I. Base Bidding Requirements This is a Base Bid project; the following describes the Base Bid requirements. Contractors are to provide bids for Base Bid #1. A. BASE BID #1: II. Work Included The following items are site specific requirements of the project. A. Contractor to repair, replace and install the necessary materials to complete the work identified below per each building identified in the Base Bid requirements. B. Contractor to engage the services of a structural engineer for design and installation guidance. C. Select demolition of existing bracing material, excavation for foundation work, and existing concrete foundation. D. Replacement of the building foundation to include epoxy coated reinforcing bar. E. Installation of new Knee Brace. F. Backfill and Paving to existing grades. III. GENERAL A. The Contractor shall provide all labor, materials, apparatus, trucking, tools, superintendence, and services required for completion of the work as described on the Statement of Work, Specifications, and Drawings. B. The Contractor is responsible for obtaining all necessary Permits from the Department of Labor and Industry (L&I) (http://www.dli.state.pa.us/), there are no fees for the Permits as long as the contractor notifies the L&I that this project is for the Commonwealth of Pennsylvania, PennDOT and is to be completed on state owned or state leased property. 1. Contractor shall provide all design, calculations, and final plans sealed by a Professional Engineer licensed in the state of Pennsylvania, as required. 2. This building does not have an occupancy permit. The Contractor will be responsible to obtain the occupancy permit for this building as part of the project. Page 1 of 7
C. Contractor shall maintain and log, all mail receipts regarding L&I correspondence and submittals, and provide copies to PennDOT upon request. D. Contractor to confirm all dimensions and quantities before bidding. Any stated dimensions/quantities are estimates only. IV SELECT DEMOLITION A. Remove damaged treated wood framing materials. B. Saw cut existing asphalt paving. C. Excavate to expose the foundation. D. Demolish the foundation concrete to expose the embedded wood framing members. 1) Contractor to legally dispose of all construction debris. V. MATERIALS A. New treated wood framing member(s) to be pressure treated for direct ground contact. B. Wood framing member(s) shall be solid sawn timbers or glue laminated timbers specified for outdoor exposure. C. Concrete to be 5000 PSI. D. Reinforcing bar to be grade 60, epoxy coated deformed round bars. E. Fasteners to be hot dipped galvanized. F. Asphalt paving to be hot mixed 19mm. G. Pipe Bollards shall be eight inch (8 ) schedule 40 galvanized steel pipe, eight feet (8 ) total length. VI. INSTALLATION A. Install new wood framing member(s) to the lines and angles as the existing framing member(s). B. Cross drill the embedded end of the timber to accept a # 6 X 16 (minimum) epoxy coated rebar. C. Install epoxy coated reinforcing bars as directed by engineer of record. D. Place new concrete so as to embed the new timber framing member(s) to a depth of the existing timber. E. Backfill excavation with 2A stone in compacted lifts in no more than eight inch (8 ) depths. F. Pave the area with 19mm hot asphalt paving to meet existing lines and grades. G. Seal all asphalt joints with liquid asphalt sealer. H. Install four (4) pipe bollards in front of the building. Page 2 of 7
I. Install a pipe bollard at the outer most corners of the building and at both door edges of the main door opening. J. Set pipe bollards one foot six inches (1-6 ) out from the face of the building, inline from side to side. K. Pipe bollards to be installed with three feet (3 ) embedded in concrete, in the ground and five feet (5 ) exposed. (see attached drawing) L. Pipe bollards shall be filled with concrete. M. Pipe bollards shall receive a Safety Yellow PVC sleeve with Red & White reflective tape. VII. SUBMITTALS AND FINAL DELIVERABLE ITEMS A. Submittals and final Deliverable items 1. The Department will provide the awarded contractor a complete breakdown of the required submittals based off of the System identified, I.E. Roofing System 2. Contractor to provide three (3) copies of the entire project documentation in three (3) ring Binders. 3. Binders to include the following items: a. Bid Documents & Specifications b. All project meeting minutes (pre-bid, pre-job, Interim, and Final inspection) c. Email Correspondence d. Submittals e. L&I approved Engineered Drawings (Paper/CD) f. L&I Building Permits, Inspection Logs, Occupancy Permit, Other correspondence g. Certifications h. Certified Payrolls (Prevailing Wage rates) if applicable i. Waste receipts j. Photos Before, during and after 1) A sufficient amount of Photos is required for all phases of construction on all buildings, base on award. 2) Building Final-Exterior all four sides k. Operation and Maintenance manuals l. Contractors Warranty m. Manufactures Warranty VIII. SUBMITTAL PROCEDURE Page 3 of 7
A. Submit Two (2) copies of all items shown on submittal schedule except for "samples". B. Identify project number, project name and location, and contractor's name and address on all submittals. C. Provide space for review stamps of Contractor and Department. Put contractor stamp of approval on each item submitted. D. Revise and resubmit submittals as required, identify all changes made since previous submittal. E. No work shall be performed until all submittals are approved by the Department. Work performed prior to submittal approval is performed at the Contractor's risk and may be subject to repair, replacement or other corrective measures as deemed necessary by the Department with no additional contract time or money. F. Review of shop drawings will be general and for the limited purpose of checking for general conformance with the design; and shall not relieve the Contractor from the responsibility for proper fitting and construction of the work, nor from furnishing materials and work required by the Contract which may not be indicated on the shop drawings when reviewed. G. The submittals required by this contract are subject to reproduction for the Department's internal use only. By signature on this bid proposal, both the Contractor and the equipment manufacturer authorize the Department to reproduce any materials including printed literature and training aids, submitted in fulfillment of this contract. IX. MANUFACTURER'S INSTRUCTIONS A. When specified in individual specification sections, submit manufacturer's printed instructions for delivery, storage, assembly, installation, start-up, adjusting and finishing, and maintenance, in quantities specified for product data. B. Identify conflicts between manufacturer's instructions and contract documents. X. MANUFACTURER'S CERTIFICATES A. When specified in individual specification sections or on submittal schedule, submit manufacturer's certificates to for review. Page 4 of 7
B. Indicate that material or product conforms to or exceeds specified requirements. Submit supporting reference date, affidavits and certifications as appropriate. C. Certificates may be recent or previous test results on material or product, but in all cases must be acceptable to Department. XI. SUBMITTAL SCHEDULE A.
