4 EXECUTIVE SUMMARY Old City Hall Block Buildings for Renovation Terracon Consultants, Inc. (Terracon) conducted an asbestos survey of the specific site structures located in downtown Duluth, Georgia. It is our understanding that these site buildings are planned for renovation by the City of Duluth. The purpose of this survey was to identify and sample suspect asbestos-containing materials (ACM) and provide information regarding the identity, location, condition and approximate quantities of ACM in interior and exterior building components. The survey was conducted on April 3 and 5, 2012, by Asbestos Hazard Emergency Response Act (AHERA)-accredited building inspectors in general accordance with the sampling protocols established in Environmental Protection Agency (EPA) 40 Code of Federal Regulations (CFR) 763. Two-hundred forty-one (241) bulk samples were collected from one-hundred eight (108) homogeneous areas of suspect ACM from the buildings scheduled to be demolished. Asbestos was identified in each of the five buildings covered by this assessment in the following materials: Window frame caulk Joint compound associated with gypsum board Caulking Floor tile and mastic Sheet flooring Silver paint Tar/mastic Please refer to Appendix B for specific materials and locations of the identified ACM. Terracon recommends that the identified ACM be removed and disposed of by a GA-licensed asbestos abatement contractor prior to the demolition activity that will disturb the asbestoscontaining materials identified in the site buildings. i

LAWRENCEVILLE STREET;OLD CITY HALL/CHURCH DULUTH, GEORGIA Terracon Project No INTRODUCTION Terracon Consultants, Inc. (Terracon) conducted an asbestos survey of the specific site structures located in downtown Duluth, Georgia. The survey was conducted on April 3 and 5, 2012 by Asbestos Hazard Emergency Response Act (AHERA)-accredited asbestos inspectors in general accordance with our proposal dated March 26, Interior and exterior building components were surveyed and homogeneous areas of suspect asbestos-containing materials (ACM) were visually identified and documented. Although reasonable effort was made to survey accessible suspect materials, additional suspect but un-sampled materials could be located in walls, in voids or in other concealed areas. Suspect ACM samples were collected in general accordance with the sampling protocols outlined in EPA regulation 40 CFR 763 AHERA. Samples were delivered to an accredited laboratory for analysis by Polarized Light Microscopy (PLM). The buildings scheduled for renovation include: 3087 Main Street 3097 Main Street New Dawn Theater 3530 W. Lawrenceville Street (large warehouse) Old City Hall/Church 1.1 Project Objective We understand this asbestos survey was requested due to the planned renovation of these five buildings in downtown Duluth for future development. EPA regulation 40 CFR 61, Subpart M, National Emission Standards for Hazardous Air Pollutants (NESHAP), prohibits the release of asbestos fibers to the atmosphere during demolition activities. The asbestos NESHAP requires that potentially regulated asbestos-containing building materials be identified, classified and quantified prior to planned disturbances or demolition activities.

6 Asbestos Survey Report City of Duluth Duluth, Georgia 5/4/2012 Terracon Project No BUILDING DESCRIPTION 3087 Main Street (Consignment store) This approximately 2,625 square foot (sf) one-story building was reportedly built in The exterior consists of brick with metal framed doors and windows. Interior walls consist of brick, plaster, and gypsum board. Ceilings consist of 1 x1 acoustic ceiling tiles. Flooring consists of 12 x12 floor tile, carpet, wood floors, and vinyl sheet flooring. The heating ventilation and airconditioning (HVAC) system is located in the storage room Main Street (FYI Design) This approximately 2,523 sf one-story building was reportedly built in 1910 and has undergone numerous renovations. The current tenant renovated the interior of the building approximately 2 years ago. The exterior consists of brick with metal framed doors and windows. Interior walls consist of brick and gypsum board. Ceilings consist of metal decking, gypsum board, 2 x2 and 1 x1 ceiling tiles. Flooring consists of sheet flooring, concrete, carpet and numerous types of 12 x12 floor tile. The sheet flooring in the mechanical room is original while the carpet and numerous 12 x12 floor tiles were installed by the tenant and are displays of the different types of flooring they sell. The HVAC system is located in the mechanical room. New Dawn Theater This approximately 5,500 sf one story building was reportedly built in The exterior consists of brick with metal and wood framed doors and windows. Interior walls consist of brick and gypsum board. Ceilings consist of 2 x4 suspended ceiling tiles. Flooring consists of floor tile, carpet, and concrete. Roof-top package units for the Theatre tenant space are located on the alleyway roof W. Lawrenceville Street (large warehouse) This approximately 12,376 sf one story building with a basement was reportedly built in The exterior consists of brick with metal and wood framed doors and windows. Interior walls consist of brick and gypsum board. Ceilings consist of gypsum board, 1 x1 acoustic ceiling tiles, 2 x4 suspended ceiling tiles, concrete, metal, and wood decking. Flooring consists of 9 x9 and 12 x12 floor tiles, carpet, and concrete. Roof-top package units condition the building W. Lawrenceville Street (former city hall/church) This approximately 19,200 sf two story building with a basement and attic was reportedly built in 1938 and The north side of the building houses the former city hall and a former church occupies the south portion of the building. The exterior consists of brick and stone with metal and wood framed doors and windows. Interior walls consist of concrete masonry unit (CMU) walls, brick, wood paneling, and gypsum board. Ceilings consist of gypsum board, 1 x1 acoustic ceiling tiles, 2 x2 and 2 x4 suspended ceiling tiles, and metal decking. Flooring Responsive Resourceful Reliable 2

HVAC system is located on the roof and in the attic. 3.0 FIELD ACTIVITIES The survey was conducted by Mr. David DeSavigny and Mr. Chester Kosmalski, AHERAaccredited asbestos inspectors. Copies of Mr. DeSavigny s and Mr. Kosmalski s asbestos inspector certificates are attached as Appendix D. The survey was conducted in general accordance with the sample collection protocols established in EPA regulation 40 CFR 763 (AHERA). A summary of survey activities is provided below. 3.1 Visual Assessment Our survey activities began with visual observation of the interior and exterior of the site buildings covered by this assessment to identify homogeneous areas (HA) of suspect ACM. An HA consists of building materials that appear similar throughout in terms of color, texture and date of application. Interior and exterior assessment was conducted throughout visually accessible areas of the site buildings. Building materials identified as concrete, glass, wood, masonry, metal or rubber were not considered suspect ACM. In the consignment store, 3087 Main Street, the tenant was not present on the day of the assessment and Terracon was not allowed access into a small locked storage area. The 2 nd floor mechanical room of the Old City Hall was locked on the day of the assessment and no key was available to open the door. Terracon lifted floor coverings in several areas in the buildings, where possible, and did not observe additional floor coverings/layers except where noted in this report; however, as Terracon could not assess beneath all floor coverings in all areas, there may be isolated areas of additional suspect material present beneath existing flooring. Roofing systems were not assessed per our scope of work. Additionally, Terracon assessed accessible areas of the buildings for evidence of water damage and mold growth. Terracon observations are provided in Appendix G. 3.2 Physical Assessment A physical assessment of each HA of suspect ACM was conducted to assess the friability and condition of the materials. A friable material is defined by the EPA as a material which can be crumbled, pulverized or reduced to powder by hand pressure when dry. Friability was assessed by physically touching suspect materials. Responsive Resourceful Reliable 3

8 Asbestos Survey Report City of Duluth Duluth, Georgia 5/4/2012 Terracon Project No Sample Collection Based on results of the visual observation, bulk samples of suspect ACM were collected in general accordance with AHERA sampling protocols. Random samples of suspect materials were collected in each homogeneous area. Sample team members collected bulk samples using wet methods as applicable to reduce the potential for fiber release. Samples were placed in sealable containers and labeled with unique sample numbers using an indelible marker. Two-hundred forty-one (241) bulk samples were collected from one-hundred eight (108) homogeneous areas of suspect ACM from the buildings scheduled to be demolished. A summary of suspect ACM samples collected during the survey is included as Appendix A. 3.4 Sample Analysis Bulk samples were submitted under chain of custody to Steve Moody Micro Services, Inc. (SMMS) of Farmers Branch, Texas for analysis by PLM with dispersion staining techniques per EPA s Method for the Determination of Asbestos in Bulk Building Materials (600/R ). The percentage of asbestos, where applicable, was determined by microscopical visual estimation. SMMS is accredited under the National Voluntary Laboratory Accreditation Program (NVLAP Accreditation No ). As the client requested Terracon to reduce sample costs where possible, Terracon instructed the laboratory to analyze the miscellaneous material samples utilizing the positive stop method where once the first sample out of the HA is identified to contain asbestos, the remaining samples of the HA are not analyzed to thereby reduce laboratory costs. 4.0 REGULATORY OVERVIEW The asbestos NESHAP (40 CFR Part 61, Subpart M) regulates asbestos fiber emissions and asbestos waste disposal practices. It also requires the identification and classification of existing building materials prior to demolition or renovation activity. Under NESHAP, asbestos-containing building materials are classified as either friable, Category I non-friable or Category II non-friable ACM. Friable materials are those that, when dry, may be crumbled, pulverized or reduced to powder by hand pressure. Category I non-friable ACM includes packings, gaskets, resilient floor coverings and asphalt roofing products containing more than 1% asbestos. Category II nonfriable ACM are any materials other than Category I materials that contain more than 1% asbestos. Responsive Resourceful Reliable 4

9 Asbestos Survey Report City of Duluth Duluth, Georgia 5/4/2012 Terracon Project No Friable ACM, Category I and Category II non-friable ACM which is in poor condition and has become friable or which will be subjected to drilling, sanding, grinding, cutting or abrading and which could be crushed or pulverized during anticipated renovation or demolition activities are considered regulated ACM (RACM). RACM must be removed prior to renovation or demolition activities which will disturb the materials. If the amount of RACM exceeds 10 linear feet or 10 square feet, the owner or operator must provide the State of Georgia with written notification of planned removal activities at least 10 working days prior to the commencement of asbestos abatement activities. Removal of RACM must be conducted by an appropriately accredited and licensed asbestos abatement contractor. Please note that demolition of any structure, which is defined as removal of a load-bearing member, requires a 10-day notification even when no asbestos is present. The OSHA Asbestos standard for construction (29 CFR ) regulates workplace exposure to asbestos. The OSHA standard requires that employee exposure to airborne asbestos fibers be maintained below 0.1 asbestos fibers per cubic centimeter of air (0.1 f/cc). The OSHA standard classifies construction and maintenance activities which could disturb ACM, and specifies work practices and precautions which employers must follow when engaging in each class of regulated work. 5.0 FINDINGS AND RECOMMENDATIONS Asbestos was identified in each of the five buildings covered by this assessment in the following materials: Window frame caulk Joint compound associated with gypsum board Caulking Floor tile and mastic Sheet flooring Silver paint Tar/mastic Please refer to Appendix B for specific materials and locations of the identified ACM. 10 Asbestos Survey Report City of Duluth Duluth, Georgia 5/4/2012 Terracon Project No At the request of the client, Terracon can provide a proposal for the development of asbestos abatement (removal) specifications, conduct an asbestos-abatement contractor bid-walk for obtaining abatement proposals, and provide asbestos abatement air monitoring services during and after the abatement prior to renovation activities for the City of Duluth. It should be noted that suspect materials, other than those identified during the April 3 and 5, 2011 survey may exist within the site buildings. Should suspect materials other than those which were identified during this survey be uncovered prior to or during the demolition process, those materials should be assumed asbestos-containing until sampling and analysis can confirm or deny their asbestos content. Laboratory analytical reports are included in Appendix C. 6.0 GENERAL COMMENTS This asbestos survey was conducted in a manner consistent with the level of care and skill ordinarily exercised by members of the profession currently practicing under similar conditions in the same locale. The results, findings, conclusions and recommendations expressed in this report are based on conditions observed during our survey of the site buildings. The information contained in this report is relevant to the date on which this survey was performed, and should not be relied upon to represent conditions at a later date. This report has been prepared on behalf of and exclusively for use by the City of Duluth for specific application to their project as discussed. This report is not a bidding document. Contractors or consultants reviewing this report must draw their own conclusions regarding further investigation or remediation deemed necessary. Terracon does not warrant the work of regulatory agencies, laboratories or other third parties supplying information which may have been used in the preparation of this report.
