Introduction South Staffordshire and Shropshire Healthcare NHS Foundation Trust recognises its duty to provide a safe internal and external environment as far is reasonably practicable, for all employees, service users, contractors and visitors. 2 Purpose The purpose of this standard operating procedure is to detail arrangements and provide a process for effectively managing working at height risks with the aim being to minimise the number of incidents occurring and injuries sustained to staff, contractors, service users and visitors. This procedure will enable managers and staff to comply with Health and Safety Regulations by identifying hazards, assessing the risks and then controlling managing and mitigating those risks so far as is reasonably practicable and introducing suitable and sufficient means of managing work at height safely. The Trust is committed to reducing accidents and exposure to hazards in the workplace and to reduce the likelihood of any person falling a distance likely to cause personal injury. 3. Scope This procedure describes the procedures which should be followed and the factors, which should be taken into account by all employees when dealing with aspects of working at height in the workplace. The procedure also addresses the trust s legal and statutory obligations. Compliance with this procedure and the use of safe working practices for work being carried out at heights is mandatory for all Persons. 4.0 Working at Height Functional Detail The Trust's Department shall implement a management regime to control and Operate the Safe Working at Heights within and on the Trusts properties / Estate and any other properties they may be responsible for. 4.1 Roles and Responsibilities 4.2 Duty Holder The Chief Executive as the Duty Holder, ensures that the Trust has an effective Health and Safety management regime to ensure safe working at Heights 4.3 Designated Person This is a requirement of Health and Safety Regulations. The designated person is an individual appointed by the Trust (a board member or a person with responsibilities to the board) who has the responsibility for Health and Safety for the premises who as a duty under the health and safety at work act 1974 to prepare and issue a Page 2 of 25

3 general policy statement on health and safety at work, including the organisational arrangement for carrying out that policy. 4.4 Responsible Persons Operational The Director of Facilities and shall appoint suitably trained people to be the Responsible Persons for the management of working at Heights 4.5 Deputy Responsible Persons Operational The Director of Facilities and shall appoint suitably trained people to become Deputy Responsible Persons for the management and supervision of competent persons when working at Heights 4.6 Supervising Persons Operational Supervisors shall have awareness training for working at Heights 4.7 Competent Persons both operational Staff & Contractors Competent persons must be trained and registered as being able to work at Heights 4.8 Responsible Persons Capital works The Director of Facilities and shall appoint suitably trained people to be the Responsible Persons for working from heights for Capital works 4.9 Supervising Officers Capital works The Director of Facilities and shall appoint suitably trained people as supervising officers who are able to issue a Permit to Work for working at heights 4.10 Hazards Associated with Roof - work The hazards associated with roof-work can be sub-divided the four sections given below, but in general the notes below should be applied to all roof-work where appropriate. For the purposes of this document working at heights will be defined into FOUR groups Access onto Roof Access across Roof Working on Roofs All other Works Involving Heights See the relevant sections depending on the works being carried out. Work at heights is defined as work in any place, including a place at or below ground level, where if the measures required by the regulations were not taken a person could fall a distance and is liable to cause personal injury. This includes having to obtain access and egress from such place of work, except by staircases in a permanent workplace. Page 3 of 25

4 There is no longer a stated height at which specific measures must be taken to control risks, a risk based approach must be adopted for all height work. The 2m rule has been removed, however work at heights of less than 2m it is recommended that a sensible and pragmatic approach is taken Hierarchy of Control Quotation from the regulations; You must do all that is reasonable practical to prevent anyone falling. Ensure - all work at height is properly planed and organised - all work at height takes account of weather conditions that could endanger H+S - those involved in work at height are trained and competent - the place where work at height is done is safe - equipment for work at height is appropriately inspected - the risks from fragile surfaces are properly controlled - the risks from falling objects are properly controlled Duty Holders Must Avoid work at height where they can Use work equipment or other measures to prevent falls where they cannot avoid working at height; where they cannot eliminate the risk of a fall, use work equipment or other measures to minimise the distance and consequences of a fall should one occur 4.12 Permit to work Permits-To-Work Contractors The Trust operates several permit-to-work systems for work requiring a degree of control from the Trusts Management Team. There are three permits for working at Heights these are - Page 4 of 25

5 Access the Roof Access across the Roof Permit-to-Work: Heights 4.13 Authorisation to Issue Permit The Permit to Work is the control measure for persons working at heights. Only certain persons are able to issue a permit to work at heights these are Responsible Person operational Deputy Responsible Person operational Supervising Officer operational Responsible Persons Facilities Deputy Responsible Persons Facilities Responsible Person Capital projects Project managers Supervising Officers Capital Projects Other permits to work may also be required for certain works or situations such as Hot works, Asbestos, Electrical safety these shall be identified before work commences Method Statements and Risk Assessments Method statements and risk assessments shall be completed prior to any work being undertaken. In conjunction with the Control Systems (i.e. Permit), written statements of the Method in which the activity is undertaken through to completion of the task shall be provided and in place. This Method Statement used with a Risk Assessment and discussions with the Supervising Officer will all go together to enable a Permit to be issued to carry out the works. The Work at Height Regulations 2005 require all persons involved in both the planning and carrying out working at height to be competent. Risk Assessment - Factors to consider Risk Assessment will have an important function in the planning of work. The factors to be considered in such a risk assessment include: The work activity (ie the proposed work at height) The equipment to be used (ladders, steps, scaffold etc) The duration of the work The location of the work activity (to determine hazards present) The work environment (including weather, lighting and space) Page 5 of 25

6 The condition and stability of existing work surfaces. The physical capabilities (and competency of the workers) Emergency procedures required in the event of a fire, incident or accident 4.15 Inventory of Plant and Equipment & Inspection of steps / ladders /scaffold / plant and equipment All the Trusts equipment and plant shall be identified, labelled and placed on an inventory There shall be an equipment / plant registrar this will be maintained and kept up to date and will include records of all the regular inspections, testing and maintenance of the items. There shall be a labelling system to mark all equipment and plant showing that the item is safe & able to be used and is within its service date Inspections Once a piece of access equipment has been installed but before it goes into general use, a full inspection should be carried out to ensure it has been installed correctly and is safe to use 4.17 Access onto Roof Working on roofs is one of the most hazardous activities undertaken by staff and Contractors alike. Injuries resulting from falls off or through roofs are usually of a very serious nature and quite often are fatal. Maintenance work on roofs in particular causes a high percentage of accidents, probably due to the work usually being of short duration. But it only takes one slip and a matter of seconds for a person to fall. Similarly falling tools and material can cause hazards to personnel and persons below Initial Access to Roof Level There are many ways to obtain access to a one, two or three storey roof level, however only three systems will be acceptable to the Trust. Any contractor / Staff found using another method not approved by the Trust may be disciplined Permanent Access Ways Permanent Access Ways are regarded as the Trust s own permanently fixed ladders and stairways. These Access points are controlled by the Trusts Department. No access is permitted through these without prior permission being given by the Supervising Officer Temporary Ladder Access Sometimes the use of the Trusts Permanent Access Ways is not practical due to location, disruption etc. Therefore the Trust will permit the use of Temporary Ladder Access. 7 However this will only be for single storey access and for short duration works (maximum 1 day). These Temporary Ladder points will be controlled by the Contractor with permission to use the system given by the Trusts Supervising Officer Temporary Scaffold Access When the use of the Trusts Permanent Access Ways and the contractors own Temporary Ladders are not practical. The Trust will only permit the use of a Temporary Scaffold for Access. This can be for any height on the site and for long duration. This Temporary Scaffold will be controlled by the Contractor with permission given by the Trusts Supervising Officer, for which they are working Control Systems and Supervision The Contractor will have overall responsibility for the system being used The equipment required to achieve the work, The safe use of that equipment, to ensure The equipment is fit for its purpose, and that the users have been trained in its proper and safe use. The Trust along with the contractor will have dual responsibility to ensure that works being carried out, are being done so as to meet the Trusts site safety and security procedures. Therefore before any access to the roof level is permitted and before any work commences the Supervising Officer is to be advised of the method of working, by the receipt of a written Method Statement and Risk Assessments if these are acceptable the Permit will be issued. The recommendations laid out on the Permit are then to be adhered to throughout the duration of the works as part of the contract Access Across Roof All works on roofs, whatever its nature or extent, require a safe access to the area of works for operatives and materials. Once access onto the roof has been established, a safe passage across the roof for all users must be determined. From Permanent Access Ways Designated Walkways are to be utilised as much as possible. If access is required to areas not covered by these walkways suitable alternative/temporary arrangements to safeguard access across the roof to the working areas shall be provided. Under no circumstances are Designated Walkways to be altered, moved or removed to suit the access for works, unless previously agreed by the Supervising Officer. Page 7 of 25

8 From Temporary Access Ways When Temporary Access Ways are in place the area between the access point and working area shall be safe to use in all conditions 4.24 Working Platforms / Scaffolding Where a platform/scaffold has been erected, they need to be inspected by a competent person, before first used, after any alteration, after any event likely to affect its stability (including weather) at regular intervals not exceeding 7 days. All Scaffold, Tower and Access Platforms shall be controlled by the contractor in compliance to, but not limited to, the following; The Provisions & Use of Work Equipment Regs The Construction Health & Safety and Welfare Regs.1996 BS 5973 Code of Practice NASC SG4 preventing falls in scaffolding This Procedure Scaffolding company should be full and regulated members of NASC Handover Certificate on completion of erection All scaffolders should hold a current CISRS card The responsible person should be trained to an advanced level Also on completed erection (in addition to a handover certificate) the scaffold, tower or access platform shall be tagged at all access points with a suitable scafftag system showing the following information; Location Date of Erection Requested By (Name) Checked By (Name) Signature Structure used for Inspection Record Page 8 of 25

9 Description of Scaffold / Tower / Access Platform Scaffold unfit for purpose Do Not Use section Inspection and Reports Inspections should be carried out by a competent person at regular intervals, and a written report (which is readily available for inspection on site) made after each inspection and should contain the following; Details of the person whose behalf the inspection was done Location and place of work A description of the work Date and time of inspection Details of any identified faults Details of required action Details of any additional actions required Name, position, signature of the inspector/competent person 4.25 Working on Roofs All works on roofs, whatever its nature or extent, require a safe place of work for operatives to carry out their works. Any work on a roof is a high-risk operation Initial Access To and Across Roof Access onto and across a roof shall only be permitted by the use of a system that best suits the work involved Working on Flat Roofs The management team shall confirm the appropriate method of access and working in relation to the roof finishes and the aspect of hand rails. The method and ability to walk on or work on these roofs shall be include in any specifications and / or method statements and risk assessments Working on Sloping / Slate / Tin / Tiled Roofs Except for work of a short duration (less than 1Hr) e.g. isolated slate/tile or felt repairs, a scaffold is required in the area of work to be suitable and sufficient to prevent the fall of persons or materials from the edge of the roof. The Contractor shall provide such means to arrest the fall of any person working on the roof. The most common being the use of Scaffold edge protection (other suitable means will be acceptable). 10 The design of the scaffolding should be on the basis as outlined in the Health & Safety Executive sheets 49 and 10 Short duration work is still dangerous and appropriate safety measures are required Working on Fragile Roofs All roofs of a fragile nature should be adequately and suitably identified by display of warning notice. The fragility of roof materials should be confirmed before work commences. Work of a short duration can be carried out from a suitable roof ladder for access, as long as physical access onto the roof is not required. It is important to note that the ladder is prevented from slipping by it bearing on the opposite slope of the roof. Never rely on support from the gutter or ridge tiles. It is also not recommended that permanent roof ladders of timber be used as these are subject to weather deterioration. All on the basis as outlined in Health & Safety Executive Guidance sheet 49 and 10. Short work on fragile roofs must be undertaken from a working platform rather than working from roof ladders, which are for access only. If working near a fragile roof then edge protection needs to be erected to prevent an operative from falling through these materials 4.30 Safety Systems Apart from falling from a height the other safety risk is materials falling from a height (including waste material) and striking a person below. Nothing should every be thrown from a roof or level, the waste should either be taken down to ground level then placed In a skip or the use of a rubbish chute should be incorporated into the scheme. The area underneath or adjacent to the working area can be a dangerous place and should barriered off accordingly There is a legal obligation to provide a safe place of work including safe access and egress as far as reasonably practicable. This includes appropriate steps to prevent any person falling from a height All Other Works Involving Heights This is to be achieved by the use of risk assessment to identify the risk and the best ways of reducing that risk, and be controlled by the use of a Permit to Work and Supervision. If a person is at risk of a fall which has the potential to cause injury then suitable barriers shall be erected to prevent such a fall. This could also include deep excavations where operatives could fall into excavations. Page 10 of 25

11 4.32 Steps and Ladders According to the new regulations any surface used to work off is defined as a working platform, including roofs, mobile elevating working platforms, steps, ladders, working platforms and scaffold. All steps and ladders owned by the Trust are registered and clearly identified as belonging to the Trusts Department SSSFT. Contractors are NOT permitted to use SSSFT steps and ladders as this contravenes the insurance policy. Contractors must provide and maintain their own equipment. The new Work at Height Regulations 2005 does not ban the use of ladders but do require that consideration be given to their use. Ladders should only be considered when the use of more suitable work equipment is not appropriate where ladders and steps are used it should be for light work and for short duration Steps Stepladders provide a freestanding means of access, but are very unstable and require careful use. They are not designed for any degree of side loading. The following safety precautions should be followed to minimise risk. 1. Steps should be fully extended and be at right angles to the work. 2. The top tread or the rear of the steps must not be used as a foot support. 3. Only one person to use the steps at any one time. 4. The surface on which the steps are placed should be sound and level. 5. The steps should be close enough to the work so as to prevent overreaching. 6. The steps should not be side loaded Ladders The primary use of a Ladder is to provide safe access and egress to and from a place of work. It should not be used as a working platform unless the work is of short duration and within easy reach of the ladder. The user is to maintain a three-point contact with the ladder at all times. A Ladder should not be used where it would be more practical to install a staircase or working platform There are three classes of Ladder, 1 Heavy Duty (for steel erection), 2 Medium Duty (for painting) Page 11 of 25

12 3 Light Duty (for domestic use). The trust will not accept the use of class 3 ladders on this site. Ladders should be set correctly on a firm level base at an angle of 75 (easy reference: for every 1m in height, the ladder should be 0.25m out, 4:1 ratio). Ladders must not be extended beyond their design length and must have a minimum of a 4 rung overlap. Ladders should be close enough to the work so as to prevent overreaching. Only one person to use a ladder at any one time. Use both hands to climb, if tools are required use a shoulder bag, tool belt or even a hoist line. The ladder should be faced when climbing and descending. The ladder should be secured at the top, someone should foot the ladder until it is secured. If a ladder cannot be secured then the ladder should be footed at all times. The ladder must extend past the step off point by 1100mm (5 rungs) unless handholds are provided by other means Those that use, inspect, and secure ladders should be competent to do so. Operatives are to be trained in the proper use of ladders. Do not use metal ladders near electrical sources. Timber ladders are not to be painted, they conceal possible damage (painted ladders will not be allowed on site). Do not use ladders that have been previously repaired Roof Ladders For minor repairs to a sloping roof the use of roof ladders is permitted. The ladder must be of sufficient length for the slope involved. The ridge hook must be properly positioned over the ridge so that it is supported on the opposite slope. Only one person to use the ladder at any one time. Pads of resilient material must be in place to the underside of the ladder to prevent damage to the roof and sudden movement of the ladder. The ladder should always be faced when climbing and descending. Page 12 of 25

13 4.36 General Access Scaffold According to the new regulations any surface used to work off is defined as a working platform, including roofs, mobile elevating working platforms, steps, ladders, working platforms and scaffold. This type of scaffold (also known as independent scaffold) provides a safe access to a roof and around the edge of a building and also provides the ability to store materials. All platforms require bracing, and shall be a minimum of 4 boards wide, and erected to the appropriate Code of Practice/regulations recommendations, with regards to loading etc All types of scaffolding are to be tagged 4.37 Working Platforms According to the new regulations any surface used to work off is defined as a working platform, including roofs, mobile elevating working platforms, steps, ladders, working platforms and scaffold. Working platforms that have been incorrectly erected or maintained can be liable to sudden collapse. Before using a platform check that The platform is fully boarded, and they are not damaged It is wide enough to accommodate materials & provide safe access. Guard rails should be provided to prevent falls Toe boards should be provided to prevent materials from falling. If materials are stacked higher then toe boards then brick guards shall be incorporated. All types of scaffolding are to be tagged 4.38 Tower Scaffolds According to the new regulations any surface used to work off is defined as a working platform, including roofs, mobile elevating working platforms, steps, ladders, working platforms and scaffold. This type of scaffold provides a safe access to a working area. As a rough guide the height of the tower shall not exceed three times the smallest base dimension (but should also not exceed that given by the manufacturer as a safe working tolerance). Only trained persons will erect, alter or operate any of this equipment. A trained person shall be someone who has been shown how to erect or operate the equipment and been issued with a certificate to indicate their competence. Certificates will be requested by the Supervising Officer as part of issuing of the permit. The tower should be erected and used on firm level ground Page 13 of 25

14 The wheels locked before use A ladder should be provided for access (do not climb on frame), and should be provided internally. Tower shall be vacated by users and materials before moving. If outriggers are provided these should be fully extended. A copy of the instruction manual for erection, use and dismantlement should be available for viewing All types of scaffolding are to be tagged 4.39 Mobile Access Equipment According to the new regulations any surface used to work off is defined as a working platform, including roofs, mobile elevating working platforms, steps, ladders, working platforms and scaffold. This type of access equipment should be for short duration, inspection, minor maintenance, and should only be used on appropriate ground conditions. Only trained persons will operate this type of equipment. A certificate of competence, maintenance records, and certificate of worthiness shall be required before the use of the equipment is granted. Wearing a harness attached to the platform provides additional protection against falls Safety Harness and Safety Belts If a situation arises where barriers/edge protection, working platforms etc cannot be provided or are inappropriate for the type of work being carried out, then access is to be limited to specific persons. A safe system should be adopted which may include the use of a fall arrest system, safety lines and harnesses from a secure anchorage point. If it is deemed necessary for a contractor to use this type of system, then safety advice and operation will be confirmed by a third party expert before any work is undertaken Personal Protective Equipment All roof / working at heights have been designated as Hard Hat and Hi-Vis vests Hard Hats and High Visibility vest must be worn at all times as a minimum, bump caps are not deemed to be suitable for this site. All persons working from heights shall have the appropriate PPE equipment for the work and type of location COSHH Refer to the Facilities and COSHH Policy as appropriate Page 14 of 25

15 4.43 Lone Working Refer to the Facilities and Lone working SOP as appropriate 4.44 DATIX REPORTING Any accidents or near misses should be reported through the Trusts reporting system and on the Datix system 4.45 RIDDOR Should an accident occur and this falls under the rules of RIDDOR this should be reported via the Trusts reporting system to comply with the regulations 4.46 Training Training will be required for all operatives required to work at height, and managers / supervisors and capital project managers who issue permits to work. This will enable them to be competent and confident with procedures, legislation and equipment associated with working at height. Training may also include the use of ladders, erection and use of scaffold or use of a cherry picker Training Records All training records will be maintained by the Facilities and Department The Work at Height Regulations 2005 require all persons involved in both the planning and carrying out of height work to be competent. Competency is a combination of experience, knowledge and the appropriate qualifications All operatives on site shall receive training/re-training to ensure they have the necessary skills to carry out work safely and without risk to theirs or anyone else s health. New operatives or first time operatives shall also be given appropriate induction training, to familiarise themselves with rules and procedures, specific to this Hospital Site. Each operative shall be inducted by the Site Manager or the Supervising Officer before the operative is permitted to work on the site. Induction will cover the relevant items required to comply with working at heights regulations and safe working practices. Working at Height Competency Regulation 5 of the Work at Height Regulations 2005 requires that any person undertaking work at height should be competent to do so or, if being trained supervised by a competent person. Competence is a combination of appropriate practical and theoretical knowledge, training and experience, which collectively should enable a person to Undertake safely their specified activity for their level of responsibility Understand fully any potential risks related to the work activity (tasks and equipment) in which they are engaged Detect any defects or omissions and recognise any implications for health and safety with the aim of specifying appropriate remedial actions that may be required in relation to their particular work activity Page 15 of 25

16 4.47 Staff The estates maintenance staff shall follow this Permit to work system and work along side the maintenance supervisor / works officers and managers to ensure a safe working environment is in place. If any staff are in doubt or require clarification on any aspects they should seek advise from their line supervisor and or / estates manager Capital projects The Facilities and project manager / officer shall Assess risks as part of the project and ensure the monitoring of significant work at height activity in areas where there is likely to be an impact on the delivery of clinical services (particularly so in wards and similar operational areas) Ensure control measures and internal procedures are put in place as part of the project and implemented where risks have been identified in relation to working at height To ensure that contractors are aware of and implement the requirements of the Trusts work at height procedure when working for the Trust. Follow and implement the requirements of the current CDM regulations as appropriate When capital designs are undertaken, the avoidance of working at height or minimizing the risk from working at height to carry out tasks shall be considered throughout the process and be documented Facilities Staff The facilities staff shall follow this permit to work system and work alongside their line supervisors and / or facilities manager 4.50 Weather Due consideration is to be given to the existing and foreseeable weather conditions, prior to any roof work or similar activity commencing. Roofs should be inspected before work commences to determine if the conditions on the roof are safe considering such items of Damp, Rain, Ice, Snow, Fog, Wind / gusts ect. Also in hot weather ensure operatives are protected from direct sunlight, by use of appropriate clothing. At the end of the working day roof areas should be inspected to ensure loose materials are fixed down over night to prevent damage from high winds 4.51 Falling Objects Where it is necessary to prevent injury from falling materials you must do all that is reasonably practicable to prevent anything falling from the working platform / area (toe / brick guards),or if this is not reasonable practicable you must ensure that no object is allowed to fall onto persons below use of nets or similar should be considered. 17 4.52 Danger Areas Where a work place contains an area in which, there is a risk of any person falling a distance or being struck by falling objects, which is liable to cause personnel injury the workplace is to be equipped with warnings / protection and a means of preventing unauthorized persons from entering beneath Plant and Equipment Contractors shall ensure that all plant and equipment used has been regularly maintained inspected and tested and that their staff are competent and been trained to use the items. The Trusts staff using plant and equipment must be competent and have been trained to use the items correctly Both contractors and the trusts staff shall require to have certified competence for certain use of equipment and plant Barriers Due to the nature of a Hospital Site, it will be necessary to sometimes work in, around or above areas that are still occupied by Staff, service users and Visitors. The contractor shall ensure that these areas are separated off from the public. Suitable barriers and similar or alternative ways are to be used to achieve this. This will be approved as part of the Method Statement for the works. Tape and cones are not suitable, solid barriers are to be used. In accordance with the Working at Heights Regulations 2005 Barriers should be set at a height of 950mm with the mid rail set 470mm below this Guard Rails, Toe Boards and Netting Secure guard rails, toe boards and netting should be provided wherever it is likely for persons to fall from edges and/or for objects to fall onto persons below. Also hardboard or double boarding with polythene between is to be used to prevent dust and small objects falling onto persons below. These are to be of sufficient strength to restrain the fall of a person and/or object. Where this cannot be achieved or where these measures need to be temporarily removed then other precautions will need to be taken to prevent falls or objects falling. Un-tensioned chains, ropes and other non-ridged materials shall not be permitted. When an opening is required in the guardrail, allow for a piece of rail to be removed and replaced immediately after use Construction Site - Security Control measures must be put in place to prevent unauthorised access to construction compounds, construction sites, construction vehicles, any temporary structure and any access point. 18 The minimum requirements to prevent unauthorised persons/public gaining access to equipment etc for this would be, secure fencing (herras type) around each of the before mentioned items, all fencing panels are to be bolted together. Ladders at access points are to be removed when not in use. Especial attention shall be given to ensuring no children or service users are able to climb onto any parts of the system and are not be able to tamper or self harm themselves on any plant equipment or climbing gear 4.57 Signage All access points and areas of works are to have clear signage to inform/warn the public of works overhead/adjacent and the dangers associated with unauthorised access to restricted areas Public Due to the nature of the site, service users, public and persons not associated with the works may have the ability to enter the area or be close by to the works in hand. Appropriate measures shall be taken to prevent any unauthorised access to the works area and also to limit their location in the vicinity of the works 4.59 Lifting Equipment The Lifting Operations and Lifting Equipment Regulations 1998 together with the Provision and Use of Work Equipment Regulations 1998 apply to the use of any lifting equipment including cranes, hoists and elevating work platforms etc. Before any lift a risk assessment shall be carried out to determine the load, its weight, shape, risk of falling, risk of striking a person and the risk of the lifting equipment failing, falling or toppling. The result of the risk assessment will allow suitable control measures to be introduced and correct selection of equipment.
