INFORMATION FOR BIDDERS Receipt and Opening of Bids The Oregon Park District invites bids on the forms attached for the 2017 Paving Improvements. The Oregon Park District will accept sealed bids until 10:00 am on Wednesday, April 26, 2017 at the Nash Recreation Center, 304 South Fifth Street, PO Box 237, Oregon, IL 61061. Bids will then be publicly opened and read aloud at the Nash Recreation Center. The bid envelope must be sealed and addressed to Andy Egyed, Superintendent of Parks, and include the following information: - Name of Bidder - Bidder s Address - Date and Time of Bid Opening - Bid Enclosed: 2017 Paving Improvements Qualifications of Bidders The Oregon Park District may investigate the Bidders ability to supply the necessary equipment, and the Bidder shall furnish all such information and data for the purpose as requested. The Oregon Park District reserves the right to reject any bid if the evidence submitted by, or investigation of, such Bidder fails to satisfy the District. Compliance with Laws Bidders shall at all times observe and comply with all laws, ordinances, and codes of the Park District and all federal, state, and local governments and other government agencies which may in any manner affect the preparation of Bids and Proposals or the performance of this Contract. Insurance Coverage & Requirements The contractor must obtain/acquire the following coverage s: - Certificate of Commercial / General Liability Insurance - Workers Compensation and Liability Insurance - Abide by the Illinois Prevailing Wage Act for Ogle County - Provide the Oregon Park District with Certified Payroll Damages to Property The contractor shall be responsible for any damage to properties caused by the acts of their work in the course of performance of this contract and shall replace or restore to its original condition any such damaged property at no cost to the Oregon Park District. The Oregon Park District shall be held harmless for all liability under the Scope of Work of this contract. Time of Completion and Penalties All paving and preparation work cannot begin until the completion of new playground installations. The playground contactor will be responsible for updating the owner on progress. Playground completion is planned by May 1, 2017. Should the paving contractor fail to complete the work by June 1 st, 2017 or within such extended time as may have been granted by the owner for weather related delays or playground installation delays, the Contractor shall be liable to the Owner in the amount of Five Hundred ($500.00), as damages, for each day of overrun in the contract time. Any damages owed the Oregon Park District may be deducted from any payments to the Contractor.
Payment Once the contractor has finished the project work, the contractor shall notify the Park District that the work is ready for final inspection. The final payment will not be released until the Park District receives all required documentation to include the projects certified payroll. Scope of Work The contractor will enter into a contract with the Oregon Park District for the following Paving Project. The contractor must provide all necessary labor, material and equipment, and must dispose of all materials. Base Bid Fairgrounds Park, 607 Fair Park Dr, Oregon, IL 61061 Walking Trail - Surface preparation must be completed, to include removal of loose materials. Any broken or missing trail sections must be replaced with appropriate base prior to paving. - Spray tack coat on existing surface and apply 2 overlay of hot-mix asphalt - Parking lot and through driveway are to be resurfaced on District property only. The bid enclosed map marks District property. - Grind and/or reshape existing trail surface that must meet existing concrete or asphalt surface edges (marked on project map as surface joints). - Cut, remove, shape, compact and overlay 10 x 75 area of walking trail near trail head (marked on project map as replacement section). The excavated section shall be replaced with 6 of aggregate base (CA-10) course and 3 of hot-mix asphalt. - Compact and finish all surface edges. The contractor shall match all existing grades. - There will be three additional paved entrances to the new playground pods at the Park. The playground pods are noted on the Fairgrounds map. The entrances will be approximately 30 sq. ft of new asphalt (5' x 5'-6' sections). The entrances will be near ground level to the playground and must be tapered from the existing path. The new entrances shall be excavated, filled with 6" of aggregate base (CA-10) course and 3" of hot-mix asphalt. - The removal and replacement of the parking blocks and repainting of parking lines will be the responsibility of the owner. Approximate Paving Quantities Bike Path: 4,100 square yards Parking Lot/Driveway: 1360 square yards Field Measurements The contractor is responsible for confirming the field measurements of each location included in this bid.

The Bidder must provide written certification that they agree to pay the Prevailing Wage, and that they have not been barred from bidding for reasons of bid rigging or bid-rotating, or for delinquency in payment of any Illinois Tax. 5. The Bidder must provide the Oregon Park District with certified payroll. 6. The Bidder must provide, no later than 14 days following the date of the award, and maintain at its own cost and expense, a Performance Bond and Payment Bond each in an amount equal to 100% of the highest cash proposal. The surety on such bonds shall be a surety company authorized to do business in the State of Illinois and approved by the Park District. In the event that the successful bidder
fails to furnish the Performance Bond and Payment Bond and the evidence of insurance in said period of ten calendar days or such extended period as the Park District may award the Contract to the next lowest bidder or re-advertise for bids. 7. The Bidder must obtain and provide copies to the Oregon Park District: a. Certificate of Commercial / General Liability Insurance b. Workers Compensation and Liability Insurance 8. The Bidder acknowledges that it is the Bidder s responsibility to ascertain whether any Addenda have been issued and if so, to obtain copies of such Addenda from the Oregon Park District. The Bidder therefore agrees to be bound by all Addenda that have been issues for this Bid. 9. The Bidder agrees that the submitted bid will be held in good standing and honored for a period of 90 days. 10.
