ADDENDUM NO. 3 TO: PLANS AND SPECFCATONS FOR: NTEROR/ETEROR BULDNG REPARS SPRNGFELD AVCRAD SPRNGFELD, MSSOUR Project No. T1327-01 Bid Opening Date: September 19, 2013 (Unchanged) Bidders are hereby informed that the construction plans and/or specifications are modified as follows: Part General: A. Keyed note #4 on sheet A-701 was inadvertently left off of the reflected ceiling plans. Clarification, all suspended ceiling and grid shown on these two plans indicated by the legend shall be removed and replaced. B. Window shown on Door Type C on Sheet A-601 is a half light. C. Wall base to replaced is specified to be 4. Some existing wall base is 6. All wall base shall be replaced to match existing size. D. n lieu of replacing laminate on existing cabinet and counter as indicated on 5/A-401, contractor can opt to construct new counter in the same style as shown on sheet A-502. Drawers as shown on section 5/A502 shall be installed in new work. E. Swinging door shown at the end of the counter no longer exists and does not need to be rebuilt. F. Bottom of roof deck is approximately 10-0 from second finished floor. G. Suspended ceiling is approximately 8-6 from finished floor. H. Clarification for stairwell enclosure. The entire wall from grade level to bottom of roof deck shall be two hour fire rated. Two hour fire rating can be accomplished with the installation of 2 layers of 5/8 gypsum board on each side of the wall. Remove existing gypsum board prior to installation of new gypsum board.. Contractor has option of using metal or wooden studs. Walls to be two hour fire rated with either option. J. Abatement of floor tile in room 202 shall be done on a weekend, Sat-Mon. This room contains the servers that operate the data and phone lines to the entire building. The servers will need to be powered down, and removed by the owner during the abatement. The servers will need to be reinstalled by the owner and operational before Tuesday morning during business hours. The same scenario must be followed during the installation of the new VCT. Contractor shall coordinate with CW2 Bruce Daniel at 417-874-8423 ext. 18423. K. Coordinate with C&C group for conduit/wiring of all control work. Turn-key contractor shall provide all raceways and wiring for the control work provided by C&C group. Coordinate actuators, power supplies, temperature sensor locations, exterior sensors, etc. L. Highway A Nevada, MO 64772 RE: Asbestos Sampling Letter Report AVCRAD Facility Client-Specified Areas 2501 N. Lester Jones Avenue Springfield, Greene County, Missouri Dear Mr. Edwards: The Missouri Army National Guard contracted EW to sample suspect asbestos-containing materials (ACMs) which will be disturbed during planned renovations of specified areas of the above-referenced facility. The materials of concern consisted of ceiling tile, drywall, floor tile, cove base molding, stair tread, laminate counter top, and associated mastics located in limited areas on the first and second floors of the east side of the building. On August 6, 2013, an Asbestos Hazard Emergency Response Act (AHERA)-certified Asbestos nspector conducted sampling of these suspect materials. The inspector s credentials and a sample location point diagram are included as attachments. The samples were analyzed by a National Voluntary Laboratory Accreditation Program (NVLAP)-approved laboratory using Polarized Light Microscopy (PLM) in conjunction with Dispersion Staining (DS) techniques, as outlined in the United States Environmental Protection Agency (US EPA) AHERA Method, 40 CFR, Chapter 1, Part 763, Subpart F, Appendix A. A copy of the laboratory asbestos analytical data and chain-of-custody are included as an attachment. The analytical results indicate the floor tile and associated mastic located in the Quality Control (QC) office on the first floor contains 3% and 5% chrysotile asbestos, respectively. Approximately 877 square feet (ft 2 ) of this material is present. Additionally, the black mastic located under all of the remaining floor tile in the proposed renovation areas (except that in the Production Room and Room 203E) contains 5% chrysotile asbestos. Approximately 5,630 ft 2 of this material is present. EW can make no representation regarding suspect ACMs located outside of the inspected area.
