FACILITIES MANAGEMENT Housekeeping Building 1 P age Addendum #2 Date: April 8, 2019 Bid Date: Sealed bid for this work will be received by: Mr. Christopher Placco, University Architect in the office of the University of North Carolina School of the Art s Facilities Management Office, 1533 South Main Street, Winston-Salem, North Carolina 27127, up to 2:00 PM, April 16, 2019 and immediately thereafter publicly opened and read for the furnishing of labor, material and equipment entering into the University of North Carolina School of the Art s Housekeeping Building. This addendum is issued prior to the receipt of bid and does hereby become a part of the Drawings and Specifications in case of conflict. The Bidder shall be responsible for issuing information contained herein to the appropriate subcontractors. The Following is Addendum #2 to the Drawings and Specifications for the above-mentioned project. This is the form that shall be used at the Bid Opening. 01230 Alternates Additive Alternate #5 Interior Waterproofing: Add an interior latex-based masonry waterproofing paint, extending four (4) foot above the finished floor line, on the existing masonry interior wall (Northeast) separating the Housekeeping Building with the adjacent building. Extend waterproofing five (5) feet down walls intersecting the primary wall. Waterproof paint should be non-hazardous and mildew resistant. Additive Alternate #6 Roof Insulation: Add R-30 Batt Insulation between the existing roof joist. The undersigned, as bidder, hereby declares that the only person or persons interested in this proposal as principal or principals is or are named herein and that no other person than herein mentioned has any interest in this proposal or in the contract to be entered into; that this proposal is made without connection with any other person, company or parties making a bid or proposal; and that it is in all respects fair and in good faith without collusion or fraud. MINORITY BUSINESS PARTICIPATION REQUIREMENTS Provide with the bid - Under GS 143-128.2(c) the undersigned bidder shall identify on its bid (Identification of Minority Business Participation Form) the minority businesses that it will use on the project with the total dollar value of the bids that will be performed by the minority businesses. Also list the good faith efforts (Affidavit A) made to solicit minority participation in the bid effort. NOTE: A contractor that performs all of the work with its own workforce may submit an Affidavit (B) to that effect in lieu of Affidavit (A) required above. The MB Participation Form must still be submitted even if there is zero participation. After the bid opening - The Owner will consider all bids and alternates and determine the lowest responsible, responsive bidder. Upon notification of being the apparent low bidder, the bidder shall then file within 72 hours of the notification of being the apparent lowest bidder, the following: An Affidavit (C) that includes a description of the portion of work to be executed by minority businesses, expressed as a percentage of the total contract price, which is equal to or more than the 10% goal established. This affidavit shall give rise to the presumption that the bidder has made the required good faith effort and Affidavit D is not necessary; * OR * If less than the 10% goal, Affidavit (D) of its good faith effort to meet the goal shall be provided. The document must include evidence of all good faith efforts that were implemented, including any advertisements, solicitations and other specific actions demonstrating recruitment and selection of minority businesses for participation in the contract. Note: Bidders must always submit with their bid the Identification of Minority Business Participation Form listing all MB contractors, vendors and suppliers that will be used. If there is no MB participation, then enter none or zero on the form. Affidavit A or Affidavit B, as applicable, also must be submitted with the bid. Fire-Resistance Characteristics: Where indicated, provide acoustical panel ceilings identical to those of assemblies tested for fire resistance per ASTM E 119 by UL or another testing and inspecting agency acceptable to authorities having jurisdiction. a. Identify materials with appropriate markings of applicable testing and inspecting agency. 2. Surface-Burning Characteristics: Acoustical panels complying with ASTM E 1264 for Class A materials, when tested per ASTM E 84. a. Smoke-Developed Index: 450 or less. C. Acoustical Ceiling Panels: Full-size panels equal to 2.0 percent of quantity installed. 2. Suspension System Components: Quantity of each exposed component equal to 2.0 percent of quantity installed. PART 2 - PRODUCTS 2.1, GENERAL A. Acoustical Panel Standard: Comply with ASTM E 1264. B. Metal Suspension System Standard: Comply with ASTM C 635. C. Attachment Devices: Size for five times the design load indicated in ASTM C 635, Table 1, "Direct Hung," unless otherwise indicated. Comply with seismic design requirements. 1. Anchors in Concrete: Expansion anchors fabricated from corrosion-resistant materials, with holes or loops for attaching hangers of type indicated and with capability to sustain, without failure, a load equal to five times that imposed by ceiling construction, as determined by testing per ASTM E 488 or ASTM E 1512 as applicable, conducted by a qualified testing and inspecting agency. 2. Power-Actuated Fasteners in Concrete: Fastener system of type suitable for application indicated, fabricated from corrosion-resistant materials, with clips or other accessory devices for attaching hangers of type indicated, and with capability to sustain, without failure, a load equal to 10 times that imposed by ceiling construction, as determined by testing per ASTM E 1190, conducted by a qualified testing and inspecting agency. 09511-1 P age

D. Wire Hangers, Braces, and Ties: Zinc-coated carbon-steel wire; ASTM A 641/A 641M, Class 1 zinc coating, soft temper. 1. Size: Select wire diameter so its stress at 3 times hanger design load (ASTM C 635, Table 1, "Direct Hung") will be less than yield stress of wire but provide not less than 0.106-inch-diameter wire. E. Seismic perimeter stabilizer bars, seismic struts, and seismic clips. F. Cap Finish: Painted white. PART 3 - EXECUTION 3.1 INSTALLATION A. Comply with ASTM C 636 and seismic design requirements indicated, per manufacturer's written instructions and CISCA's "Ceiling Systems Handbook." 09511-2 P age

B. Measure each ceiling area and establish layout of acoustical panels to balance border widths at opposite edges of each ceiling. Avoid using lessthan-half-width panels at borders. C. Suspend ceiling hangers from building's structural members, plumb and free from contact with insulation or other objects within ceiling plenum. Splay hangers only where required and, if permitted with fire-resistance-rated ceilings, to miss obstructions; offset resulting horizontal forces by bracing, countersplaying, or other equally effective means. Where width of ducts and other construction within ceiling plenum produces hanger spacings that interfere with location of hangers, use trapezes or equivalent devices. When steel framing does not permit installation of hanger wires at spacing required, install carrying channels or other supplemental support for attachment of hanger wires. 1. Do not support ceilings directly from permanent metal forms or floor deck; anchor into concrete slabs. 2. Do not attach hangers to steel deck tabs. D. Install edge moldings and trim of type indicated at perimeter of acoustical ceiling area and where necessary to conceal edges of acoustical panels. Screw attach moldings to substrate at intervals not more than 16 inches o.c. and not more than 3 inches from ends, leveling with ceiling suspension system to a tolerance of 1/8 inch in 12 feet. Miter corners accurately and connect securely. E. Install suspension system runners so they are square and securely interlocked with one another. Remove and replace dented, bent, or kinked members. Latching: 4,5,6 Tier: An 11-gauge frame hook shall be secured to the fame. The frame shall have a padlock hasp protruding through the recessed handle. A rubber silencer shall be firmly secured to the frame at each latch hook. 15. A rubber silencer shall be firmly secured to the frame at each latch hook. 16. Interior Equipment: a. Knocked Down: All locker components shall be assembled with nuts and bolts. 2.4 LOCKER ACCESSORIES A. Metal Locker Bases: 1. Base: Zee base shall be 14-gauge sheet steel, powder coated to match the color of the lockers. 2. Base: Front base/closed end base shall be 18-gauge sheet steel, powder coated to match the color of the lockers. Front bases shall be installed between the front legs without overlap or exposes fasteners. End bases shall be installed between front and rear legs of lockers at end of a row. B. Metal Locker Fillers-Vertical: Fillers shall be 20-gauge sheet steel, powder coated to match the color of the lockers. 1. Filler Width: 6 inches (152 mm). 2. Filler Width: 12 inches (305 mm). C. Metal Locker Recess Trim: Recess trim shall be 18-gauge sheet steel, powder coated to match the color of the lockers. 1. PART 3 - EXECUTION 3.1 EXAMINATION A. Do not begin installation until substrates and bases have been properly prepared. B. If substrate and bases are the responsibility of another installer, notify Architect of unsatisfactory preparation before proceeding. 3.2 INSTALLATION A. Anchor lockers to floor and wall at 48 inches (1.219 m) or less, as recommended by the manufacturer. D. Fasten adjoining locker units together to provide rigid installation. E. Install front bases between legs without overlap or exposed fasteners. Provide end bases on exposed ends. 3.4 ADJUSTING AND CLEANING A. Adjust doors and latches to operate without binding. Verify that latches are operating satisfactorily. B. Touch-up factory-finish and repair or replace damaged products before Substantial Completion. 3.5 PROTECTION A.
