TO: All Prospective Bidders SUBJECT: Addendum #2 DATE: April 8, 2013 PROJECT: Huntley High School Athletic Field Improvements Bid Release #2 OWNER: Consolidated School District #158 Algonquin, IL CONSTRUCTION MANAGER: ARCHITECT: Lamp Incorporated Elgin, IL Wold Architects and Engineers Palatine, IL This addendum forms a part of the Bidding and Contract Documents, and modified the original bidding documents dated March 22, 2013. Acknowledge receipt of this addendum in the space provided on the Bid Form. FAILURE TO ACKNOWLEDGE RECEIPT OF THIS ADDENDUM ON THE BID FORM MAY DISQUALIFY YOUR BID. GENERAL INFORMATION: Bids for the project remain due by Wednesday, April 10, 2013, at 3:00 pm. Contractors MUST review all front end documents to be sure that they are compliant with ALL requirements. If you have questions on the requirements, please contact MComiskey@lampinc.net. Failure to comply with documents will be considered a bid default. Bid Forms were issued in Addendum #1. Refer to your scopes of work for Unit Prices that will be required for each Trade Contractor. Qualification forms are due to Lamp Inc. s office by April 8 th. Please contact Mary Comiskey to see if yours needs to be updated. The Project Schedule and Site Staging Diagram were issued in Addendum #1. Please review these documents carefully, and price your bid accordingly.

WORK SCOPE CHANGES: General Work Scope Items for all Trade Packages: Add Line Item 52: Trade Contractors will not be permitted to subcontract more than 90% of their contracted work for this project. Excavation 02B-2: Line Item 19: Change to read; This Contractor shall provide the temporary construction entrance and access driveway according to the Contract Documents. When directed by the Construction Manager, this Contractor shall remove the temporary construction access driveway. The temporary construction entrance/gravel path and gate (Project Notes 43 and 44 on Sheet C3.0) are scheduled to remain, provide the additional CA-6 to the entrance when directed by the Construction Manager. Refer to Sheets C3.0 and C6.0 for details. Line Item 31: Add to the end; Also provide all final as-built drawings in PDF Format. Alternate #3 Safewater Hydrants: Add to the end; Provide SafeWater Hydrant Model M-NPL75 ¾ Freeze Resistant Post Hydrant Non-Pollutable, Lever-Operated. See attached specification sheet for more information. Electrical 16A-2: Add Line Item 18: This Contractor shall provide arc flash hazard assessment and labeling for all revisions to the electrical system, include updating schematic drawings, electronic labels, and hard copy labels. Add Line Item 19: Provide a complete installation of the transformer to the new bleacher system. Include all structural support or attachment as necessary. Add Line Item 20: This Contractor shall note that the Owner will be performing an infrared scan of all new electrical distribution as part of their punch-list inspection. Plan Note #9: Add to the end; The (4) 100A feeds come up from underground into the bottom of existing panel AB-1. The note is to be located pointing towards existing panel AB-1 on the Amenities Building Plan. The feeds would be intercepted either under the panel, or underground inside the room. 2. MURDOCK TM SafeWater Hydrant MODEL M-NPL75 3/4" FREEZE RESISTANT POST HYDRANT NON-POLLUTABLE, LEVER-OPERATED STANDARD FEATURES Model M-NPL75 delivers year-round service outside or in unheated buildings. The control valve is located below the frost line. Raising the lever opens the valve and lowering the lever shuts water off. The lever may be locked closed with a padlock (not included). After use, water inside the supply column is stored in a reservoir below the frost line and is expelled thru the nozzle at the beginning of the next usage. This potable hydrant is protected against any surface or underground contamination. Recommended maximum operating pressure is 150 psi. SUGGESTED SPECIFICATIONS Hydrant shall be Murdock-Super Secur model M-NPL75. Lever-operated hydrant shall be ASSE 1057 certified as a potable hydrant. Hydrant shall not operate using venturi action. Unit shall be capable of year-round use in freezing weather. Hydrant shall have a flow rate of 4.5 gpm minimum at 25 psi. Hydrant shall be serviceable from above grade. Unit shall extend below grade level so that supply inlet, valve and reservoir are positioned below frost line. During shutoff, a piston shall draw water from the supply column into a reservoir. An air bypass shall be mounted in nozzle to aid evacuation of supply column. At the next usage, the piston shall expel the water in the reservoir thru the nozzle before fresh water flows. The water supply shall be protected against any surface or underground contamination. Stock, top, base and handle shall be fabricated of heavy, one-piece iron castings finished with a heavy grade of oil-based green enamel. Solid brass castings in nozzle and inner supply assembly shall conform to ASTM standards B61 and B62. Lead-free castings shall be used in all waterways. MURDOCK INSTALLATION INSTRUCTIONS FOR HYDRANT M-NPL75 Prepare trench for water-supply line. Below hydrant location, prepare hole to trench depth and large enough for a person to work. Lay water-supply line into trench. Install hydrant fully assembled as shipped. Position hydrant at desired location by laying two boards across hole, one on either side of outer casing, and brace as necessary under hydrant stock. To ensure plumb installation, place a bubble level against hydrant and adjust bracing boards until unit is plumb. Purge water-supply line. Connect water-supply line, adding fittings as necessary. Back-fill trench and hole. Compact back-filled earth. Remove bracing boards. If necessary, loosen set screw in base and adjust vertical position of base on outer casing so that bottom of flared part of base is at grade level. If concrete slab is desired, prepare hole surrounding hydrant to accommodate concrete slab. Spread and compact gravel as necessary. Pour concrete up to bottom of flared part of hydrant base. (female) supply connection Supply line with fittings as required (by others) MAINTENANCE INSTRUCTIONS FOR HYDRANT M-NPL75 All normal maintenance is done from above grade. O-rings and washers should normally be replaced annually. A repair kit including these items is available from the factory. A T-handle wrench is necessary and is also available from the factory. Hydrant handle must turn 180 without resistance to turn on water. If handle meets resistance toward end of stroke or does not turn on water, turnbuckle needs to be adjusted. To replace O-rings and washers, shut off water supply. Remove nuts and bolts securing top to stock. Unscrew and remove nozzle from stock. Remove entire assembly from stock, keeping all parts in same alignment as removed so that turnbuckle adjustment is not changed. Using T-handle wrench, remove valve from bottom of hydrant housing. Replace seals as follows: Piston 1 small O-ring 2 large O-rings body 1 O-ring stem 1 tapered washer Lever Top Access plate Crank Turnbuckle Tee Stock Slide Retainer Nozzle Lubricate O-rings with FDA-approved, food-grade grease. Reassemble supply-line assembly and valve assembly. Replace valve in bottom of hydrant housing. Lower supply-line assembly carefully into stock. As assembly approaches operating position, plunger must be inserted gently into valve body. Replace slide and nozzle. Replace nuts and bolts securing top to stock. Reconnect turnbuckle to crank.
