JOB DESCRIPTION SUPPORT WORKER Wythenshawe Learning Disability Service Hours: Accountable To: Full time or part time hours. Full time is 37.5 hours per week. Hours are to be worked flexibly on a rota which will include evenings, nights, weekends and bank holidays according to the needs of the service. Registered Manager The Role: You will provide personal care and person centred support to people with learning disabilities living in their own homes. You will enable them to enjoy a fulfilling and valued life, to participate in the community and to develop their abilities to the fullest. You will collaborate with service users, families, carers, and involved professionals to provide a coordinated and consistent service that maximises outcomes for service users. You will assist the line manager with ensuring that the service meets all the requirements and standards set by the service specification, the Commissioners, the CQC, Creative Support, and other stakeholders. 1. Care and Support 1.1 Develop and sustain warm and trusting relationships with service users. 1.2 Promote self-esteem, happiness and emotional health of service users. 1.3 Respect the person s right to privacy and ensure that their dignity is maintained at all times. 1.4 Provide respectful personal care according to the needs and preferences of individuals. 1.5 Encourage and support service users in expressing needs, views and concerns. Enable service users to make choices and decisions and to participate to the fullest in planning and decisionmaking processes. 1.6 Respect and promote the rights and entitlements of people with learning disabilities, and enable them to participate to the fullest in their communities. 1.7 Be responsive to the needs of service users in line with Person Centred Plans and respond flexibly to changing needs. 1.8 Enable service users to gain independence, confidence and competence in following areas: Social skills/relationships Personal care and hygiene Daily living skills Using community resources and facilities Social, leisure and work activities Self-organisation and coping abilities Personal safety Page 2 of 8

You will achieve this through practical assistance, support, teaching, advice, role modelling, encouragement and positive feedback. 1.9 Support people who express their frustrations and needs through behaviour that can challenge services by using appropriate strategies and interventions specified by Positive Behaviour Support Plan. 1.10 Ensure that service users receive advice, care and regular health checks to ensure their physical and mental wellbeing. Promote nutrition, relaxation, exercise and healthy lifestyles in accordance with Health Action Plans and any health management guidelines. 1.11 Support service users in developing a socially valued lifestyle, including culturally and age appropriate experiences, building on strengths, interests and aspirations of service users. Enable people to access social, leisure, work and educational opportunities. 1.12 To administer prescribed medication in accordance with the Medication Policy and Procedures. 1.13 Observe and monitor service users emotional and physical wellbeing and inform staff and involved professionals of concerns or significant changes in needs, behaviour and circumstances. 1.14 Take appropriate action in the event of emergencies, ensuring that the Registered Manager and the Support Co-ordinator are informed promptly. 1.15 Follow Health and Safety guidelines and report health and safety concerns. 1.16 Contribute to service records and individual case files. 1.17 Conduct and record financial transactions involving service users within agency guidelines. 1.18 Carry out general administrative duties, housing management tasks and services as required. 1.19 Contribute to service users person centred reviews, through verbal and written reports and by attending Support Planning meetings. 2. General Duties 2.1 Promote Creative Support, its services and activities to clients, carers, other agencies and the general public and contribute to the wider business development and reputation of Creative Support. 2.2 Accept support, supervision and guidance from senior colleagues. 2.3 Carry out all work in a manner consistent with the aims of the project and the service principles adopted by Creative Support. 2.4 Ensure that you and other staff, volunteers and students on placement comply with the following: Page 3 of 8

TERMS AND CONDITIONS SUPPORT WORKER Wythenshawe Learning Disability Service Salary: Up to 8.40ph based on experience and qualifications Point One - 8.00 per hour (No Experience) Point Two- 8.20 per hour (1yr health or social care experience / Graduate) Point Three - 8.30 per hour (2yr health or social care experience / NVQ/H&SC Diploma Level 2) Point Four - 8.40 per hour (3yr+ health or social care experience / NVQ/H&SC Diploma Level 3) Under 21 years old- 7.10 per hour 21-25 years old- 7.60 per hour Please note: Our pay date is the 15 th of each month (or the Friday before if this falls on a Saturday or Sunday). You will be paid in arrears for the previous 4/5 weeks, dependent on the month. Hours of Work: Full time or part time hours. Full time hours are 37.5 hours per week. Hours are to be worked flexibly on a rota which will include evenings, nights, weekends and bank holidays according to the needs of the service. Part time hours are to be agreed subject to a minimum of 15 hours per week Waking Nights: All waking nights attract an additional payment of 50p per hour for each night actually worked. This is paid to all staff who work nights whether on the basis of regular waking nights, periodic night duty, internal rotation or occasional night duty. Bank Holidays: An enhancement is paid for working at Christmas and New Year. We do not pay enhancements for working evenings, weekends or any other public holiday. Bonus: Employees will be awarded a one off bonus payment of 100.00 (pro-rata for employees who are contracted to work less than 18.5 hours per week) once they have successfully completed their probationary period, which is subject to the following: Attendance is satisfactory Induction Checklist is complete End Probationary Review is satisfactory Line Managers recommendation Level 2 or 3 Health & Social Care Diploma: All employees will be required to undertake and complete the Level 2 or 3 Health and Social Care Diploma in a pathway appropriate to their role. If you hold NVQ 2 or 3 in health and social care or equivalent you will of course not need to do the award again, but we may support you to undertake higher qualifications. DBS Checks: Employment will be subject to enhanced Disclosure and Barring Service checks and ISA checks. Probationary Period: Page 6 of 8

The first six months will constitute a probationary period. When this is successfully completed, employment will be confirmed. Your performance due in probationary period will be assessed against the job description, person specification and competency framework of this role Holidays: 20 days plus 8 statutory days pro rata. Part Time Work: Please note that annual leave and other entitlements are calculated on a pro-rata basis for employees working less than 37.5 hours. We take a positive view of part-time work and will consider reasonable requests for part time hours for a minimum of 15 hours per week. Sickness Policy: Creative Support operates a discretionary company sick pay benefit scheme which is for the purposes of preventing hardship during times of serious illness. The eligibility criteria and conditions for payment of Company Sick Pay (inclusive of SSP) are shown in the Employee Handbook. You may be eligible for Company Sick Pay benefits subject to compliance with these criteria as follows:- Creative Support does not pay for the first three days of any sickness absence. First six months service - Not eligible for Company Sick Pay though you may be entitled to SSP. Six months to eighteen months service - Up to a maximum of four weeks at full pay. Eighteen months plus service - Up to a maximum of eight weeks at full pay followed by four weeks at half pay. Part time employees will receive Company Sick Pay benefits as detailed above but pro rata to actual hours worked each week. Company Sick Pay benefits may be withdrawn or temporarily suspended where performance or attendance is unsatisfactory. Pension and Life Assurance: Creative Support operates an auto-enrolment pension scheme with the People s Pension. All staff under Creative Support contract are entitled to free life assurance. This is a valuable benefit which provides a lump sum equal to two times annual salary. Discretionary Benefits: Creative Support offers discretionary benefits in addition to statutory benefits. These include: Paid paternity leave Compassionate leave Enhanced maternity leave Carers leave up to 5 days per annum Employee Counselling Service: All staff, their partners and members of their household have access to an independent confidential, 24 hour telephone counselling service and to legal and financial advice. In addition, up to 6 sessions of face to face counselling can be obtained. This service is delivered by professionally qualified and supervised counsellors and is provided free of charge Hospital Saturday Fund: All employees have access to a special scheme which enables membership of the Hospital Saturday Fund on preferential rates.
