Environmental Health and Safety Laboratory Safety Inspection Form Principal Investigator/ Supervisor: Department: Response Needed: Yes No Building: Inspector: Room(s): Date of Inspection: Tufts Environmental Health and Safety (TEHS) conducts annual inspections intended to assist supervisors and principal investigators in maintaining a workplace that is free of recognizable hazards, and that is compliant with applicable environmental, occupational, and fire laws and regulations. The following Laboratory Safety Inspection Form provides laboratory staff with information on issues applicable to work practices, equipment, and the work environment. Each item will be evaluated and classified as not applicable (N/A), not observed, satisfactory, and needs improvement. Furthermore, items classified as needs improvement are classified as minor (requires correction within 30 days) and major (requires immediate correction). Items noted as needs improvement will have detailed comments outlining concerns, regulatory citation, and or guidance. e, this inspection does not address issues concerning radiation, biosafety and hazardous waste, and does not replace other inspections specifically covering these subjects. Laboratory entrance area is posted with accurate and legible caution sign and other applicable signage? 2. Emergency contact number(s) of PI and other designated laboratory members are provided to TUPD? 3. Laboratory personnel have completed mandatory Annual Lab Safety Training? 4. Laboratory personnel who ship chemicals, biologicals, or dry ice have completed DOT/IATA Training? 5. Work surfaces and floors are free of debris, dirty bench top, paper/pads, spills, excess supplies, or tripping hazards? 6. Proper storage of sharp objects (scalpels, blades, etc. in dedicated areas not on open bench) 7. Damaged equipment that could cause injury is clearly marked Do Use and awaiting disposal or repair? 8. Objects stored on elevated surfaces are secure to prevent falling? 9. First aid kit is present, accessible and adequately supplied? 10. Landline Phone is available for emergency use? 11. Hoses for Bunsen burners are approved and in good condition? (No tygon or dry/cracked rubber) 12. Equipment with moving parts such as gears, belts, and fans have appropriate guards to prevent injury?

Pumps with glassware under vacuum are shielded? 15. Compressed gas cylinders are secured via a strap, chain or mounting stand? (A single chain should not support more than 3 cylinders) 16. Flashback arresters are located on all flammable gas cylinders? 17. Flammable gas cylinders are not stored beside oxidizing gas cylinders, unless in use? 18. Protective caps are in place while gas cylinders are not in use/connected? 19. Equipment (i.e Transilluminator) that generates Ultraviolet Radiation labelled appropriately? 20. Equipment, supplies, and furniture shall not be place within 18inches radius of ceiling/sprinkler heads. 5. Fire extinguishers are present, fully charged, wall mounted, unobstructed, contain designation sign, and have been inspected within the last year? 6. Portable space heaters are not present in laboratory? Electrical Safety 1. Ground fault circuit interrupters are present around wet areas such as sinks? 2. Electrical cords are not damaged or worn? (e: electrical tape on power cords does not constitute an acceptable repair.) 3. Electrical cords are not located in pathways, under rugs or draped over light fixtures? 4. Extension cords are not used as permanent wiring or connected to another extension cord? 5. Power strips are not connected to other power strips? (e: power strips should be connected directly to a wall outlet.) 6. Electrical plug adaptors called cheater plugs are not used to connect three-pronged plugs into a two-pronged outlets? 7. Faceplates are not missing on electrical outlets and switches? 8. Covers are not missing on circuit breaker panels? 9. Electrical panels and emergency shut off switches are accessible? (e: a 3ft clearance is required) 10. Exposed electrical parts over 50 volts are guarded?

Safety Plans are present for extremely hazardous materials such as poisons and carcinogens? 3. Chemical inventory containing chemical name, volume, and location is updated and provided to TEHS annually? 4. Food, drink, or cosmetic items are not stored or used in the laboratory? 5. Personal protective equipment (PPE) such as protective garments/coats, eyewear, and gloves are available to lab personnel? 6. Lab personnel are wearing appropriate ppe when working with or around specific hazards? 7. Lab personnel are wearing appropriate attire; shorts, short skirts and open toed shoes are prohibited. 8. Lab personnel remove PPE prior to leaving work area? 9. DEA controlled drugs are secured (lock/key) and are registered to an updated license? 10. Chemicals transported outside of lab are in secondary containers? 11. Chemical containers are in good condition? (Containers shall be free from leaks, cracks, or exterior contamination) 12. Chemical containers, unless used for the immediate transfer of material, are labeled with the chemical name? 13. Chemical containers are kept closed except during transfer between containers? 14. Chemicals containers that present a risk for injury are stored in cabinets and shelves that are below eye-level? 15. Cabinets and shelving used for storing chemicals are in good condition and are appropriate for the weight? 16. Incompatible chemicals such as acids and bases, or flammables and oxidizers are segregated via cabinets or bins? 17. Acids are not stored where corrosive vapors can cause damage to wiring and piping? 18. Flammable liquids are stored in flammable cabinets, away from sources of heat, direct sunlight and ignition sources? 19. Flammable storage cabinet is designed and constructed per OSHA and NFPA requirements and contain the words Flammable Keep Fire Away? 20. Flammable liquids are stored in containers of 2 gallons or less? 21. Flammable liquids do not exceed storage limit amounts according to fire code? 22. Flammable materials are stored in approved refrigerators? 23. Non-explosion/flammable proof refrigerators and freezers and Microwaves are labeled with a No Food and No Flammable Compounds sticker? 24. Peroxide forming chemicals (diethyl ether and dioxane) are dated upon receipt and when open? 25. Hazardous chemicals are not used or stored in unventilated areas such as environmental/cold rooms.

Appropriate gloves, lab coats, safety glasses, and face shield used when dispensing liquid nitrogen? 34. Liquid nitrogen is dispensed in well ventilated areas? 35. Safety shower is not obstructed, has a clear identification sign, and has been tested within the past six months? 36. Eyewash station is not obstructed, has a clear identification sign, and has been tested within the past six months? e, lab personnel should inspect monthly. 37. Chemical spill kit is available and contains absorbent berms, pads, etc.? 38. Unused mercury thermometers have been disposed? Hazardous Waste Satellite Accumulation Areas 1.
