1. TERM ~ "ENGINEER" SHALL MEAN THE PROJECT MANAGER OR THE INSPECTOR OF THE CONSTRUCTION PROJECT. 2. THE CONSTRUCTION AND MATERIAL SPECIFICATIONS TO BE USED IN THIS PROJECT ARE THE STATE OF OHIO DEPARTMENT OF TRANSPORTATION CONSTRUCTION AND MATERIAL SPECIFICATIONS, DATED JANUARY 1, 2013. 3. DETAILS OF STANDARD ODOT ITEMS IN THESE PLANS SHALL CONFORM WITH CURRENT ODOT STANDARD DRAWINGS. THE CONTRACTOR SHALL BE RESPONSIBLE FOR OBTAINING THESE ODOT STANDARD CONSTRUCTION DRAWINGS. 4. EXISTING UTILITIES SHALL CONTINUE WITH UNINTERRUPTED SERVICE DURING CONSTRUCTION PERIOD. 6. THE LOCATIONS OF UNDERGROUND UTILITIES SHOWN ON THESE PLANS ARE AS OBTAINED FROM THE OWNER OF THE UTILITY AS REQUIRED BY SECTION 153.64 ORC. EXISTING UNDERGROUND UTILITIES AND SERVICES ARE SHOWN IN THEIR APPROXIMATE LOCATIONS ACCORDING TO THE BEST INFORMATION AVAILABLE. THE LOCATIONS SHOWN ARE INTENDED ONLY AS A GUIDE AND CANNOT BE GUARANTEED AS ACCURATE OR COMPLETE. CONTACTING THE OHIO UTILITIES PROTECTION SERVICE (1-800-362-2764) 48 HOURS PRIOR TO CONSTRUCTION. 7. ALL, IF ANY, DEWATERING FLOWS ARE TO BE SETTLED IN SILTATION BASINS OR DIRECTED THROUGH STRAW FILTERS BEFORE DISCHARGING TO STABILIZED SITES (SUCH AS STREAMS OR STORM SEWERS). NO DISCHARGE SHALL BE ALLOWED ONTO EXPOSED SOILS, STREAMBANKS, OR ANY OTHER SITE WHERE THE FLOWS COULD CAUSE EROSION. 8. ALL NON-PAVEMENT AND SIDEWALK DISTURBED AREAS ARE TO BE RESTORED BY MULCHING AND RESEEDING. IF FINAL GRADING CANNOT BE ACCOMPLISHED WITHIN 30 DAYS, TEMPORARY SEEDING SHALL BE USED UNTIL LONG-TERM RESTORATION CAN OCCUR. TOPSOIL IS TO BE REPLACED AS PART OF FINAL GRADING. 9. EXCAVATED FILL MATERIAL IS TO BE STOCKPILED ON THE UPHILL SIDE OF THE TRENCH. 10. ALL DISTURBED AREAS ARE TO BE REGRADED TO ORIGINAL CONTOURS, UNLESS OTHERWISE SHOWN ON THE PLANS. 11. EXPOSED SOIL AREAS ARE TO BE KEPT TO A MINIMUM BY APPLICATION OF EROSION CONTROL MATERIALS SUCH AS STRAW SPRAYED WITH ASPHALT COATING. 12. EQUIPMENT, TOPSOIL, AND/OR FILL MATERIALS ARE NOT TO BE STOCKPILED UNDER (WITHIN THE DRIP LINE) OR NEAR EXISTING TREES IN RESIDENTIAL AREAS, WOODLANDS, OR ANY OTHER ENVIRONMENTALLY SENSITIVE AREA. 13. ACCESS FOR EMERGENCY VEHICLES IS TO BE MAINTAINED AT ALL TIMES. ITEM 201 ~ CLEARING AND GRUBBING ALL TREES REMOVED AS PART OF THIS ITEM SHALL BE CUT INTO TWO FOOT (2') LENGTHS, STACKED BEYOND THE RIGHT-OF-WAY LINE, AND LEFT FOR THE RESPECTIVE PROPERTY OWNER(S). IF THE PROPERTY OWNER REFUSES TO ACCEPT THE WOOD, THE CONTRACTOR SHALL DISPOSE OF THE WOOD AS DIRECTED BY THE ENGINEER. PAYMENT FOR WORK DESCRIBED ABOVE SHALL BE INCLUDED IN THE LUMP SUM BID PRICE FOR ITEM 201, CLEARING AND GRUBBING, AS PER PLAN. THIS WORK SHALL ALSO INCLUDE EXCAVATING AND SHAPING THE "MOUNDING AREA" AT THE DRIVEWAY FOR 137 WASHINGTON RD (DRIVEWAY # 9) AS DIRECTED BY THE ENGINEER, TOTAL EXCAVATION QUANTITY, USING 10% SLOPES EAST AND WEST OF DRIVEWAY, IS APPROX. 35 CY OF MATERIAL. ITEM 202 ~ MAILBOX, REMOVED AND REINSTALLED MAILBOXES MUST BE KEPT IN SERVICE AND ACCESSIBLE AT ALL TIMES TO THE U.S. POSTAL SERVICE. MAILBOXES MAY BE TEMPORARILY SUPPORTED UNTIL THE MAILBOX CAN BE PERMANENTLY RELOCATED. ITEM 203 ~ ROADWAY EXCAVATION THIS ITEM SHALLINCLUDE ALL WORK NECESSARY FOR THE EXCAVATION AND PLACEMENT OF EMBANKMENT AS SHOWN FOR ON THE TYPICAL SECTION AND IN THE CROSS SECTIONS. ITEM 204 ~ SUBGRADE COMPACTION AND PROOFROLLING CONSTRUCT THE SUBGRADE AS FOLLOWS AND IN THE FOLLOWING SEQUENCE: a. SHAPE THE SUBGRADE TO WITHIN 0.2 OF A FOOT OF THE PLAN SUBGRADEELEVATION. IF THERE IS UNSUITABLE MATERIAL IN A SHALLOW FILL LOCATION, THEN REMOVE AND REPLACE THE UNSUITABLE MATERIAL ACCORDING TO STEP "b" PRIOR TO CONSTRUCTING THE SHALLOW FILL AND SHAPING THE SUBGRADE. b. REMOVE AND REPLACE UNSUITABLE MATERIALS. THESE MATERIALS ARE TO BE REMOVED PRIOR TO PROOF ROLLING, THE QUANTITIES FOR THE UNSUITABLE SOILS ARE PAID FOR UNDER ITEM 203 - EXCAVATION. c. CONSTRUCT AND COMPACT THE SUBGRADE ACCORDING TO 204.03. d. PROOF ROLL THE COMPACTED SUBGRADE ACCORDING TO 204.06 TO DETERMINE THE ACTUAL LIMITS OF SOFT SOILS TO VERIFY THE UNIFORMITY OF THE SUBGRADE COMPACTION. e. UNDERCUT THE AREAS DETERMINED BY THE ENGINEER AND REPLACE WITH THE SPECIFIED MATERIALS ACCORDING TO 204.07. THE UNDERCUTS WILL EXTEND TO 18" BEYOND THE EDGE OF THE SURFACE OF THE PAVEMENT, PAVED SHOULDERS OR PAVED MEDIANS. FOR ALL AREAS WITHIN ODOT RIGHT OF WAY, ITEM 204 "UNDERCUT" AREAS WILL BE REQUIRED 1. ITEM 204 - EXCAVATION OF SUBGRADE - 18" DEEP 2. ITEM 204 - GRANULAR MATERIAL, TYPE C 3. NO ALLOWANCE SHALL BE ACCEPTED FOR ASPHALT BINDER INDEX. ITEM 611 ~ X" CONDUIT, TYPE A,B &C THESE ITEMS SHALL INCLUDE ALL WORK NECESSARY FOR THE INSTALLATION OF PROPOSED STORM SEWER PIPE TO THE BOTTOM OF THE RESTORATION SECTION. ALL RCP SHALL BE CLASS IV UNLESS OTHERWISE NOTED ON THESE PLANS. DRIVE CULVERTS SHALL BE HIGH DENSITY POLYPROPYLENEE (HDPP) STORM SEWER PIPE OR EQUAL. HDPP STORM PIPE LOCATED UNDER GRAVEL DRIVES SHALL BE BACKFILLED WITH CONTROL DENSITY FILL TO BOTTOM OF GRAVEL DRIVE LAYER. ALL OTHER PIPE LOCATED UNDER DRIVES SHALL BE BACKFILLED WITH CLASS I MATERIAL. PROPOSED STORM PIPE LOCATED UNDER EXISTING ROADWAY SHALL BE BACKFILLED WITH CONTROL DENSITY FILL. ITEM 611 ~ MANHOLES AND CATCH BASINS THESE ITEMS SHALL INCLUDE ALL WORK NECESSARY TO CONNECT EXISTING CULVERTS, SEWERS AND DRAINS TO PROPOSED MANHOLES AND CATCH BASINS. THE CONNECTIONS SO DESIGNATED SHALL BE CONSIDERED INCIDENTAL TO THE MANHOLES AND CATCH BASINS, AND PAYMENT SHALL BE INCLUDED IN THE UNIT PRICE BID FOR THOSE ITEMS. CATCH BASIN INLETS SHALL HAVE BICYCLE SAFE GRATES, NEENAH No. R-4859-C OR EAST JORDAN No. 5110 TYPE M3 OR APPROVED EQUAL. ITEM 614 ~ MAINTAINING TRAFFIC ALL WORK AND TRAFFIC CONTROL DEVICES SHALL BE IN ACCORDANCE WITH ITEM 614 AND OTHER APPLICABLE PORTIONS OF THE SPECIFICATIONS, AS WELL AS THE LATEST EDITION OF THE OHIO MANUAL OF UNIFORM TRAFFIC CONTROL DEVICES (OMUTCD). PAYMENT FOR ALL LABOR, EQUIPMENT, AND MATERIALS NECESSARY TO PERFORM THE WORK SHALL BE INCLUDED FOR ITEM 614 - MAINTAINING TRAFFIC, UNLESS SEPARATELY ITEMIZED IN THE PLANS. ITEM 659 ~ SEEDING AND MULCHING SEEDING AND MULCHING SHALL BE APPLIED TO AREAS OF EXPOSED SOIL BETWEEN THE RIGHT-OF-WAY LINES AND WITHIN THE CONSTRUCTION/GRADING LIMITS FOR AREAS OUTSIDE OF THE RIGHT-OF-WAY LINES AS INDICATED ON THE PLANS. DURING THE OFF HOURS, SUFFICIENT WARNING LIGHTS AND BARRICADES SHALL BE PROVIDED BY THE CONTRACTOR AND PLACED ALONG THE ROUTE OF CONSTRUCTION TO PROVIDE ADEQUATE PUBLIC SAFETY. SAFETY FENCE IS TO BE SECURELY INSTALLED AROUND THE EXCAVATION AT THE END OF EACH WORK DAY. THERE SHALL BE NO BURNING ON THE JOB SITE OF CONSTRUCTION DEBRIS. THERE SHALL BE NO BURYING OF CONSTRUCTION DEBRIS INCLUDING TREES & STUMPS ON THE JOB SITE. HISTORICAL ARTIFACTS IF DURING THE COURSE OF CONSTRUCTION, EVIDENCE OF DEPOSITS OF HISTORICAL OR ARCHEOLOGICAL INTEREST ARE FOUND, THE CONTRACTOR SHALL CEASE OPERATIONS AFFECTING THE EVIDENCE. THE CONTRACTOR SHALL IMMEDIATELY NOTIFY THE ENGINEER/OWNER AND OWNER SHALL NOTIFY THE OHIO HISTORIC PRESERVATION OFFICE. NO FURTHER DISTURBANCE OF THE EVIDENCE SHALL OCCUR UNTIL THE CONTRACTOR HAS BEEN NOTIFIED IN WRITING THAT THEY MAY PROCEED. THE ENGINEER WILL ISSUE A NOTICE TO PROCEED ONLY AFTER THE OHIO HISTORIC PRESERVATION OFFICE HAS SURVEYED THE FIND AND MADE A DETERMINATION OF VALUE AND EFFORT AND SUBMITTED SUCH DETERMINATION TO THE OWNER.
