1 How to access the Department Orientation Checklist on Gladiator: 1. Click on Online Forms on the Corporate Reference menu of Gladiator 2. Choose HR-Services from the drop down list of forms 3. Click on the link labeled Department Orientation Checklist Supervisor View The next page to appear is the Supervisor View. You should see your name listed for the supervisor. You should also see the new or transferred employee listed below. If the action says New that means a form has not been started yet. Since you will have the ability to save the form and come back later to update as needed the action Edit allows you to go back to the form and finish. The bottom table of data will list completed forms for your employees. At this point you have the ability to print the form for your records or the employee. The forms will remain on this page for future reference as well. Once you complete the form and electronically sign a similar page will be available for the employee to sign in, view, and electronically sign.

Blank Checklist This allows you to print a hard copy of the form. Note: Paper forms will not be accepted by Human Resources. 3. Information Icon - Information about the different sections. 4. Date Picker By clicking on the button a calendar will pop-up allowing you to choose the date the topic/skill/function was discussed. 5. Clear Button Button to clear out the date in the date field. 6. Set Today Button Button to quickly choose today s date to fill into the date field. 7. Not Applicable Checkbox Clicking this button indicated the topic/skill/function is not applicable to the employee. NOTE: Human Resources will be monitoring what is chosen as not applicable. 8. Policy Hyperlink Hyperlink to the Gundersen Lutheran policy to show and discuss with the employee. 9. Save/Continue Button Button to save the information entered on the form and continue entering more data. As always saving your work often is a good habit. 10. Electronic Sign Button Used to sign the form with your electronic signature. This is only possible if everything option on the form is filled out with exception of the comments. 11. Save/Close Button Ability to save the form and return at a later time. 12. Cancel Button Leaves the page without saving the results. If there is unsaved data on the page a message will appear to ask you if you want to save the changes.

4 Once the form is completed by the supervisor and is electronically signed then the employee needs to log into a computer or gladiator and follow the same steps to sign the form as well. If the checklist does not appear for either you or your employee follow the steps below to log into gladiator without having to log into the computer. 1. Click on Manual Login This is the first link under the Global Tab 2. A login page with load use the employees novell username and password. 3. Follow the steps above to access the Department Orientation Checklist. New hire/transfer Employee View 4. Click on the View Form Button This will bring up a view of the checklist for the employee to verify the information. (This will always be available to the employee)
5 5. If the checklist has never been signed by the employee there will be an button at the bottom of the checklist 6. Also listed at the bottom in the supervisor view is a message stating electronic signature information. If you have any questions or concerns regarding the Department Orientation Checklist, please contact your HROM or an HR Program Consultant at ext or at ext If you need to refer to the employee s checklist in the future, you may view it from the Supervisor View screen.
