PS 02 PLANT AND EQUIPMENT PURPOSE AND SCOPE The intent of this document is to eliminate or minimise the risks of fatalities, injuries and incidents arising from mobile powered plant, static plant and equipment at Laing O'Rourke workplaces. This standard gives instructions on: operating mobile plant and static powered plant safely, monitoring the safe use of mobile plant and static powered plant, maintaining mobile plant and static powered plant. This document provides the requirements governing the use of mobile plant (defined below) that Leaders, Managers, Supervisors and anyone involved in the process must uphold. This document must be read in conjunction with the OEM Manual, Plant Risk Assessment, applicable Plant and Equipment minimum standards and relevant site procedures and requirements. CRITICAL CONTROLS Plant is inspected before mobilisation and before use. Operators are verified as competent for the specific item of plant. (Refer to SR 08 Onboarding, Training, Induction and VOC) Plant and people are separated, defined exclusion zones are maintained and entry to work areas is controlled. Stability and ground bearing pressure are verified for plant set up. Plant and vehicles are parked in a fundamentally stable condition where there is a possibility of run away. Safe access for pedestrian movements. A traffic management plan is in place. Heavy vehicles and light vehicles are segregated wherever possible Rules are established and enacted for plant following or overtaking manoeuvres Dedicated refueling and servicing rules and locations are established. ADDITIONAL LOCAL CONTROLS Plant and/or equipment to be used must conform with Laing O'Rourke's Plant and Equipment Minimum Standards The work environment, temporary works and risks associated with specific types of plant are to be considered when selecting an item of plant for a task or work area.
PS 02 Plant and Equipment (Rev 2 09.12.16) Section 2 All items of powered mobile plant must be risk assessed, taking into consideration different lifecycle phases of the plant, including delivery, loading, unloading, operation, maintenance, service and inspections, as well as purpose and scope of work specific to the plant and site conditions. Where a manufacturer s Plant Risk Assessment cannot be obtained, the Supplier or Owner shall carry out a risk assessment that addresses these requirements. Plant Assessor or the form E-T-8-0938 Plant Risk Assessment may be used to document this risk assessment. Any attachments must have the necessary certificates to ensure compatibility and suitability for use before being used. Plant is maintained and serviced as required in accordance with original equipment manufacturer (OEM) operator manual maintenance schedules or as defined by a competent person Plant used for loading/offloading shall be fitted with a load indicator / weight measuring device. Safe Working Loads shall be adhered to at all times. Mobile plant and site vehicles must be fitted with warning devices including a flashing light and audible alarm that will warn people who may be at risk from movement of the plant. Mobile plant with enclosed cabins must have an emergency exit in case main door becomes obstructed. An effective system of communication based on two way acknowledgement between mobile plant operators and ground workers should be established before work commences: o Clear radio communications with a secondary backup method is preferable. o Relevant workers should also be trained in the procedures involved prior to the work commencing. o The system should stop ground workers from approaching mobile plant until the operator has agreed to their request to approach. o The system should stop operators from moving plant closer than a set distance (e.g. 15m for tracked plant, 30m for wheeled plant) from ground workers until the operator has been advised by ground workers that they are aware of the proposed movement. Loading/offloading activities are managed to: o Separate people from the activity o Control the risk of falls from heights from the rear of trucks o Ensure chain of responsibility obligations are met (refer to PS 17 Site Establishment and Logistics) Light Vehicles are fit for purpose for site requirements. IVMS must be fitted to all Laing O'Rourke vehicles. Where JV partners / subcontractors etc. utilise light vehicles as substantial part of delivering the works IVMS should be considered by the workplace when establishing contractual arrangements. A SWMS must be in place for using and working in and around Mobile Plant on construction sites. A safe system of work may be implemented at factories, laydown yards etc. EARTHMOVING EQUIPMENT All Earthmoving machinery must be fitted with R.O.P.S. and F.O.P.S. compliant to AS 2294, as required by relevant legislation in the jurisdiction of operation (refer Work Health & Safety Regulation 2011; Clause 214, 215, & 217 and WA Occupational Safety & Health Regulation 4.44 & 4.45) Quick hitches must be fully automatic double locking hydraulic type that prevents both the falling off and swinging of any attachment, in the event of a failure in the primary system.
PS 02 Plant and Equipment (Rev 2 09.12.16) Section 3 All earthmoving equipment to be used as a crane must have approval from the designated appointed person and be fitted with anti-burst hydraulic valves, a load chart, designated and certified lifting points and be used in accordance with the OEM / Lifting Operations Plan (Refer PS 03 Cranes and Lifting). E-T-8-1505p Excavator Lifting Suspended Loads Approval should be completed and signed by a competent person prior to using excavators, backhoes, loaders or integrated tool carriers for lifting suspended loads. ELEVATED WORK PLATFORMS Ensure operators have a VOC and have been assessed as familiar with the specific type of EWP. This may also include the need for a High Risk License for the type of EWP (where required by the jurisdiction) and a Working at Heights qualification when the use of harnesses is required. All operators working in Telescopic Boom lifts will wear full body harnesses with a fall restraint lanyard and be clipped on to manufacturers anchorage points at all times. To reduce the risk of crush injuries: o Select the EWP considering obstructions and plan the EWP route o Ensure anti-crush guarding on controls is present and in working condition o Scissor lift controls must have guarding to prevent inadvertent movements o Keep a safe distance away from structures o Avoid using the drive, elevate and slew controls when close to an obstruction o Driving at height shall be the last resort o Consider Operator protection from injuries through been catapulted (ground depression or over a protrusion while the platform is under or approaching an overhead obstacle) Driving EWP's on road or over long distances on site should be avoided Ensure there is a rescue procedure in place and test the procedure before commencing works Ground conditions are assessed for suitability / use of out riggers PRESSURISED EQUIPMENT Before employees are permitted to use pressurised equipment, they shall be suitably trained in their appropriate use, how to identify hazards and implement controls associated with the equipment. Particular attention must be given to the securing and whip checking of: o Air compressors, lines, fittings and associated tools and accessories. o High pressure water lines, pumps and vessels, o Hydraulic lines and pressure units. PREMOBILISATION AND INSPECTION When and plant or equipment first arrives on a site: A competent member of staff (as assigned by project / workplace leader) must inspect the plant and complete the appropriate pre-mobilisation checklist(s); Obtain copies of required documentation: o Plant Risk assessment specific to the item of plant,
PS 02 Plant and Equipment (Rev 2 09.12.16) Section 4 o Log book specific to the needs and OEM manual of the type of plant, o Service and maintenance records, o Inspection Certificate from a Registered Professional Engineer (Hong Kong specific requirement). If any defects are found plant should be tagged out of service and the supervisor/line manager must be notified (and Select when cross-hired), LORAC & Select plant that has a current pre-mobilisation check and is transferring directly between LORAC sites do not need another pre-mobilisation check completed, but this may be undertaken at the discretion of the project. All Mobile Powered Plant on site is to be registered on E-T-8-0553 Plant and Equipment Register or equivalent electronic database such as e-induct. Forecast and create a maintenance, inspection and testing schedule and book it with the relevant Select Workshop Manager for Select equipment. Each time before plant is used: it must be inspected and the relevant Pre-start Check sheet/log book/form completed, any defects found must be rectified and recorded or tagged out of service PLANT STAND DOWN PROCESS If any safety related fault is found at any time, the operator must immediately report the fault to their supervisor or line manager. The nature of the defect must be risk assessed. To continue use, authority must be sought and given by the person on the site with responsibility for managing plant. Where the plant cannot be operated safely it must be isolated from use, parked, key removed and locked (if possible) and tagged as "out of service" or removed from site. Prior to returning to service a new pre-mobilisation checklist shall be completed, ROLES AND RESPONSIBILITIES PROJECT LEADER / WORKPLACE MANAGER Appoint a suitably qualified and experienced person to undertake plant pre-mobilisation checks Undertake Fatal and Severe Risk reviews for Plant and Equipment SUPERVISOR Ensure only operators that are assessed as competent operate plant with valid VOC Ensure operators have completed their plant pre-start checks prior to operation Monitor the implementation of this standard within the workplace ADDITIONAL INFORMATION SafeWork Australia Guide: Importing and supplying safe plant
