1 BID FORM - LUMP SUM BID PROJECT: Livingston Northside Park and Soccer Fields Phase 1 Construction LOCATION: Livingston, Montana Ladies & Gentlemen: Pursuant to and in compliance with the Advertisement for Bids, Instruction to Bidders and other documents related thereto, the undersigned, having familiarized him/her self/selves with the existing conditions on the site and the conditions under which the work is to be done, hereby proposes to furnish all labor, materials, equipment, and services to perform all specified work for the Site Improvement on the above named project in strict accordance with the Contract Documents and all addenda issued thereto at the price/prices set forth on the accompanying bid form sheets attached hereto. If written notice of acceptance of this bid is mailed, telegraphed, faxed, or delivered to the undersigned within thirty (30) days after the opening of bids, or at any time thereafter before this bid is withdrawn, the undersigned agrees to execute and deliver a contract in the form specified and furnish the required bonds within ten calendar (10) days after the prescribed forms are presented to him/her for signature. 3 BID FORM- BASE BID Base Bid Lump Sum Price ($ ) Dollars. The above price includes completion of all the work called for under this Contract. The scope of work shall include all work specified herein and shown on the plans. All of the work shall be complete within 90 calendar days following contract award, weather permitting. It is the Contractor's obligation to have thoroughly familiarized himself/herself with the site, the conditions under which the work will be done, and the actual quantities required to complete the work in accordance with the intent of the plans and specifications and that the above stated lump sum price, as bid, covers the complete work as required. 4 BID FORM - ALTERNATIVES The following alternate prices may be used to adjust the contract sum in the event the Owner desires to increase the scope of the project. The Contractor understands the Owner reserves the right to select any, all or none of the alternative prices, and to adjust the contract amount accordingly. 5 BID FORM - UNIT PRICES The following unit prices may be used to adjust the contract sum in the event the Owner desires to increase or decrease the scope of work. The Contractor understands the Owner reserves the right to select any, all or none of the unit prices, and to adjust the contract amount accordingly. All unit prices shall be for all work required to furnish and install, including incidentals, complete-in-place systems. Unit price items may or may not be included in base bid work. No adjustment to base bid and alternative amounts shall be made without written authorization prior to commencement of the work.
