6 SECTION 1: POLICY STATEMENT 1.1 Statement of Intent It is the policy of Space Industries Limited to promote standards of health and safety within the workforce which will lead to the avoidance or reduction in risks to health and safety and to ensure that the best practicable methods of compliance with the Health and Welfare at Work Act 1974, and associated legislation are achieved. This document outlines the policy of Space Industries Limited in relation to the management of health and safety. Space Industries Limited is committed to managing and conducting its work activities in such a way as to ensure, so far as is reasonably practicable, the safety, health and welfare at work of their employees and others who may be affected by its business activities. This will be achieved by the following (so far as is reasonably practicable): the provision of a safe place of work, including safe access and egress; the provision of safe plant and equipment, articles and substances; the provision of safe systems of work; the provision of welfare facilities; the provision of appropriate information, instruction, training and supervision; determining and implementing appropriate preventative and protective measures; the provision of emergency plans and procedures; reporting prescribed accidents and dangerous occurrences to the Health & Safety Executive; and obtaining the services of a competent person to advise on health and safety. The detailed arrangements for achieving these objectives are set out in the main body of this document and the application of the same through a series of pro-formas where necessary. The Directors have overall responsibility for health and safety within Space Industries Limited, although Kevin Andrews takes the lead on a day-to-day basis. All employees share a responsibility with management in ensuring their own safety while at work. This Health and Safety Policy requires the co-operation of all staff, visitors, contractors and others to enable Space Industries Limited to discharge its responsibilities under law. Space Industries Limited is committed to upholding the standards outlined in this Policy. Sufficient authority and resources, both financial and otherwise, will be made available to enable staff to carry out their responsibilities in a reasonable and efficient manner. All employees will be made aware of, and have access to, this Policy Document. The Health and Safety Policy will also be available to third parties where appropriate. Employees and Operatives are encouraged to put forward suggestions for improvements to this document. The Health and Safety Policy will be reviewed at least annually and in light of amendments to legislation, experience and future developments within Space Industries Limited. 8 SECTION 3: DUTIES & RESPONSIBILITIES 3.1 Directors The Directors will be responsible for the planned implementation of effective health and safety standards within Space Industries Limited according to agreed objectives and for ensuring that health and safety standards are taken into account in planning and organising work generally. The Directors bear the ultimate responsibility for ensuring that all employed staff are given the information and training to do their job effectively and that their work targets are realistic and do not compromise health and safety requirements. The practical monitoring and implementation of the policy will be undertaken by the Managing Director, Kevin Andrews, as the Director responsible for taking the lead on health and safety issues, assisted by MSAFE Risk Management Ltd. Kevin Andrews will take the lead on advising the Board and making recommendations for health and safety management. The Managing Director, Kevin Andrews, will act as the responsible person as determined under the Management of Health and Safety at Work Regulations 1999, but has access to an external health and safety advisor (MSAFE Risk Management Ltd) to assist with his duties where requested. The Directors will ensure that: Corrective action is taken and recorded when required. Safety is a prime consideration in all planning. Adequate funds, materials, equipment and human resources are budgeted for, to meet all safety requirements. There is access to competent health and safety advice for all within the organisation. Employees are consulted on health and safety issues and are able to report any health and safety concerns. The general principles of prevention as detailed in the Management of Health and Safety at Work 1999 Regulations and Construction (Design and Management) Regulations 2007 are applied. They are familiar with the Construction (Design and Management) Regulations 2007 and company policy applicable to the work on which they are engaged and insist that the prescribed standards be observed. Ensure that regular monitoring is carried out on sites and throughout the organisation to ensure that the Policy is being complied with. Ensure that the Contracts Managers/Site Managers arrange for all reportable accidents to be reported and investigated as required. Designers appointed are competent and have adequate resources, as defined under the Construction (Design and Management) Regulations All Clients (when Space Industries Limited is acting as Principal Contractor in Construction) are made aware of their duties under the Construction (Design and Management) Regulations 2007 and a CDM Coordinator is appointed where required. They review health and safety site inspections and make recommendations directly to all Contracts Managers and Site Managers concerning health and safety. 7

9 All accidents and ill health, property damage or near miss situations arising out of the work activity are reported immediately and thoroughly investigated, recorded and reported as detailed in the Accident Reporting. An assessment is undertaken and that there is access to an adequate number of trained first aiders and equipment on each site. All employees (including those of sub-contractors) are made aware of the Health and Safety Policy and that it is available to all. They review company safety rules on a regular basis and, where necessary, recommend suitable changes. Contracts Managers/Site Managers complete site risk assessments and method statements prior to the commencement of works. Site Managers make on site amendments as necessary to the project risk assessments and method statements. In general, the forms and documents associated with the implementation of this Policy are completed by relevant persons at the relevant times. All plant and machinery operators are properly trained. Ensure that all work equipment and machinery is inspected, tested and maintained in accordance with the Provision and Use of Work Equipment Regulations, and the Lifting Operations and Lifting Equipment Regulations. All sub-contractors understand the need to comply with the site safety procedures, risk assessments and method statements. At all times, that there is an adequate supply of safety equipment and that such equipment is in good condition. All contractors appointed are competent and have demonstrated that they have allowed for sufficient resources to undertake their works in line with legislative requirements and Space Industries Limited s policies and procedures. Adequate fire risk assessment and emergency arrangements are put in place as part of site set up and reviewed throughout the project. Projects do not commence without adequate welfare facilities in place prior to the commencement of works and that a Construction Phase Health and Safety Plan is in place. 3.2 Contracts Managers Contracts Managers are responsible for ensuring the implementation of this Policy on Space Industries Limited s projects. Due to the size of the organisation, at times the Directors may also be acting as Contracts Managers. In particular, they must ensure that: They are familiar with the Construction (Design and Management) Regulations 2007 and company policy applicable to the work on which they are engaged and insist that the prescribed standards be observed. They review safety procedures and where necessary, recommend suitable changes. Pre-commencement risk assessments and method statements are completed and, where relevant, approved by the Principal Contractor prior to the commencement of works. Assist the Principal Contractor, where relevant, by supplying them with relevant information on the project and information for the Health and Safety File. The Space Industries Limited s project health and safety plan is site specific and understood by the Site Manager and sub-contractors prior to the commencement of works. Suitable and sufficient safety equipment/clothing is used by all personnel on site. 8

10 Any specific arrangements or requirements for health and safety requested by the Principal Contractor are allowed for and implemented by Space Industries Limited. Sub-contracting companies have been assessed as competent, understand the requirements of Space Industries Limited s Health and Safety Policy and any contract specific requirements and have adequately resourced to undertake the works in a safe manner in accordance with project specific risk assessments and method statements. Information regarding the presence, or otherwise, of asbestos containing materials in existing buildings to be worked upon is obtained and the results communicated to those working on site. That any areas within the site that will require emergency lighting (i.e. where there would be insufficient natural daylight to escape safely in the event of a power failure) are identified at the pre-start planning stage and suitable arrangements allowed for, or agreed with, the Principal Contractor. Any training gaps amongst Space Industries Limited s Site staff are identified and reported to the Directors. Site Managers undertake their duties in accordance with the Health and Safety Policy. Any accidents are reported in accordance with the accident reporting procedure. Any corrective actions arising from internal or external monitoring of site health and safety performance are implemented effectively. 3.3 Buyers/Procurers At present, all material orders and placement of orders with sub-contractors is undertaken through the Directors. As the organisation develops other personnel may undertake this role or a dedicated role will be formed as the need arises. Those undertaking this or a similar procurement role should ensure: Contract awards are not made on price alone but on the basis that the work will be supervised and undertaken safely with adequate resources and that the organisation can be deemed competent, in particular as set out in Appendix 4 of the CDM Approved Code of Practice. They read and understand the company policy for health and safety. The requirements of the Construction (Design and Management) Regulations 2007 (CDM) are complied with, as they apply to the procurement of materials and services supplied to the company. All equipment or materials purchased by the company are to the standards required by company policy and that they meet the requirements laid down in any Health and Safety Plan to eliminate or reduce risks. All suppliers are asked to provide full information on any hazards associated with the equipment or materials supplied and any precautions required and that this information is passed to relevant supervision and included in the Health and Safety Plan. Suppliers are informed of safe working loads of plant used for handling materials on site so that materials are delivered on suitable size loads. Sub-contractors have received lists of responsibilities and company policy statement in accordance with this policy. Rates negotiated for work carried out by sub-contractors must include all necessary safety precautions and, where appropriate, separate rates should be included for health and safety measures as defined in the Health and Safety Plan. Tenders are adequate to cover safe methods of work and suitable welfare facilities and other control measures required to meet with best and legal practice. They report on unsafe practices observed when visiting sites and non-compliance with the requirements of the Health and Safety Plan. 9

11 They set a personal example by wearing appropriate protective clothing when visiting sites. 3.4 Site Managers Site Managers will ensure that: They are familiar with the Construction (Design and Management) Regulations 2007 and company policy applicable to the work on which they are engaged and insist that the prescribed standards be observed. Forms, procedures and documents associated with the implementation of this Health and Safety Policy are completed at the prescribed frequency and distributed to the relevant persons. Presenting relevant weekly tool box talks to all site operatives and keeping adequate records of the same. Statutory notices and documents are displayed in Space Industries Limited s site office. Adequate access and egress to operatives workplaces is maintained at all times. Safe systems of work are in place and maintained. The workplace is maintained in a tidy condition and that regular clean-up periods are arranged for. The handling and storage of all tools, plant and materials is safe. First aid boxes are maintained properly. There is suitable safe access to all work at height including, staircases where appropriate and that all ladders are in good order and tied through both stiles. All persons working on behalf of Space Industries Limited, or visiting a site under their control, receive an induction without exception. Work equipment is in good order and has been inspected and certified. All portable power and hand tools are 110 volts (unless otherwise approved by the Site Manager for the site and a risk assessment is in place). Transformers, plug tops, leads, sockets and so forth are to be in good and serviceable condition. All safety records on site, including inductions, accident book, mobile tower inspections, lifting appliance certificates etc. are maintained. They cooperate with the Health and Safety Advisor and action any recommendations made in site inspection reports. All accidents are recorded and reported to the Contracts Manager and/or Director and where relevant Principal Contractor. All persons on site, site operatives, subcontractors, and visitors wear hard hats, eye protection, high visibility clothing, protective footwear and other equipment as may be required by conditions pertaining from time to time. Information required for the Health and Safety File is retained for forwarding to the Principal Contractor. Adequate provision for fire and emergency arrangements have been put in place and reviewed throughout the project. Welfare facilities are in place (compliant with the CDM Regulations) prior to the commencement of works on site and maintained in good order. Operatives do not start work on site without suitable and sufficient risk assessments and method statements in place that where necessary have been approved by Space Industries Limited s Client. They report any safety concerns or resources required to the Contracts Manager/Director. 10

12 3.5 Operatives (Employed and Sub Contract) All site operatives of Space Industries Limited and those working on their sites are asked to note that the following is your responsibility:- You must not carry out any tasks which you feel you are not competent to carry out, or one which involves unreasonably high risks. Report to the Site Manager before commencing work on site and receive a site induction. Take care of your own safety, health and welfare and that of any other person who may be affected by your acts or omissions while at work. Site operatives are reminded in particular that they have a duty by law to use all safety equipment and protective clothing as prescribed by Space Industries Limited and the Principal Contractor. In simple terms, if they fail to use such equipment then, in the event of an accident occurring, they may very well prejudice their right to recover any damages. Understand your responsibility under section 7 of the Health and Safety at Work Act to ensure you carry out your work in a safe manner so as to prevent any risk to yourself or to other persons whom may be affected by your actions and to cooperate with Space Industries Limited and the Client at all times. To comply with safe systems of work, procedures and controls as detailed in the site specific risk assessments and method statement. Report to the Site Manager to receive permits to work for any hot works, work on services and similar. Report to your Site Manager or supervisor without delay any defect in the plant, equipment place of work or system of work which might endanger safety, health or welfare of which you become aware. Report any deficiencies in safety equipment; in particular, any omission in protective gear, goggles, first aid equipment or whatever. Nobody should intentionally or recklessly interfere with or misuse any appliance, protective clothing, convenience or equipment for securing the safety, health and welfare of persons arising out of work activities. Use correct tools and equipment for the job and keep tools in good condition. Safety helmets, high visibility vests/jackets, and safety boots must be worn at all times on Space Industries Limited s projects. You are encouraged to make suggestions and raise concerns. Indeed, this document is in itself an invitation to consultation. Read it in full and if you have anything to contribute then please advise any member of Space Industries Limited s management team. There are suggestion boxes kept in the Space Industries Limited s site office. Alternatively, you may contact any member of Space Industries Limited s management team. To avoid any action which would be a source of danger to yourself and/or others and ensure that you do not engage in any acts of personal injury, etc; and Not to operate winches, scissor lifts, MEWP s, telehandlers or any other equipment for which you do not have the appropriate competence and training. Keep your work area tidy and dispose of waste in the correct manner using the facilities provided. Respect welfare facilities provided and keep them in good order. 11

13 Not to smoke in any internal area and only smoke outside in areas defined by the Principal Contractor. 3.6 Plant Operators All Plant Operators will: Carry with them in the relevant machine proof of training. Ensure that the certificates of inspection for the machine and any chains or other lifting gear are in place. Carry out his/her own work in a safe manner and take any extra necessary precautions when working on or near public roads. Ensure that the fail safe quick hitch lock mechanism is engaged at all times on excavators to prevent accidental release of the bucket. Be particularly vigilant of overhead electric power lines when tipping / slewing. Obey the rules of the road and site speed limits. Carry out visual inspections of their machine at the start of each working day. Ensure that vehicles are well maintained and report any faults. Complete daily checks and formal weekly plant inspections and report any faults to the Site Manager for action. Inform the management of Space Industries Limited at least 6 weeks before any training is due for renewal. 3.7 Health & Safety Advisor Space Industries Limited employs the services of an external competent advisor to assist in the provision of health and safety advice. The Health and Safety Advisor, where directed, is responsible for: Assisting the Directors / Contracts Managers and Site Managers in undertaking their duties as requested. Promoting health and safety awareness. Where requested, undertaking site inspections and audits to report on health and safety performance. Undertaking accident investigations, when requested. Attending Space Industries Limited s health and safety management meetings where requested. Provision of reactive telephone support to all staff within Space Industries Limited. 12

14 SECTION 4: SAFE WORKING ARRANGEMENTS 4.1 Resources Management at Space Industries Limited recognises that, for the effective implementation of the safety procedures and policies laid down in this Health and Safety Policy, adequate resources and funding must be made available. Management undertakes to ensure that adequate numbers of suitably trained staff are available to undertake all work activities carried out by the company. The Management will provide resources for the on-going monitoring of health and safety in the organisation and for the provision of information and training of all Site operatives in health and safety. All control measures shall be reviewed to ensure that they take account of the general principles of prevention in accordance with the Construction (Design and Management) Regulations 2007 as set out below: Increasing use of systems to manage risks Eliminate the risk Evaluate unavoidable risks Combat risks at source Adapt the workplace to the individual Adapt the workplace to technical progress Replace dangerous articles, substances or systems of work Development of a prevention policy Collective protective measures over individual measures Training and Instruction Increasing reliance on Personal behaviour 13

15 4.2 Risk Assessments The Directors will ensure that the hazards associated with the activities Space Industries Limited undertakes or their employees are exposed to are identified and control measures put in place to mitigate the potential effects of the same. Each Site Manager is responsible for ensuring risk assessments are undertaken in relation to the use of a construction site and the construction works. In some instances, these risk assessments will be completed by specialist sub-contractors. It is nonetheless the Site Manager s responsibility for ensuring that works do not commence until adequate risk assessments are in place for the site and the particular activity /task. The Contracts Manager should monitor this and assist by ensuring that the relevant sub-contractor supplies relevant risk assessments and method statements upon appointment. The Site Manager must be satisfied that the risk assessment adequately addresses the known and foreseeable risks associated with the works. If unsure, they are able to consult with the Contracts Manager, Directors and external Health and Safety Advisors. A form for reviewing risk assessments and method statements forms part of this policy. Whilst many of the risk assessments related to the tasks on site will be completed by others, the Site Manager will maintain responsibility for undertaking risk assessments associated with the physical site, its general organisation and in relation to provision of a safe workplace for site operatives. As such, examples of risk assessments that would normally be undertaken by the Site Manager would include, site security, temporary services, traffic management, work at height, overhead services, any site specific hazards etc. Copies of the risk assessments should be retained on site by the site manager. Risk assessments should be read and understood by all contractors prior to commencement of their particular works. Sub-contractors are expected to communicate to the Site Manager and Contracts Managers any specific action that Space Industries Limited is required to undertake; for example the adaption of scaffold or procurement of a contract lift. Under certain legislation, specific risk assessments must be undertaken in relation to the following: Work at height. Noise. Use of substances hazardous to health. Manual handling. Personal protective equipment. Vibration. Young persons (up to 18). Site Managers and Contracts Managers may seek the assistance of the External Health and Safety Advisor to complete the above assessments. The Directors will undertake Risk Assessments for activities associated with the general company operations and their offices. The results of the same will be communicated to employees affected. 4.3 Training & Induction Inadequately trained site operatives are potentially hazardous to themselves and their co-site operatives. Management at Space Industries Limited shall identify the training needs of their staff and ensure they are fulfilled. It is the policy of Space Industries Limited that all new personnel shall receive safety training as part of their induction and that this training is signed off by all site operatives. Site Inductions shall be provided by the Site Manager for all operatives on all construction sites. All operatives will be required to complete the site induction form following their site induction as a record of the induction taken place and confirming that they have understood all information and control measures explained to them during the same. 14

16 4.3.1 Records All the safety training provided will be recorded by Management. Space Industries Limited will keep training records detailing the following: Nature of training Name of the employee being trained. Date of training and amount of time taken. Training details and methods used. Signature of the trainer and employee to ensure that the training has been carried out, documented and understood Training Space Industries Limited shall provide the following training: Site specific induction training during construction projects; Toolbox talks on issues relevant to site activities; and First aid, trade, task and operation specific training, as required. The Directors are responsible for ensuring all Site Managers receive adequate instruction and training. At the time of writing, the industry standard health and safety qualification for the Site Manager on sites where Space Industries Limited are Principal Contractor is the CITB 5-day Site Management Safety Training Scheme. To supplement this, or on sites where Space Industries Limited are not Principal Contractor, operatives trained in the CITB 2-day Site Supervisors Safety Training Scheme will be in place. In addition, Space Industries Limited is responsible for ensuring that a trained first aider (HSE approved 3-day course) is available on site at all times when in excess of 5 persons are at work or the risks determine that a first aider is required. When fewer than 5 persons are working, or the risk of injury is low, Space Industries Limited will provide a trained Emergency First Aider (HSE approved 1-day course). The Contracts Manager is responsible for ensuring arrangements are in place for providing suitable and sufficient information, instruction, training and supervision in order that staff on their site may undertake their tasks and activities without unacceptable risk to their health, safety and welfare. The Site Manager is responsible for ensuring that all site staff undergo regular tool box talks and site induction prior to commencement of works on site. Where a member of staff identifies a training need or knowledge gap that could compromise the health and safety of themselves or anybody else, the Site Manager should be consulted in the first instance when on site, or if an issue associated with the wider company or office arises, the Directors can be approached directly Safety Induction New Company Staff The Directors, or others as instructed by them, shall explain to the new employee what he/she will be required to do and to whom he/she will be directly responsible and shall ensure that the employee completes the Health and Safety Induction form for new employees. The following tasks will be incorporated into the induction: Show the new employee where the Health and Safety Policy is kept, explain its purpose and ensure that the employee is aware of his/her responsibilities. Warn new Site operatives of any disciplinary procedures in the event of neglect of Company Health and Safety policies. Establish the need for and arrange specific training to be given to an employee (e.g. manual handling, scaffold inspection, abrasive wheels, first aid). 15

17 Show the new employee the location of the First Aid Box(es) and explain the procedure in the event of an accident, in particular, the necessity to record accidents, however trivial they may appear at the time. The fire and evacuation procedure should be explained and assembly points shown to the new employee. Train the new employee in any relevant risk assessments and method statements. This training must be signed off. If the operative or staff member is under 18, a specific Young Person Risk Assessment must be undertaken by the Director/Contracts Manager or Site Manager. Warn new employee of any potential dangerous areas of operation on site or in the workplace Site Safety Inductions All operatives must receive a site specific induction in accordance with the Construction (Design and Management) Regulations 2007 prior to commencing work on site. The induction shall be recorded and will cover the following issues; a) senior management commitment to health and safety; b) the outline of the project; c) the individual s immediate line manager and any other key personnel; d) any site-specific health and safety risks, for example, in relation to access, transport, site e) contamination, hazardous substances and manual handling; f) control measures on the site, including: any site rules; any permit-to-work systems; traffic routes; security arrangements; hearing protection zones; g) arrangements for personal protective equipment, including what is needed, where to find it and how to use it; h) arrangements for housekeeping and materials storage; i) facilities available, including welfare facilities; j) emergency procedures, including fire precautions, the action to take in the event of a fire, escape routes, assembly points, responsible people and the safe use of any firefighting equipment; k) arrangements for first aid; l) arrangements for reporting accidents and other incidents; m) details of any planned training, such as toolbox talks. 16

18 4.4 Safety Consultation Consultation Information concerning safety matters, including the results of risk assessments, which may affect any or all staff, is communicated to all employees at induction and through regular toolbox talks. Any matter concerning safety raised by any member of the company or any member sub-contracted to the company is investigated by the Managing Director, Kevin Andrews, so that action can be taken as appropriate. Staff are to communicate any health and safety concerns associated with Space Industries Limited s activities, a site, site activities or equipment to the Site Manager. Where possible, the Site Manager will rectify the problem and inform the Contracts Manager of the problem and the action taken. Where further assistance is required in order to rectify the problem, the Contracts Manager should consult the Directors and, as necessary, the External Health and Safety Advisor. To allow effective communication and consultation on site weekly tool box talks will be held by the Site Manager and will be attended by all operatives and sub-contractors on site. Record of the tool box talks shall be completed by the Site Manager. This tool box talk meeting shall be on a specific topic but also an open forum where operatives are free to consult on any health and safety issue. Periodic meetings are undertaken with Directors, Contracts Managers and External Health and Safety Advisor where health and safety issues form part of the agenda. In addition to the above, it is proposed that all employees are formally invited to an annual health and safety meeting with senior management to discuss all items relating to health and safety and any other items that employees wish to raise during the course of the meeting. The minutes from such meetings and any other safety issues that need to be communicated will be displayed on the notice board in the office and in addition to this ed to employees. For employees and other parties that do not have access to a hard copy will be passed to them with their wage slips Review of safety policy The Managing Director, Kevin Andrews, is responsible for ensuring that the Health and Safety Policy is reviewed at least annually or following any significant changes in the organisation, legislation or procedures. A new version is issued, if necessary, and any changes are brought to the attention of the employees and contractors. Records are kept of any changes or amendments on an amendment list. The amendment list, along with any revised or new pages, is then circulated to all relevant personnel. One electronic copy of all obsolete documentation is retained. 4.5 Housekeeping All staff have a responsibility to ensure their area of work is arranged so as to avoid risks to health and safety. Good housekeeping practices include: Regular disposal of combustible waste in order to reduce the risk of fire. Ensuring combustible materials are not kept within close proximity to sources of heat. Ensuring materials are stacked safely on a load bearing and even surface in order to eliminate the risk of collapse. Ensuring cables are routed so as to avoid a tripping hazard and reduce the risk of their damage. 17

19 Ensuring spillages are cleaned up immediately. Ensuring flammable substances are stored safely away from any ignition sources. Ensuring safe means of access and egress are maintained at all times. The Managing Director, Kevin Andrews, will ensure that the site is regularly reviewed and that good housekeeping is undertaken as detailed above. The Directors will also regularly visit remote offices/sites where Space Industries Limited s employees are working and ensure that the same standards are maintained. 4.6 First Aid A first aid kit is provided at the head office suitable for the number of persons likely to be present at any one time. There is a qualified first aider designated at the head office and Kevin Andrews will ensure that there is always a designated trained person fulfilling this role. First Aid Kits shall be provided for use on site with at least 1 qualified first aider for every 50 personnel on site. Where there is no office on site, first aid kits will be carried in all operatives company vans. Space Industries Limited will ensure that First Aid training has been provided to all nominated first aiders on sites where they are working. This training will be the HSE approved three-day First Aid at Work course where there are 5 or more persons on site (or the nature of the works is deemed high risk) and shall be reviewed and updated every three years or earlier when necessary. For additional cover and on low risk sites, appointed persons who have attended a oneday first aid course may be utilised. The names of First Aiders will be displayed throughout the site. Contents of the first aid boxes are checked as part of the Site Manager s duties and items reordered as necessary. The suggested content of the first aid boxes is detailed below, although in practice Space Industries Limited will purchase fully stocked first aid boxes and keep the same up to the same stock levels as items are used. 20 Supplies of analgesics, pills or medications must not be kept in First Aid Boxes. Individual Site operatives who believe they might have a need of these items must be responsible for their own supplies. 4.7 Accidents & Dangerous Occurrences The Managing Director, Kevin Andrews, will be responsible for reporting accidents, illnesses and dangerous occurrences as necessary to the Health and Safety Executive under the Reporting of Injuries, Disease and Dangerous Occurrences Regulations (RIDDOR). This can be undertaken on line at For fatal and major injuries call The Managing Director, Kevin Andrews, may delegate this responsibility to another Director/Responsible Person, depending on the resources available. Staff are to report all accidents or serious illnesses occurring at work as a result of work activities to their line manager. The aim of accident reporting is to identify where measures can be taken to prevent reoccurrences. Accident data also provides a useful health and safety performance indicator and can identify training gaps or areas where the Policy needs to be reviewed. If an accident is fatal or very serious, then an external health and safety advisor will be immediately advised to arrange an external investigation. If the accident is fatal, the scene of the accident must be left undisturbed for at least three days after notice has been given, other than for rescue purposes. The root course of any accident/incident or near miss is to be identified and Space Industries Limited are to undertake the following as a minimum: 1 Reinforce the existing control measures where they are sufficient but failed due to management or individual diversion from the existing control measures. This may require reinforcement of existing control measures, re-training of employees and a programme of reinforcement training to existing and new employees as required. 2 Introduce new control measures that address the root cause of the incident and undertake a programme of training and communication to ensure that all employees are aware of this new control measure. Relevant third parties are to be informed of these new control measures as appropriate. 4.8 Fire and Emergency Arrangements Each construction site should have a specific Fire & Emergency Plan and Fire Risk Assessment. The Contracts Manager will ensure that a Fire Risk Assessment is produced for sites where Space Industries Limited is acting as Principal Contractor. A copy of this should be clearly displayed in the Site office and the notice board in the welfare cabin. All staff must familiarise themselves with the Site Fire & Emergency Plan. A Fire Risk Assessment must be in place for all Construction sites. This will either comprise an adaption of the clients existing Fire Risk Assessment or Space Industries Limited will arrange to produce a bespoke Fire Risk Assessment for the works. The Managing Director, Kevin Andrews, is termed the Responsible Person under the Regulatory Reform (Fire Safety) Order He will ensure that a Fire Risk Assessment is in place for any premises from which Space Industries Limited operates and that any hazards identified in the same are addressed. Management resources and arrangements shall be put in place to address the findings of any fire risk assessments. 4.9 Display Screen Equipment Prolonged use of Display Screen Equipment (DSE) can cause several adverse health effects: Repetitive strain injury (RSI) can cause temporary cramps, aches and pains in the hands, wrists, arms, neck, shoulders or back. Usually, these disorders only occur after long uninterrupted sessions of DSE work and soon disappear but in a few cases can become permanent and even disabling. 19

21 Stress / fatigue and headaches are more likely to result from poor job design, work organisation or user interaction rather than the physical aspects of the work station. However, stress / fatigue and headaches may result from screen glare, poor image quality, a need for spectacles, stress arising from work demands, anxiety about new technology, working for prolonged period without a break, poor posture or a combination of the above. Skin disorder can be caused in rare circumstances. A small number of people have suffered from skin irritations and rashes after prolonged work. This is thought to be caused by a combination of dry air caused by heat from the DSE and static electricity, although there is no conclusive evidence to support this. The Managing Director, Kevin Andrews, is responsible for ensuring risk assessments are undertaken for all users as defined by the Health and Safety (Display Screen Equipment) Regulations A user would normally use DSE for continuous spells of an hour or more on a daily basis. Space Industries Limited will arrange for eyesight tests to be undertaken for users who request it and meet the basic cost of any corrective appliances or other equipment as required by legislation Welfare Facilities The following welfare facilities are to be provided and maintained by Space Industries Limited to meet the minimum requirements of Workplace (Health, Safety and Welfare) Regulations Suitable supplies of drinking water at convenient locations. Facilities for changing and storing clothes. Facilities for boiling water and a means of preparing hot food (microwave). Facilities for rest and to eat meals. Washing facilities (with hot and cold water, soap and towels or equivalent). Suitable number of WCs as detailed in table below: Number of people at work Number of water closets Number of wash stations 1 to to to to to These facilities are currently provided at the head office from which Space Industries Limited operates. The facilities have been reviewed and the arrangements are adequate and sufficient for reasonable future expansion in the premises. Site welfare facilities The following welfare facilities will be provided on all sites where Space Industries Limited is working in accordance with The Construction (Design and Management) Regulations These arrangements will be made by Space Industries Limited where they are acting as Principal Contractor or contracting directly to a client. Where they are sub-contractor, Space Industries Limited will review with the Principal Contractor to ensure that these facilities are provided: Mess area with chairs with backs, tables, drinking water, facilities for boiling water and a means of preparing hot food (microwave). Drying room/area with heaters. Area to store clothes and change. Washing facilities (with hot and cold water, soap and towels or equivalent). Toilets that where practical discharge into the mains sewer. Access to hot and cold running water for washing with soap and a means of drying hands. 20

22 Facilities may be provided through proprietary welfare cabins or use or adaption of existing facilities at a clients premises. The contracts manager will ensure that these arrangements are in place prior to commencing works on site Personal Protective Equipment It is the policy of Space Industries Limited to engineer out all hazards where possible and only use Personal Protective Clothing/Equipment (PPE) when further risk reduction is not feasible. Space Industries Limited will supply all relevant personal protective equipment e.g. safety shoes, hardhats, safety gloves etc. as necessary to their Site operatives. All sub-contractors are expected to supply their Site operatives with the necessary PPE. All protective equipment purchased by the company will be to approved British Standards. It is essential that all protective measures are correctly selected, and users are instructed in their correct use. In this respect, training will be provided as found necessary. Whenever possible, appropriate manufacturer's guidance will be heeded. All personal protective equipment and clothing shall be used in a safe manner and maintained in a clean and serviceable condition. All equipment and clothing must only be used for the purpose they were intended. All faulty or damaged protective equipment or clothing must not be used and shall be reported and replaced as soon as possible. Staff should not interfere with or alter any item of protective equipment or clothing. Supervisors will inform any person observed carrying out any procedure which requires the use of protective clothing or equipment of both statutory and company policy requirements and such persons will be instructed not to continue working until protective clothing or equipment is obtained and used. This applies not only to Site operatives but also to contractors. All management and supervisory staff will set a good example in the wearing of personal protective equipment Third Parties affected by Space Industries Limited Space Industries Limited applies the same level of health, safety and welfare standards to visitors as it does to Site operatives. All short-term visitors are the responsibility of the person they are visiting. In an emergency situation, they will take instructions from that employee as necessary. Any contractors sub-contracted by Space Industries Limited must report to site management prior to works commencing. When they are working unaccompanied, they must establish their designated emergency marshalling point before commencing work on site. Space Industries Limited will plan and manage their works so that third parties are not affected or exposed to hazards associated with the works through excluding them from the work areas and where close to boundaries and adequate exclusion zone is established. Where this is not possible detailed consultation will take place with the relevant third parties to put alternative control measures in place Non-English Speaking Workers A site specific assessment will be undertaken prior to allowing non-english speaking workers to work on site. Non-English speaking operatives will not be permitted to work in isolation and will be part of a crew in agreed locations or activities where at least one person has reasonable working English and suitable levels of supervision will be maintained. Space Industries Limited are to be sure that the operative understands what is required before allowing them to work on site Young Persons Young persons may be at more risk due to lack of experience, learned skills and in some cases physical ability. The Contracts Manager is responsible for ensuring all subcontractors do not allow staff below the age of 18 to work on Space Industries Limited sites without prior notification. 21
