1 MPH Learning Plan Instructions PRIOR to completing the DocuSign MPH Learning Plan, you must complete the MPH Learning Plan Worksheet and submit to your MPH Practicum Advisor for their input and approval. Steps: 1. Download and save the MPH Learning Plan Worksheet (linked under Worksheets on the Forms page of the Field Education website). 2. Draft your preliminary MPH Learning Plan in collaboration with your agency-based field instructor and/or supervisor. 3. Submit this draft MPH Learning Plan Worksheet to your MPH Practicum Advisor. 4. Revise as instructed by your MPH Practicum Advisor and gain Advisor s final approval. 5. Launch the DocuSign MPH Learning Plan (linked on the Forms page of the Field Education website). Instructions for launching and completing DocuSign forms are linked on the Field Forms webpage. 6. Make sure that names and addresses are correctly entered into the routing form window. 7. Copy and paste the information from the MPH Learning Plan Worksheet into the DocuSign MPH Learning Plan. 8. The form will route to those individuals you have entered into the Routing Form for signatures. 9. The completed form will be uploaded to your placement profile in the Field Database and will then always be accessible to you. You will need access to this document when you prepare your SOWK 688 MSW/MPH Learning Plan and Final Evaluation form. 10. Your approved DocuSign MPH Learning Plan will be attached to the SOWK 688 MSW/MPH Acceptance Contract that you, your Field Instructor, and your On-site Supervisor (if applicable) receive via DocuSign for your review and signatures. (See below for example of MPH Learning Plan) 1

2 MPH LEARNING PLAN Before completing this learning plan, please FIRST complete the MPH Learning Plan Worksheet (Word document) available under WORKSHEETS on the forms page of the website. SECOND, this DocuSign MHP Learning Plan is then to be completed by student and may require feedback from field instructor and/or on-site supervisor. This form must be submitted to MPH practicum advisor after the field placement offer has been made, but before accepting the offer. Allow for at least 4 business days for the practicum advisor to review and provide feedback. Explain basic principles and tools of budget and resource management 11. Select methods to evaluate public health programs 12. Discuss multiple dimensions of the policy-making process, including the roles of ethics and evidence 13. Propose strategies to identify stakeholders and build coalitions and partnerships for influencing public health outcomes 14. Advocate for political, social or economic policies and programs that will improve health in diverse populations 15. Evaluate policies for their impact on public health and health equity 16. Apply principles of leadership, governance and management, which include creating a vision, empowering others, fostering collaboration and guiding decision making 17. Apply negotiation and mediation skills to address organizational or community challenges 18. Select communication strategies for different audiences and sectors 19. Communicate audience-appropriate public health content, both in writing and through oral presentation 20. Describe the importance of cultural competence in communicating public health content 21.
