Why Safety Management? Senior management were too easily satisfied that the system was being operated correctly, relying on the absence of any feedback of problems as indicating that all was well Piper Alpha Oil Platform Disaster (6 Jul 1988) It is not enough to talk in terms of absolute safety and of zero accidents. there must also be proper organisation to ensure that actions line up to work Clapham Junction Rail Crash (12 Dec 1988)
What is a Safety Management System? It s a management decision making tool Identifies safety issues (hazards) Assesses the risks Takes action to minimise the risk Assigns ownership of actions and the risk Checks that the actions taken are working as expected It s not a document its what you do!
Safety Management System The management system should be proportionate to the size, nature and complexity of the activities, and the hazards and associated risks inherent in these activities 10
What is your organisational culture? Is there a just culture? Do your people know what to report? Are they reporting the right things? What attitudes and behaviours do you expect of your people? What attitudes and behaviours do you expect your supervisors and managers to encourage? Do you assess your organisational culture?
Composite materials? External damage detected but not considered worth further investigation aircraft returned to service
internal damage found during next base check
Implementing SMS Invest wisely in your SMS and you will get a return Try and do it on the cheap and you probably won t! Communication is key; both internally and externally.with your customers and contractors and with other operators. Safety Management requires collaboration The more you share and collaborate the more you learn.so everyone wins! Don t forget we are all on a journey
Need To Keep SMS Simple "Any intelligent fool can make things bigger, more complex, and more violent. It takes a touch of genius and a lot of courage to move in the opposite direction." Albert Einstein
Capability Authority evaluation of an SMS An SMS evaluation tool needs to assess compliance and performance. Initially focusing on whether the elements are Present and Suitable Then assesses for Operating and Effectiveness Looks for objective evidence show me It will be Subjective Regulators need to recognise an SMS takes time to mature The SMS Journey Best Practice and towards excellence An effective SMS Operating and Effective Present and Suitable A Time
As the Industry Continues to Evolve How do we Continue to Provide Safety Assurance?
State Safety Programme (SSP) Considerations Criticality of Aviation to the State Meeting International Standards Safety Governance and Leadership Influencing beyond our remit Acceptable Level of Safety Performance Focus on Risk to the consumer
The Future More aircraft in finite airspace Better use of emerging technologies More collaboration More data exchange More working in partnership with other states to improve operational safety performance Integrating proactive solutions to emerging problems into the existing safety framework
Performance Based Regulation and Oversight Audits Internal Review Meeting Agreed Actions and Timescales Accountable Manager Meeting
