Narrative Report PURPOSE PROCEDURE The purpose of this inspection (with Glenn Lough from Central Office) was to obtain additional information about collision damage from an over-height vehicle traveling south on CR 21 on August 10, 2013 (CM5137R). PROCEDURE Information for this inspection report was obtained July 16 and 17, 2014. The inspection party consisted of Team Leader D. L. Whited and A. J. Miracle. The inspection consisted of visually observing the superstructure elements in Span Two. The underside was observed from the ground and from a platform truck. The original Special Inspection was made August 12, 2013 by D. L. Whited, who document the collision damage. SUMMARY & RECOMMENDATIONS A tractor trailer hauling a steel shipping container on a flat bed trailer (Infra source of Ypsilanti, Michigan) struck the bottom flanges of Beams Three - Six near the first diaphragm from Pier One in Span Two. The damage, scrape marks, was less severe on Beams Four and Five. The contact points with the most severe damages are on Beams Three and Six. The worst being Beam Six where the bottom flange is bent out 7-1/16", the south side of the bottom flange is bent up 2-3/4, the web is 5-1/4" out of plumb, and the bottom flange of Beam Three is bowed out 1-5/16". There is now a 1/2'' crack in Stringer Six at the contact point on the bottom flange. Dye Penetrant testing was done on Stringer Six to confirm the crack (Photos 1-5 & Condition Documentation Sketches). The first diaphragm north of Pier One, between Beams Three and Four has collision damage. The bottom flange diaphragm connection welds are cracked for 1-1/4 on both stringers and the diaphragm connection at Beam Six is torn loose. The Pier One Cap and column have spalled concrete due to the container impact. The beams should be replaced, repaired, or straightened, as necessary. The diaphragms and their connections will need to be repaired or replaced and reattached to the structure. All steel repairs should then be primed and painted. The Pier One column and cap should have concrete repairs and the asphalt shoulder damage should be repaired. ENGINEERS COMMENTS New Structural Repair Unit Leader, Glenn Lough, PE, requested another review of the accident damage by CM5137R. This discovered additional damage, in the form of a crack in Beam 6, from this accident. From August 12, 2013 Inspecton for CM5137R The trailer and container were measured at 14'-10" per Ron Hartshorn, D-3 Safety Specialist, and is included in his DOH-107 Report. The pulling out of the welded diaphrams on Beam 6 could have damaged the beam in that area. There was also some damage to the asphalt shoulder, due to crash. 1

Narrative Report The shoulder of the bridge over Beam 6 on I-77 N was closed with barrels and signs, as a precautionary measure. CM5137R was set up to contain all the costs incurred by the DOH related to this accident. Structural Engineers will be scheduled to review the damage and decide a repair plan.
