ABOUT WINDERMERE Windermere is an independent community service organization, working across south east Victoria to help those who need it most. Since our beginning more than 150 years ago, we have been working to create a stronger, more connected and supported community. Our support comes in many forms as we work together to find the right solutions for the many and varied complex issues faced by children, families and individuals in our community. Our aim is to get in early by providing programs and services within five primary areas: Family Wellbeing to create positive behavioural changes, greater understanding and respond to violence and/or neglect Childhood Development, Education & Support including child care and services for children and adults with developmental delays and disability Assistance and support for victims of trauma, assault and/or violent crime Community Strengthening designed to respond quickly to critical and emerging needs. Homelessness services to support individuals and families to secure and maintain accommodation and to build capacity to reduce the cycle of homelessness Whilst we receive funding for some services from state and federal governments, others are funded solely by donors and sponsors to whom we are truly grateful. KEY RESULT AREAS, RESPONSIBILITIES AND PERFORMANCE MEASURES Key Result Areas Responsibilities Performance Measures Service Delivery Provide quality Early Childhood Intervention Services to children birth to school age with a developmental delay or disability, and their families. Provide service utilising the following service principles: - Transdisciplinary Key Worker model - Family Centred Practice - Strengths Based Practice - Natural environments - Evidence based practice. Ensure all targets as determined by funding bodies and as directed by Management are met. Plan and prioritise work effectively and work efficiently. Have an understanding of the service principles. Work within and contribute to the team, and demonstrate the ability to work independently. Develop and strengthen networks with key stakeholders to maximise a coordinated approach. Provide support for families to transition from service. Work flexibly and be adaptive to a changing environment. Demonstrate preparedness to work over multiple Windermere sites. Targets reached for children as specified under agreements with funding bodies and as directed by Management. On average at least 90% of work hours are provided with a combination of consumer and program specific duties. 100% of consumer related material filed within specified timeframes. Complete reporting and data collection requirements accurately and in a timely manner. Outlook Calendar is up-to-date; including consistent evidence that time is managed effectively. Attend 80% of team meetings and planning days and contribute actively. Provide service utilising Windermere service principles. Provide services at home or in the community, in individual or small group settings. Regularly contribute to the development of systems and procedures within the program to achieve excellence in service delivery. Attend events and contribute to networks. Where necessary and in a timely manner, liaise and/or refer to appropriate services. Work collaboratively and establish and maintain positive working relationships with families, professionals and key stakeholders. Complete closure documentation within specified timeframes. As industry-directed funding changes (NDIS) are implemented, meet requirements as required Carry out responsibilities within skill set as requested by Management. Windermere Page 2

Assessment and Intervention Transdisciplinary Key Worker Supervision of students (TH2 only) Organisational expectations and directives in relation to policies and procedures and the agencies mission, vision and values Provide discipline specific assessment and intervention to children 0-18 years old with a range of disabilities and their families. Utilise a range of standard and informal assessment tools appropriate to the child and family. Provide discipline specific goal-orientated support. Continual and specific planning and evaluation of the effectiveness of programs. Assess and organise the prescription of equipment/aids to enable children to develop maximum function. Actively participate in professional development activities and enhance clinical service delivery by informed best practice. Assist in maintaining discipline specific information and resources. Assist in maintaining therapy equipment supplies within budget. Provide transdisciplinary key worker support for children and their families. Support families to identify their goals, develop their Family Service and Support Plans and engage the family to identify strategies to achieve these goals. Provide supervision support to students providing regular feedback on their performance. Familiarise yourself with and adhere to Windermere s Policies and Procedures, including the Code of Conduct, Human Resources policies and guidelines and Occupational Health and Safety obligations Demonstrate dedication and commitment to work in accordance with Windermere s values and behaviours Apply appropriate informal and formal assessment tools to identify key goals and generate appropriate programs. Assessments, funding applications and reports completed in a timely manner. Respond to internal requests for consultation within specified timeframes. Effectively manage the internal referral list. Planning and evaluations completed for all intervention including individual, groups and workshops. Equipment/aid trials and funding applications completed in a timely manner. Actively research industry trends and innovation and review current literature in order to provide evidence based practice Discipline specific information and resources are maintained up to date. Discipline specific supplies are maintained within budget. Maintain required caseload of key worker families. Make contact with families within specified timeframes (in addition to consultations, liaison and group attendance). Complete and review Family Service and Support Plans (FSSPs) within required timeframes. Provide supervision and support to students completing their placement at Windermere. Ensure policies, procedures and codes are complied with at all times Ensure all interactions are undertaken in accordance with the behaviours set, as outlined in the Code of Conduct Windermere Page 3

Attend prearranged dates scheduled for supervision and agency wide training, including agency forums and on line induction and be actively involved in the 6- week performance review, 3 and 6-month probationary review and a recurring annual performance review with the relevant supervisor Work within the Windermere Well-being Framework in accordance with the principles of Appreciative Inquiry and the Strengths Based Approach. Contribute to or participate in Continuous Quality Improvement (CQI) activities of the agency, and will implement CQI strategies into their work practices Meet the challenges of change as it occurs within the service and agency Attend or complete foundation and position specific training courses set by the agency and attend or complete discretionary training as approved by the supervisor Actively assess, manage and where possible mitigate workplace risk including (OH+S), consumer related risk, reputation risk and personal risk. 100% attendance at performance reviews. Completion of induction programs within set timeframes. Exhibit workplace practice, actions and behaviours in line with Windermere s Well-being Framework. Ensure work practices comply with Windermere s Continuous Quality Improvement principles. Positively embrace and adopt change as it occurs. Ensure arrangements are made so that 100% of courses are attended or completed. Report risk to the appropriate Windermere personnel and utilise current risk management tools and procedures available. Protect the rights, safety and wellbeing of children and provide a child safe environment The employee will be expected to perform other duties outside those set in the position description as directed from time to time which are within the employee s skill, qualification, experience and competence level to meet the agencies operational needs. The Position Description may be amended from time to time at the agency s discretion. Where there is inconsistency between KPI s in this Position Description and those within the Agency Objectives, the Agency Objectives will stand. 4. KEY SELECTION CRITERIA Bachelor of Speech Pathology and current registration with Speech Pathology Australia Minimum three years experience working in paediatric practice, preferably with early intervention experience Sound theoretical knowledge with particular reference to paediatric practice Demonstrated knowledge of typical development and common conditions such as autism spectrum disorder, cerebral palsy and developmental delay Knowledge and skill in the assessment, implementation and evaluation of plans and services for children & families The ability to meet service KPI s and provide a consumer-focused, high-quality and responsive services Willingness to embrace the service principles to provide high quality Early Childhood Therapeutic services using Family Centred Practice, Strengths Based practice, natural environments and evidence based practice Experience in working within and contributing to a team, and have a highly developed ability to work individually and independently Demonstrated knowledge in the prescription of equipment/communication aids Registration with Medicare and DSS preferred and willing to apply Ability to use a computer and all relevant technology including tools that enable mobility Current Victorian Drivers License Willingness to undertake a Police Check and Working with Children Check 6. CONDITIONS OF EMPLOYMENT Terms and conditions of employment are in accordance with the Employment Agreement and Windermere Child and Family Services policies and procedures. Pre-Employment Disclosure - All appointments are subject to the candidate completing a Pre-Employment Disclosure Form regarding pre-existing illnesses or conditions that may affect their ability to perform the inherent requirements of the position and consideration of the completed form by Windermere. Medical Examination - All appointments are subject to the satisfactory completion of a pre-employment medical examination at Narre Warren Medical Centre at Windermere s expense. Windermere will be advised by the medical practitioner whether the individual is fit to perform the role. Any medical opinion obtained by Windermere in respect of an unsuccessful candidate will be destroyed at the end of the selection process. In the case of an appointee, the medical opinion obtained will be stored in a secure location. Probationary Period - The first three (3) months of your employment is a probationary period in which either of the partied may terminate your employment on 1 days notice in writing to the other party. At any time during, or at the end of the threemonth probationary period, the Employer may advise you as to whether and/or on what basis your employment will continue beyond the initial probationary period. A Qualifying Period of six (6) months applies to your employment. After the 3-month Windermere Page 5

probationary period is completed, one weeks notice is required by either party upon termination in the final 3 months of the qualifying period. Police Record Check - All appointments are subject to a clear National Police Record Check. Working with Children Check All appointments (dependent on position responsibilities) are subject to a clear Working with Children Check. The appointee is required to provide details of the Working with Children Check to Human Resources. Occupational Health & Safety To adhere to OH&S policies, procedures and guidelines and use all the necessary safety equipment provided and to report any defect in any such equipment or workplace hazards as soon as it comes to your attention.
