Case Worker Goldfields Mental Health Portal Services Duty Statement POSITION TITLE: STATUS: REPORTS TO: Case Worker - Goldfields Mental Health Portal Community Support Services Full Time Program Manager 1. OVERALL PURPOSE Covering the Goldfields region this position will deliver place-based low intensity support and psychological interventions to clients. Working within the WA Primary Health Alliance Stepped Care approach, this position will match client needs to the level of intervention required and endeavour to complete regular outreach services as depicted by the needs of the service, communities and individuals incorporating community workshops, group activities and educational seminars. Low intensity psychological interventions will be performed by Case workers and counsellors, dependant on assessed need. In addition, this position will provide accessible, responsive, high-quality and intregrated community mental health services ranging from whole of population needs for mental health promotion and prevention, through to severe, persistent and complex conditions. 2. SPECIFIC RESPONSIBILITIES 2.1. Provide information, advice, service provision and referral options, to clients and their families in accordance with program objectives and agency policies including: 2.1.1. Holistic screening and assessment of clients needs; 2.1.2. Selecting and providing appropriate strategies and interventions or clients that are based on evidenced informed practice; 2.1.3. Evaluating efficacy of interventions and modifying safety, support and recovery plans where necessary; 2.1.4. Referring and assisting clients to access appropriate specialist services and support groups; 2.1.5. Case management of clients in accordance with the service requirements. 2.2. Providing information and education to clients, carers, families and other service providers; individually and through group facilitation. 2.3. Provide quality outreach services to clients in the region which will include the preparation of, and participation in; individual case work, focus groups, community events and community partnerships. 2.4. Providing and/or arranging support for families and carers where appropriate. 2.5. The promotion of mental health services and resources pertaining to the mental health and wellbeing for clients. 2.6. Perform special duties as directed by the Executive Manager e.g. assist with community development and liaison, represent the agency on public committees, give presentations to community groups etc. 2.7. Establish strong partnership and links with services that deliver support to vulnerable and disadvantaged people in the local community, including those from Indigenous and CALD backgrounds. 3. PROFESSIONAL DEVELOPMENT 3.1. Receive regular supervision and consultation as per agency policy. 3.2. Maintain professional reading and knowledge. Page 3 of 7

4. ADMINISTRATION 4.1. Develop and maintain statistical records of intervention, consultative, educational and other services provided, in accordance with Centrecare program and agency requirements. 4.2. Maintain case notes in accordance with agency policies and procedures 4.3. Maintain up to date community resource information. 4.4. Prepare reports as required in conjunction with the Program Manager. 4.5. Participate in agency planning and administration as agreed with the Executive Manager. 5. GENERIC RESPONSIBILITIES 5.1. Contribute to and be part of the organisational culture, where team work, cooperation, client service, quality, safety, confidentiality and a positive work environment are the focus. 5.5. Ensure appropriate dress at all times in accordance with the requirements of the Centrecare Dress Code Policy. 5.6. A minimum of a Diploma in the field of Mental Health, Social Work, Community Services, Counselling or equivalent 7.2. A minimum of 1-2 years experience in a mental health related service 7.3. Demonstrated knowledge and understanding around case managing vulnerable, disadvantaged and complex needs clients 7.4. Experience and/or understanding of working with remote Aboriginal communities and their unique needs 7.5. An understanding of working within a stepped care approach to mental health 7.6.
