No. 1A The Nevada Universal Service Fund (NUSF) is funded by an assessment on total retail billed Nevada intrastate telecommunications revenue from both regulated and unregulated services. The NUSF is assessed to all telecommunications carriers that offer intrastate telecommunications services within the state of Nevada. The Commission ruled that telecommunications carriers may recover the NUSF charges from their retail customers by passing on the assessment rate.

ESTABLISHMENT OF TARIFF This Tariff is established to provide standardized schedules of tariff rules, regulations, rates, services, and concurrences for CenturyTel of the Gem State, Inc. B. RATES, CHARGES, RULES AND REGULATIONS Rates, charges, rules and regulations stated herein apply uniformly to all exchanges. Local exchange service and other offerings which are not universal to all exchanges will be noted as such and they shall apply only to the exchange where applicable. C. No. 1A Original Definition Sheet No. 1 DEFINITIONS Access Line A line between a central office and a customer premises which includes a Network Interface Device (NID) and which provides access to local and toll calling. Additional Listing Any listing of a name or information in the directory or Company's information records in connection with a customer's telephone number beyond that to which he is entitled in connection with his regular service. Air Line Mileage The shortest distance between the points involved. Applicant A person or other legal entity who (1) applies for service with the Company or (2) reapplies for service at a new or existing location after permanent disconnect. Base Rate Area A specific area within any exchange service area as set forth in the tariff maps. Local exchange service within this area is furnished at uniform rates without mileage charges.

13 Tariff P.U.C.N. No. 1A Original Definition Sheet No. 2 Billing Date The latest of: Building 1. The date stated on the billing. 2. Date billing was placed in the mail DEFINITIONS A structure under one roof, or two or more structures under separate roofs but connected by enclosure passageways, not crossing a public thoroughfare other than an alley, in which telephone wires or cables may be safely run. Pipes and conduits are not considered enclosed passageways. Bridged Lines (See Combination Main Service) Business Service Business service is exchange service furnished to customers whose actual or obvious use of the service is for conducting a business, trade or profession or whose use of the service is obviously not confined to primary domestic use. Call Forwarding A function which allows incoming calls to be advanced to another telephone number. The number the calls are advanced to may be changed as required by the customer. Call Waiting A tone provided to the party using the telephone to indicate another call is waiting on the line. Successive transfers between calling parties can be accomplished through switchhook operation. Central Office A switching unit in a telephone system which provides service to the general public, having the necessary equipment and operating arrangements for terminating and interconnecting customer access lines and trunks.

No. 1A Original Definition Sheet No. 3 DEFINITIONS Channel A path for communication between two or more stations or customer locations, furnished in such a manner as the company may elect, whether by wire, radio, fiber optic or a combination thereof. Circuit The term applies to a channel used for the transmission of electrical energy in the furnishing of telephone service, business, residence, public and semi-public service. Class of Service The various categories of service generally available to the customer: business and residence. Combination Main Service This service provides for serving separate business and residence locations from the same oneparty line with separate rings and rates for each location. Commission As referred to in this tariff is the Public Utilities Commission of Nevada at Carson City, Nevada. Communications Systems Denotes customer-provided channels or other facilities which are capable, when not connected to telecommunications service, of communications between customer-provided terminal equipment.

15 Tariff P.U.C.N. No. 1A Original Definition Sheet No. 4 DEFINITIONS Company CenturyTel of the Gem State, Inc. Connecting Arrangement The equipment used to accomplish the direct electrical connection of certain customer-provided facilities with the facilities of the Company. Connecting Company A corporation, association, partnership or individual owning or operating one or more exchanges and with who traffic is interchanged. Contiguous Exchanges Two exchanges which share a common boundary. Continuous Property Property owned or leased and occupied by a customer, which is not separated by public thoroughfare or by property occupied by others. Contract The service agreement between a customer and the Company under which service and facilities are furnished in accordance with the provisions of the applicable tariff and under special conditions not found in the tariff. Cost The cost of labor overhead and materials including the usual Director, Tariffs & Compliancy expenses.

16 Tariff P.U.C.N. No. 1A Original Definition Sheet No. 5 DEFINITIONS Customer A person or legal entity who has applied for, been accepted and is currently receiving service. Customer Premises Equipment (CPE) Telecommunications devices, equipment and associated wiring located on the customer's side of the Network Interface Device (NID). Customer Premises Inside Wire (CPIW) Wire for telecommunications purposes which is on the customer's premises. Customer Premises Inside Wire begins on the customer's side of the Network Interface Device (NID). Date of Presentation The date upon which a bill or notice is mailed with first class postage prepaid, in a sealed envelope properly addressed to the customer, or if not mailed, the date upon which that bill or notice is presented to the customer by a representative of the Company. Demarcation Point (See Network Interface Device) Deposit A cash payment made by the customer to establish or reestablish credit with the Company. Direct Electrical Connection Denotes a physical connection of the electrical conductors in the communications path. Directory Listings Essential information in the telephone directory or information records of the Company whereby telephone users may ascertain the telephone number of a listed customer.

No. 1A Original Definition Sheet No. 6 Directory Period DEFINITIONS The directory period is from the day on which the directory is first distributed to the subscribers to the day the succeeding directory is first distributed to customers. Drop Wire Wires between an open wire lead, aerial or underground cable terminal and the point of entrance to the building (Network Interface Device) in which the customer's telecommunications service is located. Employee's Telephone Service Basic Exchange Access Service available to eligible employees at a discounted monthly rate. Exchange A specified area established for the furnishing of communication service. It usually embraces a city, town, village or unincorporated community and environs thereto and may consist of one or more central offices, together with the associated plant used in furnishing service within that area. Exchange Access Line (See Access Line) Exchange Service Telephone service furnished between customer's stations located within the same local service area. Extended Area Service (EAS) Interexchange telephone service furnished at flat or message rates between one or more exchanges.

No. 1A Original Definition Sheet No. 7 DEFINITIONS Facilities Supplemental equipment, apparatus, wiring, poles, cables and other materials and mechanisms necessary to, or furnished in connection with telephone service. Farmer Line Service Farmer line service is suburban exchange service furnished to customers beyond the base rate area by means of lines and stations which may be owned and maintained beyond a designated junction by organized associations, or customers. Flat Rate Service Service furnished at a fixed monthly charge. Foreign Exchange Service (See Tariff P.U.C.N. No. 3A)

No. 1A Original Definition Sheet No. 8 DEFINITIONS Grade of Service The number of parties served on a telephone line such as one-party, two-party, four-party, suburban, etc. Grandfathered Equipment Equipment classified by the FCC as having been directly connected to the network by a telephone company. Its use may be continued as long as the equipment remains serviceable and the circuitry is not revised. Grandfathered Service Existing service not available to new customers. Individual Line Service (See one-party service) Inside Wire (See Customer Premises Inside Wire - CPIW) Installation Charge An initial non-recurring charge associated with the installation of telecommunications service. Interconnection The method by which telecommunications facilities of a company are arranged to transmit to or receive information from customer premises equipment.

No. 1A Original Definition Sheet No. 9 DEFINITIONS Interexchange Toll Receiving Service (Zenith Service) A combination of exchange and toll service, whereby a business customer in one exchange may arrange to receive calls placed to a special number by the subscribers from another exchange. The customer agrees to accept such calls on an operator handled station toll call basis. The special number is published in the directory of the exchange where the call originates. Line Extension The outside plant required in addition to existing facilities to render telephone service in areas without service. Local Channel The portion of a channel which connects a location to the interchange channel; it also applies to a channel connecting two or more locations within an exchange area. Lifeline/Link Up A service that offers reduced charges to qualifying low-income subscribers.

21 Tariff P.U.C.N. No. 1A Original Definition Sheet No. 10 DEFINITIONS Local Exchange The exchange in which the primary exchange access line is provided. Local Message A message not subject to toll charge. Local Private Line A non-switched line located wholly within an exchange, furnished for the customer's own use for communication or signaling between points on that line. Local Service Telephone service furnished between customer's location within the same local service area. Local Service Area The area within which telephone service is furnished under a specific schedule of rates. This area may include one or more exchanges without the application of toll charges. Measured Service A service for which charges are based upon the number of originated messages placed by the customer to stations within the same local or message rate calling area.

Mileage Charges Charges in addition to the access line charge recurring rates based on distance measurement as provided in the tariff. Minimum Contract Period The minimum length of time for which a customer is obliged to pay for service, facilities and equipment, whether or not retained by the customer for such minimum length of time. Multi-Party Service Party line service, other than one-party service. Network Interface Device The points of common termination of Company-provided and customer-provided facilities.

23 Tariff P.U.C.N. No. 1A Original Definition Sheet No. 12 DEFINITIONS Non-Contiguous Exchanges Exchanges whose boundaries do not adjoin. Non-Listed Telephone Number Service The omission of the customer's listing from the telephone directory, but not from the information records, at the customer's request. Non-Published Telephone Number Service The omission of the customer's listing from both the telephone directory and the information records, at the customer's request. Non-Recurring Charge (NRC) A one-time charge covering the installation of facilities or the provision of other standard services provided by the Company. Off-Premises Extensions and Lines Off-premises extensions are additional appearances of a private branch exchange or key system station line, or an additional appearance of a one-party access line or station, at a location outside the premises in which is housed the primary access line.

24 Tariff P.U.C.N. No. 1A Original Definition Sheet No. 13 DEFINITIONS One-Party Service An access line serving only one customer. Party Line Service An access line serving more than one customer. Payphone Services Payphone Services provide telephone service to customer-leased or owned payphones with or without coin collecting devices. Permanent Disconnect A service is permanently disconnected when the customer's service has been totally discontinued. Premises Business Premises: The building, portion or portions of a building, used and occupied by the customer in the conduct of his business. Where floor space in adjoining or adjacent buildings is made continuous at one or more floor levels by suitable conduit or covered walkways, furnished by the customer, all floor space in both buildings is considered as the same premises insofar as the customer who uses and occupies such continuous floor space is concerned. Residence Premises: That portion of an individual house or building or one flat or apartment occupied by the customer and his personnel. Private garages and caretaker's quarters and other locations, such as private laundries, patios, garden houses, and private swimming pools, which are a part of the customer's domestic establishment and used in connection with an individual residence are considered as a part of the premises of that residence if (a) located on the same continuous property, (b) not more than 300 feet from the main building, and (c) not separated from the residence by a public thoroughfare.

25 Tariff P.U.C.N. No. 1A Original Definition Sheet No. 14 DEFINITIONS Private Line A line consisting of dedicated communication channels connecting two or more locations. Protector (See Network Interface Device - NID) Rate A recurring monthly charge made in conjunction with the provision of a service. Registered Equipment Denotes terminal equipment, multi-line terminating systems and protective circuitry which comply with and have been approved within the registration provisions of Part 68 of the Federal Communications Commission Rules and Regulations. Residence Service A class of exchange service furnished to an individual at a residence or place of dwelling where the actual or obvious use of the service is for domestic purposes. Route Mileage The distance measured along the right-of-way of the circuit between any two or more given points on that circuit.

No. 1A Original Definition Sheet No. 15 Service Connection Charge DEFINITIONS A non-recurring charge applicable to the installation of telephone service and facilities or the provision of other standard services provided by the Company. (Also see non-recurring charge.) Speculative Project An undertaking of a speculative nature which, in the opinion of the Company, appears to involve risk of failure. Speed Calling A feature allowing a telephone user to reach frequently called numbers by abbreviated dialing.

27 Tariff P.U.C.N. No. 1A Original Definition Sheet No. 16 DEFINITIONS Subscriber (See Customer) Suburban Area That portion of the exchange area located outside of the Base Rate Area. Suburban Service The grade of party line service furnished outside the Base Rate Area, but within the exchange area, and to which no mileage charges apply. Supersedure The transfer of service, including the telephone number, from one customer to another with the express consent of the relinquishing customer and with the agreement of the new customer to assume the responsibility for all charges outstanding. This arrangement requires continuous billing. Supplemental Services Services other than basic service furnished to meet the special needs or convenience of customers. Tariff The rates, charges, rules and regulations adopted and filed by the Company and approved by the Nevada Public Utilities Commission of Nevada.

No. 1A Original Definition Sheet No. 17 DEFINITIONS Telephone Company (See Company) Temporary Disconnect The suspension of telephone service at the request of the customer or on the initiative of the Company without permanent disconnect of service. Temporary Service Local service definitely known to be required for a short period, such as service provided for contractors for use during the construction of a building, sales campaigns, athletic contests, conventions, fairs, circuses, etc. Terminal Loop The wire facility used to serve off-premises stations. Termination Charge The charge applicable when an agreement for service is terminated by the customer before the expiration of the minimum agreement period. Three Way Calling The ability to hold an existing call and, by dialing a prefix code and the telephone number of a third telephone, extending the call to that telephone. Toll Line A toll line is a line between two or more exchanges or toll stations over which service is furnished on a toll message rate basis.

No. 1A Original Definitions Sheet No. 18 DEFINITIONS Toll Message A completed call between two exchange stations located in different local service areas, between two toll stations, or between a toll station and an exchange station. Toll Rate The charge prescribed for toll messages based upon the duration of the message and distance between exchanges. Toll Service Telephone service between exchanges of locations for which a toll rate is charged. Touch Calling Where central office facilities are available, a type of high speed address signaling (dialing) in which buttons in a bank of twelve can be depressed to give one of a group of audio tone pairs for signaling directly over the circuit. Trunk Line An exchange access line between a private branch exchange and a Company central office. Type of Service Refers to flat rate service, message rate service or payphone services.

32 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 3 RULE AND REGULATION No. 1 APPLICATION OF REGULATIONS A. The regulations set forth herein apply to intrastate services and facilities furnished within the State of Nevada by CenturyTel of the Gem State, Inc. hereinafter referred to as the Company, subject to the jurisdiction of the Public Utilities Commission of Nevada. B. No officer, employee or agent of the Company has authority to change, amend, or waive any rate or regulation approved or prescribed by the Commission. Rates and regulations may be changed or cancelled only with the consent or approval of the Commission. C. The Company furnishes exchange, toll and private line service throughout the territory it serves, as shown by its filed rates, regulations and maps. D. The Company does not transmit messages, but offers the use of its facilities, where available, for communication between parties subject to the terms and conditions specified in this tariff.

33 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 4 RULE AND REGULATION No. 2 DESCRIPTION OF SERVICE A. GENERAL 1. Exchange service is available through facilities owned and maintained according to the standards of the Company, and in multi-office exchanges, is operated from the central office designated by the Company. 2. Each exchange area is generally divided into a base rate area and a suburban area. 3. Telephone service is provided for the use of the customer, his family, employees or business associates, or persons residing in his house-hold. 4. No resale of telephone service by the customer is permitted, except as specifically specified elsewhere in the Tariff. 5. Where commercial power is required in the operation of equipment and service, the customer, where requested, shall furnish, install, and maintain the necessary power wiring and power outlet on his premises and supply any necessary electrical energy at his expense. 6. The customer shall provide on his premises and at his expense, space, satisfactory to the Company, for placement of all equipment and facilities necessary for the furnishing of service to him.

Individual and two-party line business and residence service, key system exchange access line and private branch exchange system trunk or station line service are rendered in the suburban area under rates for that service applicable in the base rate and supplementary base rate areas, plus mileage rates. 2. Miscellaneous service, including private line and supplemental equipment is furnished by the Company under its schedule of rates. 3. Service is furnished at the rates shown in the Exchange Service schedule at the customer's premises.

35 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 6 RULE AND REGULATION No. 2 DESCRIPTION OF SERVICE D. APPLICATION OF RATES The applicability of business and residence rates is governed by the actual or obvious use made of the service. The use to be made of the service will be ascertained from the applicant at the time of the application for service. 1. Business rates apply at the following locations: a. In offices, stores, factories, and all other places of a strictly business nature; b. In boarding and rooming houses, colleges, clubs, libraries, lodges, hospitals, public, private and parochial schools, offices, lobbies and halls of hotels, apartment buildings, churches, and other similar institutions; c. At any location when the listing of "office" is provided, or when any title indicating a trade or profession is listed (except as may be modified under the directory listing rules and regulations and conditions governing directory listing service); d. At residence locations with an off premises business extension or when the customer has no regular business telephone service and the use of the service by the customer, members of the household, or guests, is more of a business than residence nature as might be indicated by advertising through newspapers, handbills, billboards, circulars, business cards, or otherwise; e. In general, at any place where the substantial use of the service is occupational rather than domestic.

In private residences; in residential apartments of hotels and apartment houses; and when all stations are in locations which are a part of a domestic establishment. b. In general, at any place where the substantial use of the service is domestic. 3. If it is found that a customer is using residence service for business purposes, the Company will thereafter require the subscriber to take business service, except in cases where the customer thereafter uses the service for social or domestic purposes only.

37 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 8 RULE AND REGULATION No. 3 APPLICATION FOR SERVICE A. APPLICATION FOR SERVICE 1. Prior to the establishment of service, the Company may require an applicant to: a. Sign an application form furnished by the Company. b. Make an advance payment of any service connection and/or installation charges; and the charge for service for the period for which bills are regularly rendered as specified in the Rate Schedule. c. Post a deposit in accordance with Rule and Regulation No. 4. d. Federal, State or Municipal governmental agencies will not be required to make advance payments or post deposits. 2. The furnishing of service by the Company and acceptance thereof by the customer shall be deemed to constitute an agreement between the Company and the customer for the payment of the rates and charges under the applicable schedules in effect. 3. The Company will accept oral or written application from a customer for additions to, or changes in, the existing service. 4. An application is merely a request for service and does not in itself bind the Company to serve except under reasonable conditions, nor does it bind the applicant to take service. 5. An application for service cancelled by the applicant or the Company prior to the establishment of the service applied for is subject to the following conditions:

38 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 9 RULE AND REGULATION No. 3 APPLICATION FOR SERVICE B. CANCELLED BY APPLICANT 1. If cancellation is requested prior to the start of installation, the application will be cancelled by the Company and no charge applies. 2. If cancellation is requested subsequent to the time installation has been started, the application will be cancelled by the Company, and the Company may collect the lesser of the following charges: a. A charge equal to the estimated costs incurred in such installation less estimated net salvage. b. The basic termination charge, installation charge, non-recurring service connection charge, as appropriate. NOTE: Installation is considered to have been started when the Company incurs any expense in connection there with or in preparation therefore which would not otherwise have been incurred, provided: (1) The customer has advised the Company to proceed with the installation, and (2) The Company has accepted the order. 3. If cancellation is requested after completion of an installation, it will be treated as a discontinuance of service and the minimum requirements of the rate will be applicable.

39 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 10 RULE AND REGULATION No. 3 APPLICATION FOR SERVICE C. CANCELLED BY THE COMPANY If the applicant refuses to comply with the Company's Rules and Regulations prior to the establishment of service, the Company may cancel the application, in which event any amount collected from the applicant will be refunded.

40 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 11 RULE AND REGULATION No. 4 CREDIT AND DEPOSITS A. ESTABLISHMENT AND MAINTENANCE OF CREDIT 1. Establishment of Credit The Telephone Company is not obligated to furnish service to any individual or firm that owes for service previously-rendered at the same or a different address, until arrangements have been made to liquidate such previous indebtedness to the Company. In order to ensure the payment of all charges due for its service, the Telephone Company may require any customers who are known credit risks to establish and maintain credit in one of the following ways: (a) (b) (c) By furnishing references acceptable to the Telephone Company. By providing a suitable guarantee in writing, in form prescribed by the Telephone Company. Make an appropriate deposit. 2. Amount of Deposits The amount of deposit required for the purpose of establishing a customer's credit shall not exceed his estimated bill for exchange service and toll charges for one normal billing period plus forty-five days. The Telephone Company may require the customer to increase the amount of the deposit at any time if, in its opinion, the charges billed against the customer are found to warrant such an increase.

41 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 12 RULE AND REGULATION No. 4 CREDIT AND DEPOSITS A. ESTABLISHMENT AND MAINTENANCE OF CREDIT (Continued) 3. Deposit Not to Affect Regular Collection Practices The fact that a deposit has been made shall in no way relieve the applicant or customer from complying with the Telephone Company's regulations as to advance payments and the prompt payment of bills on presentation; nor constitute a waiver or modification of the regular practices of the Telephone Company providing for the discontinuance of service for non-payment of any sums due the Telephone Company for services rendered. The Company may discontinue service to any customer failing to pay current bills without regard to the fact that such customer has made a deposit with the Company. 4. Interest to be Paid on Deposits Interest shall be paid by the Company on all deposits made for the purpose of establishing credit. Interest will be computed at the rates as set forth in Chapter of Nevada Revised Statutes and paid from the date of payment of the deposit to the date of withdrawal or refund of such deposit. When the deposit is held for more than 12 months, interest will be paid annually to the customer or credited to the customer's account. 5. Discontinuance of Service for Failure to Establish Credit Service may be discontinued for failure to establish credit, as authorized above, within five (5) days after the Company has served or mailed notice requiring the subscriber to do so. 6. Restoral of Service Charge Where service has been discontinued for failure to establish credit as authorized above, the regular restoral of service charge will be made and collected by the Company.

42 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 13 RULE AND REGULATION No. 5 RENDITION AND PAYMENT OF BILLS A. A customer is responsible for the payment of all exchange, toll, and other charges applicable to the customer's service, including local, state and federal taxes made in accordance with the Company's schedules of rates and Rules and Regulations as contained in this tariff. The customer is held responsible for all charges for telephone service rendered at his telephone, including charges for toll messages on which the charges have been reversed. Failure to receive a bill does not relieve the customer of the responsibility for payment in accordance with the provisions set forth herein. 1. Bills Regular monthly bills are issued in accordance with Commission rules. Bills may contain a notation regarding payment of the bills. For billing purposes each month is presumed to have 30 days. 2. Rendition of Bills a. Flat Rate Exchange Service and Semipublic service may be rendered in advance and are payable upon presentation. b. Toll Service (1) Bills for toll service will be rendered monthly in arrears, except, at the option of the Company, they may be rendered daily, weekly or any other period in arrears. (2) Special toll bills may be rendered, if warranted by the judgment of the. In such cases the amount billed is due and payable on demand.

43 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 14 RULE AND REGULATION NO. 5 RENDITION AND PAYMENT OF BILLS A. (Continued) 3. Minimum Contract Period a. Except as specified elsewhere in this Tariff, the minimum contract period for exchange service is one month from the date service or additions to service are established at a particular location. The minimum charge is the established rate for one month. b. Special contractual arrangements for special equipment or special assemblies of equipment not otherwise provided for in this Tariff are developed as required. c. The length of contract period for directory listings, where the listing actually appears in the directory, is the directory period.

44 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 15 A. (Continued) 4. Payment of Bills RULE AND REGULATION No. 5 RENDITION AND PAYMENT OF BILLS a. Payment of bills for telephone service shall be made by mail or to a duly authorized collector of the Company. b. Closing bills, special bills, bills rendered on vacation of premises or bills rendered to persons discontinuing exchange service are payable upon presentation. c. Non-recurring charges for re-establishment of service are payable before service is restored. d. All bills for local, toll or miscellaneous services are due when rendered and payable by mail or to an authorized collection agency, on or before the 10th day of the month in which the bill is rendered. e. In the event of failure by the customer or those responsible to pay any regular bill is rendered or to promptly settle special toll bills, the Telephone Company may discontinue service without further notice at any time during such default. The service need not be restored unless or until all amounts due on the day of payment are paid in full including the restoral of service charge. 5. Prorating of Bills Bills for telephone service are normally rendered on a monthly basis. Any bills rendered will be prorated on the basis of a thirty (30) day billing period.

45 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 16 RULE AND REGULATION NO. 5 RENDITION AND PAYMENT OF BILLS A. (Continued) 6. Returned Check Charge A service charge of $5.00 will be billed to any customer whose check is returned to the Company by a bank because that customer's account is closed or does not have sufficient funds to cover such check. Should the Company, after having notified a customer of its intent to discontinue service for non-payment of an account, receive such check as payment, it may disconnect service. The Company may require payment of the account before service is restored. 7. Adjustment of Charges A refund for over-billing of the excess charges will be made when such amount can be determined. When the period cannot be determined, the maximum refund will not exceed an estimated amount equal to such over-billing for a three year period. 8. Restoral of Service The regular restoral service charge will be made for reconnecting services which have been discontinued for non-payment of charges due. No allowance will be made for loss of service during the period service is disconnected for nonpayment if payment is made and service is reconnected before the completion of an order to terminate the service. Subsequent to the completion of an order to terminate the service, it may, at the option of the Telephone Company, be reestablished only on the basis of a new application.

46 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 17 RULE AND REGULATION NO. 5 RENDITION AND PAYMENT OF BILLS A. (Continued) 9. Late Payment Charge A late payment charge of 1.5 percent applies to each customer's bill when the previous month's bill has not been paid in full, leaving an unpaid balance carried forward. The 1.5 percent charge is applied to the total unpaid amount carried forward and is included in the total amount due on the current bill.

47 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 18 RULE AND REGULATION No. 6 DISCONTINUANCE OF SERVICE A. TERMINATION OF SERVICE 1. Service may be terminated prior to the expiration of the initial contract period upon notice being given to the Telephone Company, and upon payment of the termination charges given below, in addition to all charges due for service which has been furnished. a. In the case of service for which the initial contract period is one month, the charges due for the balance of the initial month. b. In the case of directory listings and joint-user service where the listing has appeared in the directory, the charges due to the end of the directory period, except that in the following cases, charges will be continued only to the date of termination of the extra listing, subject, however, to a minimum charge for one month. (1) The contract for the main service is terminated. (2) The listed party becomes a customer to some class of exchange service. (3) The listed party moves to a new location. (4) The listed party dies.

(Continued) (d) Contracts for periods of longer than one month covering services whose installation required line extensions may be terminated upon payment of all charges that would accrue to the end of the contract period, or the contract will be transferred to a new applicant who is to occupy the same premises and will subscribe to the service effective on the day following termination by the original customer. 2. Service may be terminated after the expiration of the initial contract period, upon the Telephone Company being notified, and upon payment of all charges due to the date of termination of the service.

49 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 20 RULE AND REGULATION No. 7 NOTICES A. NOTICE FROM COMPANY Notices the Company may give to a customer supplied with telephone service by the Company, may be given to the customer or his authorized representative orally unless otherwise provided by these Rules and Regulations and the rules of the Public Utilities Commission of Nevada, or by written notice, either delivered at the customer's address hereinafter described or properly deposited in any United States Post Office, postage prepaid, addressed to the customer at the address specified in the customer's application for telephone service or at such address as may subsequently be given by the customer to the Company at its local business office. B. NOTICE FROM CUSTOMER Any notice from any customer to the Company may be given orally to the Company by the customer or his authorized representative at the Company's local business office where service is rendered to the customer unless otherwise provided by these Rules and Regulations, or by written notice properly addressed and mailed to the Company.

50 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 21 RULE AND REGULATION No. 8 CONSTRUCTION, MAINTENANCE AND USE OF FACILITIES A. GENERAL 1. Except as otherwise provided in this tariff, the Company will, at its own expense, furnish, install, and maintain in the Base Rate Area all facilities for basic access service necessary to serve applicants or customers in accordance with its lawful rates, rules and regulations, and in accordance with its established construction standards. 2. Except where designated by law, the type of construction (direct burial, underground conduit, or aerial) is the prerogative of the Company. 3. When the Company is requested by the customer to install initially, relocate, rearrange or change outside plant facilities from one type to another, the cost of constructing the new and removing the old construction shall be borne by the customer with consent of owner if applicable. 4. The Company has the right of ingress and egress from the premises of customers at all reasonable hours for any purpose reasonably connected with the furnishing of telephone service and to exercise any and all rights secured to it by law of these Rules and Regulations. The Company has the right to remove any and all of its property installed on the customer's premises at the termination of service as provided for in these Rules and Regulations. 5. All telephone equipment and apparatus furnished by the Company shall be carefully used and shall not be removed from the customer's premises except by an authorized representative from the Company. The customer will be held responsible for loss of or damage to any equipment or apparatus furnished by the Company, unless such loss or damage is due to causes beyond his control.

51 Tariff P.U.C.N. No. 1A Original Rule and Reg. Sheet No. 22 RULE AND REGULATION No. 8 CONSTRUCTION, MAINTENANCE AND USE OF FACILITIES B. OBLIGATION OF COMPANY 1. Furnishing of Service a. The Company's obligation to furnish service is dependent upon its ability to secure and retain, without unreasonable expense, suitable facilities and rights for the construction and maintenance of the necessary circuits and equipment. b. Where facilities beyond those normally required are provided to satisfy customer requests not specifically covered in this Tariff, charges based on the additional costs incurred will apply. c. When a customer orders installations, moves or changes which cannot be completed during scheduled working hours, he may be required to pay overtime charges. Such overtime charges will be the actual overtime rate of pay of the installer, and will be in addition to the normal installation, move or change charge. The customer must agree to this provision before such overtime work will be performed. d. When the construction of certain facilities is necessary for the furnishing of a service, the ownership of such facilities will be vested in the Company, even though all or a part of the cost of construction is borne by the customer. e. The Company will determine the type of facilities to be provided for the furnishing of a service. f. The Company will be reimbursed for the costs associated with customer requests for relocation or rearrangement of facilities.
