Overdose - Med scanned multiple times - Med supplied is a larger dose than what was ordered (ie. giving half a tablet) 2. Click on the Results Column of that med 3. Correct the dose appropriately 4. Click OK and proceed with med admin process Underdose -User is not finished with scanning the ordered dose 1. Med Scanned Not An Active Order Med Scanned Not On MAW Tasklist Pharmacy Verification -Med scanned could be the wrong product, wrong dose, and not in the time range. -Med scanned may not be ordered - Order exists but no task available in the current time range -Med discontinued or inactive -Med not ordered -Linking issues between order and med scanned - Med has not been verified by Pharmacy. 2. Verify the med against the order a. If med ordered is incorrect STOP. Investigate and complete incident report. If med ordered is correct, due at this time, and appears on the MAW, bypass the med scan and notify pharmacy of alert 2. Verify status of med order a. If med order does not exist- STOP. Investigate and complete incident report b. If med order exists, determine when med is due and administer or reschedule when appropriate If med order exists, and med is appropriate for time range, bypass the med scan and notify pharmacy - If med is needed for an urgent situation (code blue, rapid response, intense pain) click Yes and proceed with med administration. - If med is not urgent, click No and wait for pharmacy verification. Early/Late Message -Med order is either overdue or is being administered too early 1. Specify a reason why the medication is being documented either early or late. 2. Complete any other necessary details for medication. Mismatch between Route & Route Not all ingredients scanned - Med scanned does not match the route entered by the pharmacy. (ie. Scanned med is ORAL and ordered route is INJ) - This is usually a premix IV solution that is made in pharmacy. 2. Contact pharmacy to see if the route/form is clinically appropriate. c. Click No to exit the window without documenting. 3. Click Cancel to return to the Med Administration Wizard. No Device Location Defined -This happens if user tries to access the icon without opening a patient s actual chart first. 1. Click OK to close the alert. 2. Open patient s chart, and then proceed to utilize the icon for medication administration. Order Update Warning -This error occurs when an update to a current order has been performed. Click ok to close the message, and re-chart medication and any other associated details as necessary.
