2 Principles in the Optus Contractor WHS management process CONTRACTOR MANAGEMENT STAGES PRINCIPLES THIS STANDARD REQUISITION Requisition WHS hazard areas are a key consideration in the selection of the appropriate procurement channel. PROCUREMENT CHANNEL SELECTION SPECIFICATION EVALUATION APPROVAL AND RELEASE Specification Hazard areas and Optus expectations in relation to these are communicated to potential providers in relevant project / task requirements (e.g. tenders, statement of works, purchase orders). Appropriate WHS specifications are included in the tender / statement of work conditions / purchase order (if required) and are reflective of Optus role (e.g. principal contractor). Evaluation The contractor submission suggests they are competent to manage the task and WHS risks of their workplace. ENGAGEMENT Engagement All parties have a consistent understanding of their mutual accountabilities as they relate to WHS risk management. Appropriate WHS specifications and obligations are included in the relevant contracts negotiated and signed by Optus. MONITORING Monitoring Relevant WHS controls are implemented and monitored at the workplace to manage the specific conditions and WHS risks of that workplace. SATISFACTORY PERFORMANCE? HANDOVER CLOSE AND REVIEW Handover Customers (internal and external) are made aware of potential risk areas and legislative expectations at handover. Close and review Contractor WHS performance is assessed and considered when using the contractor in future. Project feedback is captured and used to enhance the contractor WHS management process.

3 IMPORTANT NOTE THIS CONTRACTOR WORKPLACE HEALTH SAFETY (WHS) STANDARD HAS BEEN DEVELOPED TO ASSIST OPTUS CONTRACTORS IN THE EXECUTION OF OPTUS CONTRACTED WORKS. THE STANDARD MUST BE ENACTED IN CONJUNCTION WITH OTHER APPLICABLE OPTUS CONTRACT CONDITIONS REGARDING WHS AND THE GOVERNING LEGAL REQUIREMENTS. WHILE THIS STANDARD HAS BEEN DEVELOPED IN CONSIDERATION OF THE LEGAL REQUIREMENTS IT IS NOTED THAT WHERE ANY LEGAL REQUIREMENT EXCEEDS THIS STANDARD THE LEGAL REQUIREMENT SHALL TAKE PRECEDENCE. Purpose and scope This Contractor WHS Standard (Standard) Refurbishment and Demolition aims to ensure the minimum WHS requirements are met by Optus Contractors when undertaking refurbishment and demolition work. In addition, this Standard supports the Commonwealth WHS Act 2011 (Act) where a duty is imposed (under the Act) on a person to ensure health and safety, it requires the person to eliminate so far as is reasonably practicable, and if it is not reasonably practicable to eliminate risks to health and safety, to minimise those risks so far as is reasonably practicable to workers. Demolition work, as prescribed by the Commonwealth WHS Regulations 2011 is classified as High Risk Work and is termed as involving the demolition of an element of a structure that is load bearing or otherwise related to the physical integrity of the structure. This definition is supported by the Commonwealth Code of Practice for Demolition 2012 and further defines structures as anything that is constructed, whether fixed or moveable, temporary or permanent, and includes buildings, sheds, towers, chimney stacks, silos, storage tanks. This Standard applies to entities contracted by Optus where demolition work in undertaken, such as the demolition of structure and buildings and this work is a part of their scope of works. Roles and responsibilities Optus Contractors, so far as is reasonably practicable, are generally responsible for: Complying with all contractual obligations for ensuring workplace health and safety for demolition work. Reviewing Optus risk management processes such as Risk Register(s) and this Standard and to factor this information into their safe systems of work. Establishing accountability and authorities for refurbishment and demolition work and ensuring a competent person undertakes the demolition works and is provided with all engineering information necessary to enable refurbishment and demolition work to be carried out in safe manner. Conducting a risk assessment and documenting the outcome as a Safe Work Method Statement or similar, for demolition work and in accordance with applicable legislative requirements. Providing communication and consultation to workers and their representatives, as related to refurbishment and demolition work. Conducting inspections of demolition work to ensure compliance with safe systems of work and making such reviews / records available to Optus where and as requested. Keeping and maintaining a SWMS after the completion of work or where a notifiable incident occurs, a copy of the SWMS for a period of 2 years. Developing emergency preparedness and response provisions for refurbishment and demolition work that may include access permits, vehicle movement plans and procedures, rescue plans, external notifications and response hardware.

4 Investigating and reporting events such as incidents, accidents, dangerous occurrences and serious illness, within the required contractual period to Optus and where required, to the applicable Regulator. In addition to the above, Optus Contractors, so far as is reasonably practicable, are specifically responsible for: Ensuring the relevant authorities is notified and all necessary approvals are obtained before refurbishment and demolition work commences. Ensuring the demolition contractor has all available descriptions of the building and/or structure to be refurbished and / or demolished, including design specifications, engineering drawings, site surveys, plans of services and information on the nature and location of hazardous materials, the nature of building materials and the building or structure s relationship to surrounding properties. Verifying the location and condition of all underground tanks, vaults, wells, voids and structures, and ensure that any chemicals, volatile fuels and gases contained in them are completely removed. Ensuring the demolition contractor is licenced and provides a Work Plan and Demolition Safety Plan for the proposed demolition work, including the method or methods of demolition. The provision of safe access and egress to the work areas in which demolition plant and equipment is operating, commissioned, maintained and/or dismantled. Ensuring the necessary certification, licencing, training and competencies involving such things as plant and equipment (cranes, EWP s and hoists), propping, asbestos removal, work at heights (fall arrest systems), welding/cutting and scaffolding, PPE use etc. Ensuring personal protective equipment selected is suitable, used and maintained. Ensuring all demolition plant and equipment is installed, inspected and maintained to manufacturers and legal requirements. Work planning Work planning includes, but is not limited to: Identifying a risk management process in order to manage the hazards and risks associated with refurbishment and demolition work. Ensuring the risk management process is carried out by a competent person(s). Identifying the required licences, certification, training and / or competencies for persons undertaking refurbishment and demolition work. Identifying all Commonwealth, State, Territory and local statutory requirements for refurbishment and demolition work. Ensuring the design associated with the demolition work has been risk assessed, and appropriate documentation (developed in accordance with such provisions as local, State, National and International) and this information, as relevant, is available those persons carrying out demolition works. Planning for an Engineering Investigation Report in accordance with AS 2601 The demolition of structures. Planning for the development and securing of legal, regulator, client and utility service provider approvals / demolition licences / permissions and permits. Securing a licenced demolition contractor, where and as required, and the provision for a Work Plan and Demolition Safety Plan for the proposed demolition work, including the method or methods of demolition.

5 Procuring the required plant, vehicles, equipment and materials that meet all necessary requirements for demolition work. Planning safe access and egress routes including emergency provisions. Planning for the recycling of building waste wherever possible and the safe disposal of all other refuse and debris such as asbestos. Planning for all potential emergency preparedness and response procedures appropriate for the particular circumstance. Document control Uncontrolled when Printed. Implement work planning In order to establish a safe workplace, the implementation of the above Work Planning, may include, but not be limited to the following: No SAMPLE - WORK PLANNING CHECKLIST YES NO NA Implement the recommendations of the Engineering Investigation Report that may include such things as the type of plant and equipment is to be used, loading provisions for suspended floors, design or integrity of the structural arrangements, structural damaged or weakened structures (by fire, explosion or other causes), special buildings or structures, the type of structural system involved, component members, current load-carrying capacity of the structure, proposed methods and sequence of demolition of the whole, and / or part of, the structure. Approve, communicate and implement the risk assessments (e.g. SWMS, Plant Risk Assessment). Ensure, as required, that all services such as electrical, gas, water, sewer and other essential services (above and below ground) not required in the demolition process are disconnected, isolated, capped or otherwise controlled, at or outside the building line and before demolition work starts. Note: In each case, the relevant utility service provider should be notified in advance, and its approval or services gained if necessary. Ensure the Work Plan and Demolition Safety Plan is communicated, implemented and reviewed. Verify licences (including high risk work licences) and competencies of person(s) who undertake refurbishment and demolition work. Verify the location and condition of all underground tanks, vaults, wells, voids and structures, and ensuring that any chemicals, volatile fuels and gases contained in them are completely removed prior to commencement of the works. Ensure all asbestos or asbestos-containing material (ACM) at the workplace (or assumed present) is identified by a competent person and an asbestos register is prepared for the workplace and is kept up-to-date. Erect all appropriate fencing, overhead protection barriers and signage for the protection of the public and workers around the refurbishment and demolition work zones. Ensure all plant, vehicles, equipment and the work environment has the appropriate safety devices (e.g. audible travel/warning and reversing alarms, motion sensors, limiting devices, lights, radios, braking devices, exhaust emission units, horns, bells, guarding, interlocks, ROPS / FOPS, lighting (spot and / or hard wired), emergency stops, fire extinguishers, signage, etc.), are in good working order, in accordance with manufacturers specifications, inspected prior to operation and are suitable for refurbishment and demolition work. Ensure defective plant and equipment is isolated, and a prominent and approved tags / locks (e.g. Do Not Use / "Out of Service Tag") are fitted to the plant, vehicles and/or equipment prior to maintenance, repair, installation and/or cleaning.

6 Ensure the risk assessments are verified / audited at specified intervals and records kept. Provide protective equipment (e.g. eye and hearing protection, helmets, non-slip safety boots, sun cream and high visibility reflective clothing) to persons conducting refurbishment and demolition work while ensuring its use and maintenance. Ensure the safe removal and / or recycling of building waste and all other refuse and debris such as asbestos. Note: where asbestos waste is disposed the relevant disposal certificates must be obtained. Ensure emergency preparedness and response provisions such as plans, procedures, communications (internal and external) and hardware such as fire extinguishers are in place prior to the commencement of demolition works and they are scheduled for testing at regular intervals.
