6 Recent story. March 14, 2016 ALSS Recycling Inc. faces $84K in fines after failing to correct previously cited workplace safety hazards Employer name: ALSS Recycling Inc. Inspection site: 2600 Republic Blvd., Birmingham, Alabama Investigation findings: The serious violation was cited for not ensuring employees were trained to operate powered industrial trucks. The other than serious citations relate to the employer not inspecting fire extinguishers annually and failing to ensure employees voluntarily wearing respirators were medically cleared. Additionally, the company did not develop and implement a written hazard communication program for employees who handle recycled iron products. Proposed penalties: $84,000 Quote: ALSS Recycling did not fulfill its requirement to ensure the hazards that were cited in the previous inspection were corrected," said Ramona Morris, OSHA's area director. "We are expecting management to take immediate action to permanently correct the hazards in order to protect its employees."

20 How do I identify workplace hazards? By asking these 5 questions: 1) What can go wrong? 2) What are the consequences? 3) How could it arise? 4) What are other contributing factors? 5) How likely is it that the hazard will occur?

21 How do I identify workplace hazards? Good hazard scenarios describe: Where it is happening (environmental) Who or what it is happening to (exposure) What precipitates the hazard (trigger) What is the outcome that would occur should it happen (consequence) Any other contributing factors

28 Proper Planning We are all owners of certain responsibilities. It is not easy to implement or change the owner of a responsibility because other departments do not want to get more responsibility and paper work, or they may feel threatened by losing certain responsibilities. So if you want to change responsibilities in your Company, first you need to see and ask a permission to change the Culture from the TOP. Based on approval from the TOP, you can then take the next steps: On site mentoring for a program like this might take over 18 months

29 What benefits will materialize for the Company? Is there ROI? Since Supervisors/Managers will be fully accountable for their team, injuries and near misses are reduced significantly. Safety staff has more time to spend on education, training, and auditing of the safety program. And see the Vision, Strategy, and Plan towards continuous improvement. Visible Leadership in safety will appear everywhere starting from the Top down. Workers involved in creating and growing a Safety Culture. They will begin to offer up better ideas to improve their work areas and their well being. Workplace conditions will improve.

31 Stay grounded! Proper training, appropriate equipment and preventative techniques are vital when working at height Because a life is on the line 10 Practical guidelines for working at height Identify potential hazards, assess the risk and mitigate each hazard before working at height Only trained and authorized persons can be allowed to work at heights Guardrails must be built around every appropriate elevated work area Mobile elevation devices such as man lifts must be kept in good condition and inspected regularly Scaffolding must be provided with handrails, complete floors, kickboards and internally mounted ladders Ladders must be secured and used only for means of access Before working on roofs, fragile roof parts, unprotected openings and edges, and safe access points must be checked Proper training on the use of personal protection equipment must be provided A full body harness and appropriate lanyards must be worn and always attached to a secure anchor point A fall rescue plan and equipment must be in place at the time of the work Three Step Hazard Reduction 1 HAZARD ELIMINATION Eliminate the task or the need to work at height by completing the task at ground level 2 HAZARD If you must work at Prevention height, isolate people From the hazard of falling by using guardrails, secured work platform such as scaffoldings or lifts 3 neither HAZARD Ifelimination Protection nor prevention is practicable, use personal protection equipment to mitigate the effect of a fall. This includes harness, lanyards, life lines and anchor points.
