Scenario C-SS-1, Construction Vessel/Subsea Operation, number 1 Assessee Copy: Assessment Narrative: A [DP-2 / DP-3] [OSV / MPSV] has embarked a team of ROV operators in order to take core samples at an identified GPS location [XX-XX N, XXX-XX W], 300 meters away from the vessel s current position. The mission of the vessel is to support the ROV team in acquiring the necessary core samples requested by the oil major for the risk assessment of an exploration project. The ROV will embark and disembark from the Starboard side, mid-ship. The client generally follows the operations guidance put forth by the MTS DP Committee. Any documents related to this guidance, such as ASOGs, CAM, etc.; or other guidance, such as standing orders shall be provided to the Assessee prior to the start of the operation. At the start of the operation, there is a [10-20 knot] wind from the [NE E] and the current is [0.8-1.8] knots from the [ENE NE]. The waves are [1.0 1.5 meters] from the [ENE NE]. The operation is starting at 0900 local time under clear conditions and is forecasted to increase wave conditions by 1100. The Assessee should use manual control (non-dp) to maneuver the vessel to the specified location, set the vessel up on DP, and perform all required pre-operation procedures. Once on location, the Assessee should give the ROV team permission to launch the ROV. As an assessment for a DP-2 or DP-3 vessels, this assessment is a good candidate for Phase 5 Assessments. 1

The operation is starting at 0900 local time under clear conditions, an increase in wave conditions is expected by 1100. These conditions shall move the vessel to the edge of allowable levels, thereby causing the DPO to make a decision whether to halt the operation (the waves can be increased beyond the above-specified level if doing so is required to push the vessel to the edge of its operation envelop and force a decision). After the Assessee performs the pre-operation checklist and tells the ROV team to deploy, the acoustic signal begins causing instability in the DP model. As the sea state increases (see above), the instability of acoustic signaling causes the ROV to veer off its intended course and the ship loses its station-keeping ability in ROV follow mode causing the vessel to exceed its excursion limits. Due to the above-described failures, the operation is halted and the ROV is safely retrieved. Shortly after retrieval, the ship experiences a partial blackout. Assessment Instructions: Please note that the items listed below are in a suggested order, however, Examiners/QOBAs are encouraged to group items together or score items in a different order than listed if doing so expedites the assessment or makes for a more logical flow of the assessment. Additionally, assessment scenarios are not designed to cover every aspect of a DP operation, thereby ensuring the assessee does not know what is and is not included on their assessment. As such, the assessee should be instructed to perform all aspects of a DP operation even if they are not instructed to perform the item by the Examiner/QOBA. As an assessment for a DP-2 or DP-3 vessels, this assessment is a good candidate for Phase 5 Assessments. 3

Item 7e-22 and Item 6a-1 are Tier 1 Items. If the assessment is being conducted on a simulator or at times when the environmental conditions are well known, Items 6a- 1 and 6a-8 can be done while the vessel is approaching the 500 meter zone line. 7e-22 6a-1 6a-3 6a-8 Demonstrate bringing the vessel to a full stop while in manual mode. Based upon conditions at the start of the operation and the ASOG, can DP operations be initiated? Based upon conditions at the start of the operation and the CAM/TAM, can DP operations be initiated? Based on Section 5a and the vessel's intact capability plot, can DP operations be initiated? With the vessel moving at its normal transit speed and under manual mode, prompt the Assessee to bring the vessel to a complete stop within 50 meters of a simulated 500-meter zone line without exceeding Chief Engineer's or Master's standing orders. The item is passed if the Assessee is able to perform the stop in a safe and controller manner within the criteria. Prompt the assessee to evaluate the conditions at the start of the operation (or review the information pre-printed in this subsection) and determine if conditions allow for DP operations. The item is passed if the assessee correctly determines conditions (if required), understands the results of these conditions on the vessel, and correctly determines if these conditions could cause the vessel to approach any of the relevant limits (e.g. weather, vessel footprint, or thruster load) listed in the ASOG. Prompt the Assessee to evaluate the conditions (or review the information pre-printed in this subsection) and determine if conditions are within the limits specified in CAM/TAM. The item is passed if the Assessee correctly determines conditions (if required) and correctly determines if these conditions are within the limits found in the specified resource or decision support tool. Prompt the assessee to complete Section 5a (or review the information pre-printed in this subsection) and determine if conditions are within the limits specified in the capability plot. The item is passed if the assessee correctly determines conditions (if required) and correctly determines if these conditions are within the limits found in the specified resource or decision support tool. Item 6g-7 can be done as the vessel approaches location or during the planning time prior to the start of the operation. Alternatively, if this item can be done as part of the checklist later in the assessment. 6h-7 6g-7 Determine a safe location to complete the pre-operation checklist. Demonstrate the creation of an approach plan for a DP operation. Prompt the assessee to determine a location to conduct the preoperation checklist. The item is passed if the assessee determines a location which is outside the 500 meter zone (or complies with alternative charterer instructions), is clear of any obstructions, and considers any other vessels operating in the area. Prompt the assessee to use the information listed in the assessment exercise or scenario to plan an approach to an operation. Note. if assessment is being "layered" the QOBA may prompt the assessee to create an approach plan for the real operation. The item is passed if the plan prepared by the assessee provides for a safe and effective way to move from the vessel's current position to the vessel's working position, is within the vessel's capabilities, factors in simultaneous operations by other vessels, does not endanger the crew, the vessel, the installation, or the environment, and complies with company policy, the charterer's instructions, and industry guidance. 4

7e-23 is a Tier 1 Item. 7e-23 7e-1 Using manual mode, hold the vessel's position and heading. Demonstrate selecting and entering auto position mode. With the vessel in manual control, prompt the assessee to hold the vessel's position and heading. The item is passed if the assessee is capable of holding the vessel's heading within five (5) degrees and the vessel's position within an average of five meters (5) and absolute excursion limit of 10 meters for a 15 minute period. During this time the environmental conditions shall be within those prescribed by the ASOG or other decision support tool. Prompt the assessee to select and enter the required mode. The item is passed if the assesse can select and enter the required mode without the vessel exceeding limits prescribed in the ASOG or decision support tool. 6h-4 is an unlisted and unprompted item. 6h-4 Know to complete a field arrival checklist The assessee shall know to complete the required checklist. The item is passed if the assessee knows to complete a field arrival checklist at the correct time and performs all of the verifications required by the checklist. The below-listed items should all be recorded as the checklist is completed. Note, the entire checklist should be completed, regardless if every action listed on the checklist is reflected in an Assessment Item below. 6d-1 6j-1 6c-1 Describe how the vessel's power generation and distribution system could be set up for the operation using the vessel's Critical Activity Mode (CAM) or maximum redundancy mode as a guide. Set the position and heading alarms for the specified DP operation Describe which thruster(s)/propulsion system(s) should be enabled for the operation and explain to the Examiner/QOBA why these choices were made. Prompt the assessee to use the assessment form to record or otherwise explain how the vessel's power generation and distribution system could be configured in order for these systems to be configured within the vessel's CAM. The item is passed if the assessee is capable of ensuring the vessel's power generation and distribution systems are set up in accordance with the vessel's CAM or identified maximum redundancy mode. Prompt the assessee to set up the position and heading alarms for the specified DP operation. The item is passed if the assessee sets the alarm limits within those prescribed by the ASOG, charterer instruction, or company policy. Prompt the assessee to use the assessment form to record or otherwise explain which thruster he or she will enable. Once he or she has completed this section, ask the assessee why these choices were made. The item is passed if the assessee selects thrusters which are appropriate for the operation; lessens the ability for outside influences; and chooses a sufficient number to comply with IMO equipment class requirements, charter instructions, or ensure the vessel does not exceed worst case failure. 6j-2 Demonstrate enabling the vessel's consequence analysis function. Prompt the assessee to initiate the required function. The item is passed if the assessee correctly enables the function. 5

7c-1 7c-3 7a-7 7a-5 6g-5 6g-8 6g-1 Demonstrate enabling the required number of gyrocompasses or heading sensors in the DP system and confirming each is proving consistent data. Demonstrate enabling the required number of wind sensors in the DP system and confirming each is proving consistent data. Demonstrate verifying that the vessel is configured in accordance with the vessel's redundancy concept. Demonstrate the creation of an exit plan for a DP operation. Demonstrate utilizing the vessel s FMEA to identify the vessel s worst case failure. Prompt the assessee to enable the number of gyrocompasses or heading sensors required for the operation and confirm each is providing consistent data. The item is passed if the assessee is able to enable these sensors and validate the data or signal from these sensors. Prompt the assessee to enable the number of wind sensors required for the operation and confirm each is providing consistent data. The item is passed if the assessee is able to enable these sensors and validate the data or signal from these sensors. Prompt the assessee to set up, validate and enable the number of acoustic position reference systems required for the operation. The item is passed if the assessee is able to set up and validate the data or signal from these systems and enable them in the DP system. Prompt the assessee to set up, validate, and enable the number of satellite-based position reference systems required for the operation. The item is passed if the assessee is able to set up and validate the data or signal from these systems and enable them in the DP system. During the planning of an operation, prompt the assessee to complete the DPO checklist and ask him or her to describe how the items listed on the checklist verify the vessel has been set up in accordance with the vessel's redundancy concept. The item is passed if the assessee goes through the checklist and describes how each applicable item supports the vessel's redundancy concept as determined by the FMEA. Prompt the assessee to use the information listed in the assessment exercise or scenario to plan field exit plan. Note. if assessment is being "layered" the QOBA may prompt the assessee to create a field exit plan for the real operation. The item is passed if the plan prepared by the assessee provides for a safe and effective way to move from the installation to outside the 500 meter zone, is within the vessel's capabilities, factors in simultaneous operations by other vessels, does not endanger the crew, the vessel, the installation, or the environment, and complies with company policy, the charterer's instructions, and industry guidance. During the planning of an operation, prompt the assessee to identify the vessel s worst case failure capability by the use of the vessel s FMEA. The item is passed if the assessee is able to use the FMEA to find the required information. 6

Once the checklist is complete, the Assessee shall inform the ROV operator that the vessel is ready to start the operation. The ROV operator informs the DPO that the ROV is being deployed and that the DPO should switch to follow target mode (or equivalent if DP equipment in use utilizes a different term). 7e-4 Demonstrate selecting and entering follow target mode. Prompt the assessee to select and enter the required mode. The item is passed if the assessee can select and enter the required mode without the vessel exceeding limits prescribed in the ASOG or decision support tool. Once the ROV is in the water and the vessel is following the ROV utilizing the track follow mode, the Assessee should be instructed to monitor the operation. 8c-2 Utilize the DP console to determine DP sensors in use. At a random point during the operation, ask the assessee to relay the required information. The item is passed if the assessee is able to use the DP console to determine and relate this information to the Examiner/QOBA within 30 seconds. As the operation continues, create (or simulate or mimic) the change in the environmental conditions referenced in the above assessment narrative (an increase in the wave actions that takes the vessel to the edge of its allowable levels) and record the following Assessment Items. If this environmental change causes the Assessee to halt the operation, restore the environmental levels to their previous state after these Assessment Items have been recorded. 8a-21 8a-11 Demonstrate monitoring of the vessel's position verses its excursion limits and determine if operations can continue. Demonstrate an awareness of the vessel's heading and position during an operation and if station keeping is improving or degrading. Create (if on a simulator) or simulate or mimic (if on a vessel) vessel movements that approach the vessel's excursion limits and observe the assessee's response to these movements. The item is passed if the assessee evaluates the situation against the vessel's ASOG or other decision support tool and determines if a change in the vessel's set up or position can improve its station keeping capabilities. During the operation ask the assessee to turn away from the DP console and then relate to the Examiner/QOBA the vessel's current position and heading and if deviations are increasing or decreasing. The item is passed if the assessee can accurately (vessel position within 1 meter relative to the installation heading within 1 degree) relate this information without looking at the DP console. 8b-6 is a Tier 1 Item. Do not prompt that the Assessee to make the communication. 8b-6 Demonstrate knowledge of what crew members (or other parties) must be notified of a change in vessel status and what communication methods to utilize to notify these parties. Observe the assessee notifying individuals of a change in vessels status. This item is passed if the assessee notifies the correct crew members and/or other parties as dictated by the vessel's ASOG or decision support tool and notifies these parties via the most appropriate communitarian method. This item must be done in conjunction with an assessment item causing a change in vessel status (e.g. a fault, failure, or change in condition). If not using an ASOG or decision support tool does not list whom should be notified see ASOG in the MTS Operations Guidance (Part 2, Appendix 3). 7

8c-1 9a-6 Utilize the DP console to determine the number of position reference systems currently enabled. Understand that an updated informal risk assessment must be conducted after a change in conditions. At a random point during the operation, ask the assessee to relay the required information. The item is passed if the assessee is able to use the DP console to determine and relate this information to the Examiner/QOBA within 30 seconds. During an operation cause (if on a simulator) or simulate or mimic (if on a vessel) a change in environmental conditions and inform the assessee of this change and observe his or her response. The item is passed if the assessee understands that they should conduct an informal risk assessment, comparing the updated conditions against the vessel's ASOG or other decision support tool. Once the above items are recorded, return the environment to its initial state and instruct the Assessee to continue monitoring the operation (Space is provided below to record the Assessee s answers). 8a-7 is a Tier 1 Item 8a-16 8a-7 Demonstrate an awareness of the current environmental conditions. Demonstrate an understanding of what conditions would cause the vessel to move into a yellow or "degraded" status and how to respond. During the operation ask the assessee to turn away from the DP console and then relate to the Examiner/QOBA the current environmental conditions. The item is passed if the assessee can accurately (wind direction and speed within five (5) knots (at steady state), estimated current within 0.5 knot and trained), relate this information without looking at the DP console. During an operation, ask the assessee to identify five (5) probable events, changes, faults, or failures would cause the vessel to move into a yellow or "degraded" status and how to respond to each. The item is passed if the assessee can identify five (5) events, changes, faults, or failures which would move the vessel into a yellow or "degraded" status and describe a response to each that is within the vessel's ASOG or decision support tool. (This question can be asked after the operation if the Examiner/QOBA does not want to disrupt the assessee's concentration.) 8c-18 Demonstrate a verification that the DP system is following commands Prompt the assessee to verify the system is following commands. The item is passed if the assessee knows how to perform this procedure and performs the procedure correctly within 30 seconds. 8

After the above-listed verification is completed, cause (if on a simulator) or simulate or mimic a problem with the signal provided from the ROV, specifically the acoustic signal should have in instability which causes ROV to deviate from its track, subsequently causing the vessel to lose position as it follows the ROV. As this event is happening, record the following Assessment Items. The abandoning procedures need to be carried out only through a point where the Assessor can determine the Assessee knows these procedures; the vessel does not need to leave the 500-meter zone). If an operation cannot be easily reset, following an abandonment procedures, these assessment items can be moved to the end of the scenario. 9a-3, 9e-10, and 9e-15 are Tier 1 Items. 9a-3 is an unlisted and unprompted item. 9a-3 9e-10 9e-15 Demonstrate the response to an instability in the PRS data from a single PRS. Demonstrate a determination that an operation is in "abandon" or red status and abandon an operation that has a long time to terminate (connection to an installation, equipment in the water). Demonstrate abandoning of an operation (i.e. quickly disconnecting or retrieving equipment). During the operation (without announcement or notice) cause (if on a simulator) or simulate or mimic (if on a vessel) one of the enabled position reference systems providing the data problem mentioned by the item. The item is passed if the assessee realizes the situation, evaluates the situation against the vessel's ASOG or decision support tool (and possibly class requirements) and takes the action required by evaluation. During an operation with a long time to terminate, create (if on a simulator) or simulate or mimic (if on a vessel) a situation that puts the vessel into "abandon" or red status and prompt the assessee to respond. The item is passed if the assessee correctly determines (using the vessel's ASOG) that the vessel is an abandon status, terminates the operation (via the instructions in the ASOG or charterer requirements), and leaves the 500 meter zone quickly and safely. 500 meters of a fixed or floating installation. During an operation with a long time to terminate, inform the assessee that the operation must be abandoned and observe his or her reaction. (This item should be mimicked if conducted on a vessel). The item is passed if the assessee quickly disconnects from the installation or quickly retrieves the equipment and abandons the operation under the instructions in the ASOG or charterer requirements and the operation escape plan. Once the abandoning procedures have been carried out (see above note about procedures that cannot be easily reset) the operation should be reset back to the point before the failure referenced in Item 9a-3 and the Assessee should be instructed to continue the operation. 8a-18 8a-19 Demonstrate monitoring of the position reference systems to ensure all are providing accurate data. 8c-3 Utilize the DP console to determine the number of thrusters currently enabled and percentage of power in use. At a random point during the operation, ask the assessee to relay the required information. The item is passed if the assessee is able to use the DP console to determine and relate this information to the Examiner/QOBA within 30 seconds. After the above-listed monitoring items have been conducted, simulate a call from the ROV operation stating that they need the vessel change its set point in order to continue the operation. Once this call is simulated, the Assessor should record the below assessment item. 8c-11 Utilize the DP console to change the vessel's set point (absolute mode). Prompt the assessee to utilize the DP console to input the required commands. The item is passed if the assessee is able to make the required change within the activity-imposed time constraints. Once the above-listed item has been recorded, cause, simulate, or mimic an event that causes the below-listed alarm and record the results. Assessments being conducted on (or simulating) a DP-1 vessel, can substitute a different item 9e-1 is a Tier 1 Item. 9e-1 Demonstrate a response to a consequence analysis alarm that is appropriate to the alarm, the current conditions, and the operation. Create (on a simulator) or simulate or mimic (on a vessel) a consequence analysis alarm and observe the assessee's response. The item is passed if the assessee responds to the alarm, investigates its cause; evaluates the result of the alarm against the current conditions, ASOG, and/or operation; and makes a decision based upon these factors. After the above-listed alarm has been handled, a partial blackout should be caused on the operation. This black out should take out half of the vessel s generators and put the vessel post worst case failure. Because of this problem, the Assesse loses access to thrusters, the DP console currently in use, and a DP sensor. These problems cause the operation to be abandoned. Once all of the below assessment items have been recorded, the assessment is concluded. 9c-1, 9b-5, 9b-1, 9d-1, 9e-3, and 7c-27 are Tier 1 Items. 9c-1 and 9b-5 are Do Not List, Do Not Prompt Items. 9b-1 and 9d-1 are Do Not Prompt Items. 9c-1 Demonstrate a response to a partial blackout. The item is passed if the assessee recognizes the alarm, acknowledges the alarm, takes the action prescribed by the vessel's ASOG or other decision support tool, and (if required) notifies crew/charterer/client/others. Create (simulator) or simulate or mimic (on a vessel) an alarm involving one (1) thruster. The item is passed if the DPO/Prospective DPO recognizes the alarm, acknowledges the alarm, takes the action prescribed by the vessel's ASOG or decision support tool, and (if required) notifies crew/charterer/client/others. During the operation (without announcement or notice) cause (on a simulator) or simulate or mimic (on a vessel) a DP console failure and observe the assessee's response. The item is passed if the assessee realizes the situation, evaluates the situation against the vessel's ASOG or other decision support tool (and possibly class requirements) and takes the action required by the evaluation. In a close-quarter situation (less than 50 meters from a simulated or mimicked installation, another vessel, or other asset), and the vessel in manual mode, set up (or prompt the assessee to set up) the vessel in a configuration which mirrors the vessel's post-worst case capability. Once completed, prompt the assessee to maneuver the vessel to a point more than 100 meters from current position and change the vessel's heading to a specified heading (must be at least 90 degrees off current heading). The item is passed if the assessee is capable of maneuvering the vessel at least 100 meters and change the vessel's heading to a specified heading (must be more than 90 degrees from current heading). All changes must be made in a controlled manner. 11
