STEM OPT Responsibilities Form All applicants for STEM OPT must ensure they have met all Immigration-mandated responsibilities to be sure they have maintained their visa status while on Regular OPT. This form will help you be aware of what you must do once you ve been recommended for STEM OPT by VISA. Please INITIAL EACH SECTION in the space provided to indicate your understanding of the items below. This form is to be submitted with your STEM OPT application. Any changes to the following policies and regulations will be announced on the ISSlist-l listserv to which all students on ODU I-20s must belong and/or placed on the STEM OPT page. While on STEM OPT (24-month authorization), I understand that: 1. I understand that travel outside the U.S. while my STEM OPT application is being processed by Immigration is strongly discouraged and that re-entry while it is pending may cause problems. 2. I will submit the OPT Employment Update form to VISA every six months AND/OR if there are any changes in my employment. I understand that Immigration will terminate my SEVIS record if I fail to do this. 3. I will notify VISA of any information updates while I am on OPT (e.g. name change, address changes, etc.) via the proper forms on the VISA web site. 4. I will keep my passport valid for at least six months into the future for the duration of my OPT. 5. I am submitting my STEM application to VISA 90-120 days before my Regular OPT expires. If not, I am aware that VISA may not being able to process my application in time. 6. If I leave and return to the U.S. after receiving my STEM EAD in the mail, I must be returning to begin or resume a job upon re-entry. I will have proof of my employment, my STEM EAD and my I-20 (with valid travel signature) in order to re-enter the U.S. Information about travel on OPT can be found on the TRAVEL REMINDERS on the RESOURCES page on the VISA site. 7. It takes up to three business days for a travel signature from VISA and that I can mail my I-20 to VISA for the signature (allowing for mailing time in addition to the three business days). 8. My STEM OPT will be terminated once my SEVIS record is transferred from ODU to another school and/or if an I-20 issued for me to begin another program. I will communicate with the international student advisor issuing my I-20 to coordinate the issuance date. 9. I can work anywhere, as long as my work is directly related to my major, is at least 20 hours per week and the employer is e-verified. If I change my job or become unemployed, I understand that I must notify VISA via the OPT Employment Update Form on the VISA site within five days. If there are no changes in my employment, I will still do the updates, as required, every six months. Between Regular and STEM OPT, I have a total 120-day period of unemployment. 10. Upon issuance of my STEM OPT I-20, VISA will send me an e-mail with instructions on what I must do with my application. In that e- mail, there will be specific requirements explained and I understand that my failure to follow them can/will result in the termination of my SEVIS record by Immigration. 11. I can register for one or two classes per semester for recreational or professional development reasons as a nondegree student. 12. Having health insurance is not required by immigration but is STRONGLY recommended when I am doing OPT and that I am still eligible for the ODU health insurance. 13. I understand that the Affordable Care Act requires tax residents to purchase a health insurance plan that meets specified minimum essential coverage. Individuals not covered may have to pay a tax penalty. 14. I am required to keep all I-20s (current and prior) in a safe place forever. They will need to be submitted again if I request another type of immigration benefit or a change of status in the future. 15. I will send a black and white PDF of my STEM EAD to VISA, as well as notification (I-797 approval notice or new visa stamp) of any visa status changes within five days of updates. 16.
