 8 Can you supply the well size and depth for each well and the number of wells per facility? Answer Please see the Answer to Question #1. 9 Is a compositor sampler required for the wastewater samples? 10 Do you currently use any other equipment for sample collection that the bidder is required to supply? 11 Giardia and Cryptosporidium are not listed as line items on the bid price sheet. Will Giardia and Cryptosporidium be added as line items on the price sheet? 12 The bid states All analytical results shall be reported within seven (7) calendar days from the time of pick-up, with the exception of the coliform analysis, which shall be reported within 24 hours, by to the Department s WWTP Operator. Does the 7 calendar day turnaround time apply to Drinking Water analysis as well? Typically, Drinking Water analysis has a minimum 2-3 week turnaround time 13 There is no scope of services listed for Avon Park CI, Hardee CI, Lake CI, Lowell CI, Polk CI, and Sumter CI. Are these facilities included? 14 Pricing is specified globally for each region, rather than by facility as in the past. Are you requiring that a lab service all facilities in a given region? If not, can individual facilities be specified in the bid? Insofar as the travel, pick up, and sampling time must be baked into prices, the costs per facility will vary substantially based on the scope of services and location of the facility. Yes it is required. The successful Bidder will pick up the composite sample from the wastewater plant. No, the Bidder will be required to provide all its own sampling equipment, as appropriate, for the type of sample it is testing. Please see the Revised Attachment II and III, included with this Addendum. All analytical results would include drinking water. In accordance with Section 2.3 of the ITB, the successful Bidder will establish a sampling plan in conjunction with the Department, to ensure that all necessary samples are in compliance with the various Institutions permits and rules of the Florida Department of Environmental Protection (DEP). A proposal for any analysis that will take a minimum of 2-3 weeks can be outlined in the sampling plan, and an exception to the requirement may be granted by the Department. Please see the revised Attachment II, included with this Addendum. Please see Section 4.9, Basis of Award of the ITB. Four total awards will be made, one per Region. Bids must be firm, and include all packaging, handling, shipping, delivery charges, environmental and service fees, and any trip fees based on the size of the Region. Addendum #004 FDC ITB

4 Question Question Number 15 With regard to Performance Measure #1: There are tests which cannot be completed in 7 calendar days, including Whole Effluent Toxicity and some radiological tests (possibly others). Some of these facilities are remote, and may ship coliform samples, resulting in violation of the 24 hour criteria. Strict assessment of the financial consequence clause under these circumstances would be unfair and inappropriate. Allowing judgment by DOC personnel would seem to violate the bid specification and result in arbitrary application. I suggest that more reasonable time criteria could be used and still meet the DOC's needs.
