Instructions for the trainee Your answers to the assessment tasks need to show the assessor that you have a full understanding of the topic. The assessor may require you to discuss your responses. This assessment is open book, which means that you can use any information you wish when you are completing this assessment. This information may include learning materials, books, the internet and your organisation s policies and procedures. Your answers can be written or spoken. When you write your answer, use pen in the space provided. Initial any alterations you make. Please write your name on any additional pages and attach them to the assessment. When you answer verbally, the assessor will make full notes of your answer in the space provided in this assessment or may use a digital voice recorder. You need to show that your work complies with your organisation's policies and procedures. If you require assistance with any aspect of the assessment, please contact your assessor. Definitions of terms The word trainee in this assessment refers to the person being assessed. Other terms that may be used are candidate, student or employee. Person refers to a person accessing services in a health or disability setting in either a residential care facility or in a private home the person s own or a friend s, group s or family member s. Other terms used for the person being supported include client, consumer, individual, resident, service user or tūroro. Workplace assessor or assessor means the person who will assess the trainee. A verifier is a workplace supervisor or manager who understands the assessment, works closely with the trainee, and can confirm that the trainee has performed the task competently. Human lifespan stages refers to a chronological classification of people from birth to old age into a range of age groups, for the purposes of determining the defining features, capabilities, and developmental changes (psychological, physical, emotional, cognitive, psychosocial) associated with each age group. Wellness is a dynamic process whereby a person becomes aware of, and makes conscious choices towards achieving, a balanced and healthy lifestyle. This process includes learning new life skills that address both the positive and negative aspects of human existence. Abbreviations ER refers to the evidence requirements of the unit standard the evidence (descriptions, explanations, documents etc) that you must provide and/or the actions that you must do or demonstrate. Task Two Describe interventions, care, and support for people at different human lifespan stages (written or spoken answer) ER 1.2 In this task, you will be assessed for your ability to describe one example of each of interventions, care, and support for people at each of the following human lifespan stages: 1 infant (a person aged between 0 and 23 months). 2 child (a person aged between 2 and 18 years). 3 adult (a person aged between 19 and 64 years). 4 older person (a person aged over 65 years). You must base your answers on the information in the references (page 3 of this assessment guide), and record which references you have used. You can use examples you are familiar with in your work or personal life, or you can invent your own scenarios. You must provide at least one indicator of wellness in each of your examples, and match them to intervention(s), care, and support. Sample answer: An older person, Mr B, has been a relatively heavy smoker for all of his life since adolescence. He is showing the following indicators of wellness: Breathlessness and exhaustion in completing tasks such as gardening, and chopping and carrying firewood into his house. He has been diagnosed with emphysema, related to his history of smoking. He has made a decision to immediately stop smoking. Intervention (decided on with Mr B): Development of a stop-smoking plan with Mr B, including discussing relapse prevention strategies with Mr B, and whether stop-smoking medication might help him and whether Mr B wants to try it; if so, which kind, how to use it, and where to find the cheapest source of supply. (May include referral to a stop smoking service for free medication; or for support by a smoking cessation worker, if this is outside the support worker's job description or level of expertise.) Care (discussed/decided with Mr B): Acknowledging the wisdom of Mr B's decision to stop smoking, and the courage to do so; reinforcing and supporting Mr B's decision; and making it clear that I am available to support him in his stop-smoking strategies.
