Position Objectives Supporting HACC eligible people access HACC and other services as required, through assisting people to access appropriate needs identification, assessment and care planning based on the persons expressed goals, aspirations and needs. The position will target HACC eligible people who are not able to access HACC services, or require additional support because of their diversity. Key Selection Criteria Experience in the HACC sector with demonstrated experience in assessment, care coordination / case management or advocacy or other relevant role within a service coordination context. A demonstrated understanding of diversity issues as they relate to individuals and their engagement with community care services. Excellent interpersonal skills Familiarity with the service coordination framework including the key elements of initial contact, initial needs identification, assessment, care planning and the service coordination tool templates and standards. Highly developed verbal and written communication skills with a demonstrated ability to write in a way that is clear, appropriate and readily understood by the target audience. Key Responsibility Areas Build strong partnerships and relationship with local service provides. Actively support people with diverse needs to remain living in their home independently for as long as possible. Participate in relevant service system, networking and partnership meetings Undertake verbal and written referrals and advocacy for eligible clients. Provide support to eligible clients through the care pathway and collaboration with agencies. Advocate on behalf of clients to HACC assessment services and other services providers as appropriate. Undertake reporting as required by the Department of Health and Human Services Actively promote the Access and Support program. Undertake diversity quality improvement and monitoring. Work sensitively and diplomatically with a diverse range of individuals and service providers and to advocate on behalf of clients and empower people to self-advocate. Work within the HACC Active Service Model framework, ensuring the implementation of person centred strength based approaches and client self-empowerment strategies. Specialist Skills and Knowledge An ability to analyse a wide range of needs for people of diverse backgrounds Demonstrated experience in assessment, case management, care coordination, advocacy or other roles within a service coordination context. Extensive interpersonal skills with the ability to create productive networks and a good rapport with individuals. An ability to exercise judgement and decision making in response to presenting client situations. An ability to plan and organise work within strict timelines. Demonstrated ability to work independently. Demonstrated understanding of the issues facing the aged care and disability service industries. The ability to manage his/her time and work program to deliver the required outcomes of the position. Demonstrated competency in the use of computers to enter client data, maintain records, collect and enter HACC MDS data, and complete electronic referrals. Demonstrated understanding of the challenges and opportunities in working in rural environments. Ability to prepare high standard reports and client notes. Page 2 of 6

Qualifications and Experience A relevant post-secondary qualification and experience relating to an area of health and / or community services practice for example Certificate IV in HACC/Aged Care, Disability Studies, Aged Care, Welfare Studies, Case Management or Social work. Demonstrated ability and commitment to working with the frail aged, people with a disability, their carers and persons from diverse backgrounds. The successful applicant will be required to undergo a Police Criminal Records Check and a working With Children s Check prior to the position being confirmed. It is a requirement of the position that the incumbent have and continue to hold a license to drive a motor vehicle in Victoria and achieve a positive response to a Police check. Well-developed written and verbal communication skills including proven ability to convey messages clearly and simply as well as ability to liaise and communicate with relevant stakeholders. Recordkeeping Responsibilities As an employee of the Victorian Public Service Sector, it is your responsibility to create and capture a full and accurate record of your work related decisions and activities. Accountability and Extent of Authority The incumbent is subject to annual review (pursuant to Local Government Authorities Award 2001) and is accountable for: The Officer is accountable to their Manager for providing accurate, effective and customer focused support to meet the needs of the Council. The position allows the Officer the authority to make decisions about the implementation of appropriate work practices to achieve the objectives and responsibilities of this position. The Officer has the authority required to carry out the duties and responsibilities of the position in accordance with established procedures, business rules and standards. Extent of Authority Subject to, and within the framework of, the provisions of relevant legislation and statutory requirements, and the Council s policies and procedures, the incumbent has the authority to provide advice and assistance to management and staff in accordance with: the Council Plan; relevant human resources, industrial relations, and anti-discrimination legislation; relevant Awards and certified agreements; Council s policies and procedures; and accepted organisation standards. Emergency Management The Greater Shepparton City Council understands and accepts its roles and responsibilities in emergency management operations described in the Emergency Management Act 1986 and it is a core function of Council business. At times the incumbent may be asked to assist in Council s emergency management operations, within reason.
