1 PBCS FOOD Bid December 15, 2016 Pre Bid Questions and Answers Due to a database error several items did not populate correctly. Please note Bid line 80 and now appear in the correct order. In addition, you will also find items that were split from groups or numbered incorrectly at the bottom of the pricing document. This was necessary in order to keep the same bid line numbers and not renumber the whole document. The original pricing document may still be submitted and the addendum changes will be applied once the bid is submitted. 1Q: Are Child Nutrition labels required for spices? 1A: No, but you must provide a nutrition facts label and ingredient statement. 2Q: Can you provide the current bid price for all 4 items on line 284 (fruit slush)? 2A: All prior year pricing can be found on the District s Website 3Q: The first document attached,additional Allowance Worksheet, only has to be completed if there's an additional allowance for a Commodity Item in addition to the PTV? 3A: Correct, you only need to complete the additional allowance worksheet if you are bidding a commodity item NOI+A and the commercial cost minus the commodity PTV does not equal the commodity bid price and an additional allowance is used to net down to the commodity price. 4Q: In reference to the 2nd document attached (Proposer's Statement of Principal Place of Business) this is a new document that we have never had to complete before. You asked for an , so you can get an answer as soon as possible. Can you please provide as much information for completing this document as possible. Who is the 'Proposer'...is it the broker sending in the bid or each individual Client/Manufacturer providing pricing for this bid? It involves Attorneys, so we greatly appreciate a response on this one as soon as possible. If necessary, I would have to send it to our Clients for their attorneys to complete. 4A: The Proposer would be the person that is submitting the bid. If you are a Florida company you do not need an attorney. This document is per a Florida Statute. The District is bound by the Statute to include this in each bid for general procurement. 5Q: Please confirm that the Excel pricing sheet is to be submitted only on CD or Flash drive & NOT through OnDemand/Onvia bid portal as previous years 5A: Correct, for the purpose the food bid, OnDemand/Onvia is only used as a way to notify registered vendors. The pricing document must be submitted on a CD or Flash drive and delivered to Sysco Southeast prior to the bid opening. 6Q: Can line read crumbles as well as diced, or add a line for crumbled alone? 6A: No, currently the diced is used for salads. New lines cannot be added during the bid process.

2 7Q: Can lines and be separated for Cargill new product? 7A: No, currently we do not pull enough of either item to warrant splitting the group. Meeting minimum quantities would be a concern to the district. 8Q: Can line be separated? 8A: No, the hot dog only pulls dark meat and by pairing it with the meatball we ensure a better utilization of our turkey allocation for processing. 9Q: Can line be separated? No mfg can meet the specifications of all the lines. Separate all line, group no antibiotic and 92.05, then separate the remaining 3 lines, as options. 9A:See Addendum 10Q: Based on a pre bid meeting there was discussion about adding a line item for the following products: BBQ Baked Beans, Sriracha Pinto Beans, Taco Sticks (4 flavors). Are these items going to be added to the bid for next school year? 10A: There was consideration for adding the above items. The district has decided not to add the BBQ Baked Beans and Sriracha Pinto Beans. See addendum for Taco Sticks. 11Q: Line Are you looking for a cheese filled breadstick? 100% or 50/50. Are you looking for it to be filled with sauce and pepperoni? 11A: We will accept 100% or blend cheese filled breadstick without sauce or pepperoni 12Q: Item #116 Bagels, Specification says : White whole Grain: Is whole wheat or whole grain flour acceptable. We are listed as an approved brand. We do not use White whole wheat flour. A: No, the district prefers a white wheat bagel product formulation. 13Q: Item #117 Bagels, Specification says : White whole Grain: Is whole wheat or whole grain flour acceptable. We are listed as an approved brand. We do not use White whole wheat flour. A: No, the district prefers a white wheat bagel product formulation. 14Q: tem # 218 Hoagie sub roll. Bid specification requests Hearth baked. We use rack rotary ovens. Since the bid has only one approved brand can we submit a sample for approval. Bid specification: 6 length. What is the width?. Also, approved Brand Sky Blue website does not describe sub roll as Hearth baked. Please confirm. A: If you bid this line and are the lowest price, a sample will be requested. We do not specify the width. 15Q: Line 125 Pullman loaf. The specification says 24 slices, O Pelle has 28 slices. Is that acceptable to bid? 15A: Yes, the specification asks for a minimum of 24 slices per loaf. Any size greater than 24 will accepted. 16Q: We don't show that column P has a drop down box. Should column P have a drop down box? If so; can we get an updated pricing document. 16A: A new electronic pricing document is posted on the website.

3 17Q: Bid line ;.02 Grilled Cheese The specification does not state bulk or IW. Can it be bid bulk and/or IW. 17A: The product is intended to be bulk packed. See Addendum 18Q: Does the Proposer's Statement of Principal Place of Business need to be filled out and returned for each manufacturer submitting pricing? 18A: If you are a broker, bidding on behalf of manufacturers, only the broker needs completes the document. 19Q: Bid line ;.02;.03 Kellogg s discontinued the Sea Salt flavor in November. This is represented by A: See Addendum 20Q:Bid line ;.02;.03 Cheese Crackers. Kellogg s discontinued the Taco flavor due to low volume in September. We now only have 2 flavors (original and atomic). 20A: See Addendum 21Q: Bid line fruit snack has the same flavor as Can be changed to assorted fruit? 21A: See Addendum 22Q: Line 75.01/.02 BBQ cups previous spec read max of 225 mg sodium, this spec is 100 mg. can the sodium be edited to read max 220 mg? we cannot bid on this item at the 100 mg level. 22A: See Addendum 23Q: Line 76.03/.04 marinara cups can this item be ungrouped and stand as own line? we cannot bid on this item with the CN requirement. 23 A: See Addendum 24Q: Line /.02 pudding cups can the fiber content requirement be removed? 24A: See Addendum 25Q: Line 112 Yogurt Bulk, Vanilla Blended lowfat or fat free. 4 29Q: Line 163 it does not appear that all of the listed brands would meet the sodium specifications for all 4 items. Could an addendum be considered to add 15 mg of sodium to all 4 sub categories? 29A: See Addendum 30Q: Line 447 If 2 of her portions meet 2 grain and the rest of the nutritionals match will you accept? 30A: No, the 2 grain component needs to be in one package. 31Q: Question 1. If the brand we want to bid meets the specifications, but is not listed and is low bid,will you award that item to that company, assuming of course that the item meets the specifications noted in the bid. 31A: If the unit price is lowest and the product meets the physical specifications we will request a sample for testing. If the product is considered an acceptable equal, then an award would be granted to this product. 32Q: Bid line 236 If the tortilla exceeds the specification, specifically a 10 inch diameter verses a 9 inch tortilla will it be accepted if it is low bid per tortilla. 32A: Yes, we will accept a 10 inch tortilla if all other criteria for the product meets. 33Q: Bid line 434 you state acceptable equal: and then list brands. If the brand we want to bid meets the specifications, but is not listed can it be awarded to that company based on low bid even though the brand is not listed? 33A: If the unit price is lowest and the product meets the physical specifications we will request a sample for testing. If the product is considered an acceptable equal, then an award would be granted to this product. 34Q: Is the NOI + additional allowance worksheet to be completed and submitted only if the manufacturer offers an additional allowance? 34A: Correct, you only need to complete the additional allowance worksheet if you are bidding a commodity item NOI+A and the commercial cost minus the commodity PTV does not equal the commodity bid price and an additional allowance is used to net down to the commodity price. Q: Does the Proposer's Statement of Principal Place of Business need to be completed for this food bid? A: Yes, if you are a broker bidding on behalf of manufacturers, only the broker needs to complete the document. Q: Please break out line items and.02 into two separate lines because one item is a mini corn dog and one item is a chicken nugget which are supplied by separate manufacturers. A: See addendum Q: IW cookie mentions product needs to be made in a nut free facility. The dough doesn't state this. Seems to be inconsistent, I know Otis, David's, Bonzers, etc. do not operate nut free facilities. We also do not, but obviously follow strict cleaning and safety standards that I am happy to provide. I am questioning if the baked cookie spec can be made similar to the cookie dough spec and eliminate the peanut free language.
