1 JOB DESCRIPTION Responsible to: The Home Manager Direct line manager: Team Leader (Day Opportunities) Work Base: The service will operate from a base at Dorrington or/and Bardney The nature of the post requires the Support Worker to travel between activities and have access to a vehicle to transport people being supported, for which a mileage allowance will be paid. SUMMARY OF ROLE To support the delivery of a range of Day Opportunities to adults with a learning disability. The service will be outcome focused and based on a person centred approach that meets individual needs and aspirations; fosters self awareness, personal growth and independence and gives each person the strongest voice with regards to decision making and lifestyle choices. The Support Worker will be responsible for providing support specifically to individuals receiving Day Opportunities. MAIN RESPONSIBILITIES, TASKS & DUTIES Support Encourage people to make choices and decisions enabling each person to take control of their own lives and increasing independence. Encourage the involvement of service user in the development of and day to day operation of the service. Actively promote effective communication, including accessible information to facilitate involvement and decision making. Contribute to the development of person centred planning, ensuring each individual s plan has meaningful and achievable goals with positive outcomes that meet aspirations as well as needs. Ensure all personal care tasks are carried out with respect for privacy and dignity and in line with the needs, wishes and preferences of the individual as defined in the support plan. Treat people with respect and maintain dignity at all times. Support people to participate in the community by accessing local facilities and wider community resources and promote social inclusion. Support people to develop, expand and maintain social networks and friendships. Support people through the transition process, gathering all the relevant information to ensure that needs led support is delivered. - 1 of 5-

2 Act as a key-workers/ co key-workers to coordinate and develop the support to the individual including Person Centred Plans, Daily Activity Plans and other agreed plans. Monitor progress of individuals and contribute to / attend reviews and multi disciplinary meetings. Contribute to the development of positive working relationships with family and carers and develop their understanding of the principles of the service delivery. Work in partnership with care managers and other professionals to improve life chances for individuals. Provide on call cover as arranged by the Team Leader, working within on call guidance. Promote healthy life styles for individuals, liaising with other professionals and agencies as appropriate. Administer medication in line with the policy and the individual s care plans. Work in line with the company s Adult Protection and Prevention of Abuse Policy and Whistle Blowing Policy to ensure that people are kept safe at all times. At all times to work within the company s Equal Opportunities Policy and Procedure. Maintain the security of the building and the safety of the people receiving support and members of the staff team. Ensure risk assessments are implemented as necessary, in line with policies and procedures. Be flexible and responsive at all times to meet the changing needs of the service, the people being supported and the staff team. Values and Attitudes Promote and practice the principles of Rights, Individuality, Choice and Inclusion as defined in Valuing People Value diversity and demonstrate a commitment to support individuals in a non judgemental way, based on the principals of anti- discriminatory practice. Treat individuals with respect, maintain dignity and encourage and enable each person to express their individuality. Performance and Development Work within Home From Home Care s policies and procedures. Work in accordance with the Home From Home Care s mobile phone and IT policies. Maintain confidentiality of all information relating to the people being supported, staff and company. - 2 of 5-

3 Work within current legislation and adhere to the GSCC Codes of Practice and staff handbook. Work in a way that meets the statutory requirements of employees under Health and Safety at Work. Act as a positive role model and behave in a manner that reflects positively on the company at all times. Enter actively into all supervision and appraisal processes. Attend regular team meetings, contributing to setting the agenda and ensuring minutes are kept. Update knowledge and skills and attend training on a regular basis to ensure that all mandatory and personal training and development needs are met in line with company and statutory requirements. Work closely with other members of the team for the ultimate benefit of the people being supported. Finance Work within allocated finances. Support individuals to manage their own money on a daily basis in line with their support plans. When handling cash, obtain and retain a receipt and give to the Team Leader together with all relevant paperwork and any unspent money accompanying paperwork, before the end of each day. Financial discrepancies must be reported to the team leader/manager immediately. Keep updated with all financial policy and procedures. Hours Be available to work flexibly on a rota basis to meet the needs of the Day Opportunities service; this includes weekends, evenings and bank holidays, where necessary to meet individual needs. Administration Ensure all records are maintained accurately, appropriately and timely in compliance with the company requirements and that records made and personal information used are in compliance with the Data Protection Act and the standards of information governance.
