Original Page Service Expedites General The Company will accept orders requiring expedited installation intervals for review in the attempt to satisfy a Customer s request. A service order (non-recurring) Expedite Charge will be applied to each accelerated installation approved by the Company and accepted by the Customer. The Expedite Charge will be determined based upon the actual date of service delivery in relation to the standard provisioning interval. Due to resource allocation costs and charges incurred during the accelerated provisioning process, a minimum Expedite Charge may be assessed for service deliveries that occur outside the target interval. While the Company will make every effort to provide the requested accelerated service installation, the Company makes no guarantees that the request will be approved, or that any approved expedited installation intervals will be met Service Intervals All Standard Service Intervals (SSIs) are given after receipt of order (ARO) by the Company. Receipt of order is defined as the point in time at which a clean (complete) sales order package is transferred from the Company s Sales Order Verification Group to Order Entry for processing. The Sales Order Verification Team will notify the Customer s Account Team regarding incomplete orders at which time the Account Team may provide the required information or the package may be returned for additional documentation. Facility Type DDS, DS0 and DS-1 DS-3 OCn Standard Service Interval 35 Days 45 Days Determined on an Individual Case Basis ("ICB") Date Posted

Original Page Service Expedites (continued) Rates The Expedite Charge consists of two elements: (1) a flat amount which is applied when an expedite is processed and (ii) an amount based on the number of days that the service is delivered to Customer prior to the SSI. The first element of the Expedite Charge always applies for processing the Customer's request, regardless of whether or not there is an improvement over the SSI. The expedite charges indicated below will be applied for each DS-3 and below bandwidth Dedicated Access channel, Frame Relay or ATM port, and/or each circuit in a Private Line Service Order. The total charges for these expedites (DS-3 and below bandwidth) will not exceed $1, DDS, DS-0 and DS-1 Expedite Charge Per Access Circuit Facility Type: DDS, DS-0 and DS-1 Standard Interval: 35 Calendar Days Tier 1: Less than 35 or equal to 25 calendar days $200 Tier 2: Less than 25 calendar days $800 DS-3 Expedite Charge Per Access Circuit Facility Type: DDS, DS-0 and DS-1 Standard Interval: 45 Calendar Days Tier 1: Less than 45 or equal to 35 calendar days $200 Tier 2: Less than 35 calendar days $800 (C) OCn Expedite Charge Per Access Circuit Facility Type: DDS, DS-0 and DS-1 Individual Case Basis Standard Interval: ("ICB") - Subject to Network capacity Tier 1: No expedites. ICB Tier 2: No expedites. ICB

Original Page Service Cancellations If the Customer, either on behalf of itself or an Authorized User or End User, orders a BellSouth Long Dedicated Access channel, BellSouth Long Distance Private Line service, BellSouth Long Distance Frame Relay service or BellSouth Long Distance ATM service from the Company and later cancels the order before service begins, a charge shall be made to the Customer for such cancellation. These cancellation charges vary based upon the facility bandwidth ordered and/or whether an Access Service Request (ASR) has been issued by the Company. A Pre-ASR Charge will be applied when the Customer requests a service cancellation prior to the Company issuing the ASR. A Post-ASR Charge applies when the Company receives the cancellation request after issuance of the ASR for the particular facility. The cancellation charges indicated below will be applied for each Dedicated Access channel, for each Port in a Frame Relay Service Order, for each Port in an ATM Service Order and/or each circuit in a Private Line Service Order Rates Nonrecurring Charge Pre-ASR Cancellation Charges: Per DS-0 and DDS Type Channels, Ports or Circuits $ Per DS-1 Type Channels, Ports or Circuits $ Per DS-3 Type Channels, Ports or Circuits $1, Post-ASR Cancellation Charges: Per DS-0 and DDS Type Channels, Ports or Circuits $ Per DS-1 Type Channels, Ports or Circuits $ Per DS-3 Type Channels, Ports or Circuits $2, Telecommunications Service Priority (TSP) System Service Description The Telecommunications Service Priority (TSP) System is a structured coding scheme that prescribes the order in which National Security Emergency Preparedness (NSEP) telecommunications services are installed or restored. The TSP System was developed to support the requirements of the U. S. Government and applies only to NSEP telecommunications services to which the Company is able to apply priority treatment. It requires and authorizes priority action by the Company. Conditions of emergency or crises that cause invocation of NSEP treatment can only be declared by authorized officials of the Federal Government or other officials (Federal or non-federal) specified by the Manager - National Communications System (NCS) on behalf of the Executive Office of the President of the United States.

Original Page Telecommunications Service Priority (TSP) System (continued) Service Limitations (C) (D) (E) (F) Priority installation and/or restoration of NSEP telecommunications services shall be provided in accordance with Part , Appendix A, of the Federal Communications Commission's Rules and Regulations. In addition, TSP System service shall be provided in accordance with the current guidelines set forth in "Telecommunications Service Priority (TSP) System for National Security Emergency Preparedness (NSEP) Service User Manual" and current "Telecommunications Service Priority (TSP) System for National Security Emergency Preparedness (NSEP) Service Vendor Handbook". The customer for the TSP System service must also be the same customer for the underlying Exchange Service with which it is associated. The Company will arrange for the installation and/or restoration of TSP System service upon receipt of the proper certification as specified in A. preceding. It is the responsibility of the TSP user to provide the TSP Authority Code to the Company with each service request. When performing Priority Installation or Priority Restoration (repair) on TSP-designated services in compliance with the Rules and Regulations cited in A. preceding, the Company may not be in a position to notify the customer regarding additional labor charges if additional labor is required. The customer recognizes that quoting charges and obtaining permission to proceed with the installation or restoration of service may cause unnecessary delays and grants the Company the right to quote charges after the installation or restoration has been completed. Priority Installation or Repair: Rules and Regulations.1 When a customer for TSP System service requests that service be installed or repaired on an expedited or emergency basis in accordance with Part , Appendix A, of the Federal Communications Commission's Rules and Regulations, the customer will be required to bear the excess costs of providing service on an expedited basis..2 The calculated excess costs would be in addition to all other service and installation charges normally applicable. Under certain conditions, it may be necessary to preempt one or more customer services with a lower (or no) restoration priority in order to install or restore NSEP telecommunications service(s). If preemption is necessary and if circumstances permit, the Company will make every reasonable effort to notify the preempted customer of the action to be taken. No charge applies when a TSP designation is discontinued.

Original Page Telecommunications Service Priority (TSP) System (continued) Rules and Regulations (continued) (C) With the exception of credit information, a customer obtaining TSP System service acknowledges and consents to the provision of certain customer service details by the Company to the Federal Government to allow for the proper maintenance and administration of the TSP System. That information includes but is not necessarily limited to: TSP Specific Definitions.1 Confirmation of completed TSP service orders directly to the Manager, National Communications System (NCS);.2 Verification of installation and/or restoration priority level assignment(s) with the Manager, NCS;.3 Reconciliation of TSP service information with the Manager, NCS, or the customer (prime service vendor). National Communications System (NCS) - The NCS is established under the Executive Office of the President of the United States and is responsible for the day-to-day operations of the TSP System. This includes maintaining a twenty-four hour point-of-contact to handle emergency provisioning requests, assigning priority levels and Authorization Codes and maintaining data on TSP assignments. National Security Emergency Preparedness (NSEP) Services - NSEP services are telecommunications services that are used to maintain a state of readiness or to respond to and manage any events or crises (local, national or international) which causes or could cause injury or harm to the population, damage to or loss of property, or degrade or threaten the NSEP posture of the United States. Prime Vendor - The service vendor from whom the service user or its authorized agent orders service. Priority Installation (PI) - Provisioning, on a priority basis, of a new TSP service authorized as so urgent that it must be provided earlier than the Company's standard provisioning interval. Priority Restoration (PR) - Restoration, on a priority basis, of an existing TSP service for which any interruption would have serious adverse impact on the supported NSEP function. Subcontractor - The service vendor from whom the prime vendor obtains service for the completion of the prime vendor's end-to-end service. Telecommunications Service Priority (TSP) System - TSP is a structured coding scheme that establishes the order in which NSEP services are to be installed or restored in the event of an emergency. The TSP System was developed to ensure priority treatment of the nation's most important telecommunications services. TSP Authorization Code - A twelve character code that identifies an NSEP TSP service and denotes the order in which that service is to be provisioned (installed) and/or restored.

Original Page Telecommunications Service Priority (TSP) System (continued) TSP Rate Categories The following rate categories apply to TSP System service:.1 Priority Installation / Priority Restoration Set-Up / Development.2 Level Change. Maintenance/Administration Certain activities associated with the TSP System are included in the rate elements as follows: Rates and Charges TSP Service charges include order coordination, system development, verification and confirmation. The following TSP rates and charges are in addition to all other rates and charges that may be applicable for other services furnished in conjunction with TSP service: Nonrecurring Charges (NRC) NRC TSP Priority Provisioning and/or Restoration Service of a new IXC circuit without local access coordination (IXC only): $ TSP Priority Provisioning and/or Restoration Service local access coordination (Per local access circuit): $ TSP Priority Level Change Charge (Per Local Loop and/or IXC Circuit): $50.00 Monthly Recurring Charge (MRC) TSP Administrative/Maintenance Charge (Per Local Loop and/or IXC Circuit): MRC $20.00
