Introduction The DfE have samples of medical healthcare plans and forms for administering medication on their website: https://www.gov.uk/government/publications/supporting-pupils-at-school-with-medicalconditions--3 EIS have created these as reports that can be run directly from SIMS.net for school use. The reports are optional and do not have to be used by schools if unsuitable. They are designed to be used for individual pupils with specific medical needs where appropriate. The Reports There are four available reports to download based on the DfE templates: 1. Individual Healthcare Plan A This report will produce a healthcare plan for individual pupils: The first page contains basic information for the pupil along with any recorded Medical Conditions, contacts ticked as having Parental Responsibility, their Medical Practice and GP name (where recorded) and their SEN Status, Need Types and next SEN Review Date (where recorded). The second page is the suggested layout from the DfE to make a plan for their medical needs, and the wording has been taken from their template. This page has blank comment boxes for schools to fill in as appropriate. 2. Individual Healthcare Plan B This report will produce a form to be completed by the parent/carer of the pupil and then returned to the school, giving permission for agreed medication to be administered to the pupil during school hours. The first contact with a priority of 1 recorded will be displayed in this report. 3. Individual Healthcare Plan C This report will produce a pro-forma for the school to complete each time medicines are administered to an individual pupil, as recommended by the DfE. 4. Individual Healthcare Plan D This report will produce an alternative form for the school to complete each time medicines are administered to an individual pupil, as recommended by the DfE.
