1 Legal Notice: This document and the intellectual property rights in it belong to Scottish Water and except where you are specifically licensed to use this document, you have no right to re-use it. Disclosure of this document under freedom of information does not change the position on (i) ownership and (ii) re-use. Purpose. To provide the guidance, knowledge and understanding of the procedures, which are required to be followed by everyone involved in work accessing the SW water network. These procedures are designed to safeguard the supply and quality of water to Scottish Water s customers and to demonstrate that Scottish Water, and their partners apply due diligence when working on the distribution network. Why? To safeguard the quality of water to customers and to demonstrate that Scottish Water, and their partners apply due diligence when working on the distribution network. Who will be affected? All Staff who work on the water network system, including partners / contractors. There is the need for all to be aware of the procedures so to ensure that they are followed at all times. DOMS Impact Assessment Form. A DOMS Impact Assessment Form (DOMS IAF) is the control method for Scottish Water to authorise access to all parties onto its water distribution network. The DOMS IAF must be completed for all planned and unplanned intervention on the water distribution network. Any DOMS IAF completed must be Risk Assessed for Maintaining Supplies to include any alternative supplies overland requirements, or tankering operations etc to mitigate any risk to both Water Quality and prevent any unnecessary Interruptions to Supply. All Authorised DOMS IAF are required to be sent to the Operation Log for Situational awareness with a Proposed or Planned start date. All valve operations on SW water distribution network MUST only be completed by staff trained in DOMS both internal and external (EU Skills registered). All Boundary Valves MUST only be operated by designated staff who are authorised by the local Water Operation Regional Manager. Boundary Valve Authorisation Levels have been increased for all Scottish Water NSO s / LFT`s and any other designated staff authorised by the local Water Operation Regional Manager Up To DMA level ONLY.. BV Authorisation and BV Operators A comprehensive list Of All Staff for both the BV Authorisation and BV Operators will be updated by the Regional Manager and retained by the BV control line for Boundary Valve Management with any Un-Authorised BV operations being reportable to the Local Water Operation Regional Manager for action. 2 Scottish Water s DOMS Impact Assessment Forms must be completed for any valve operations or hydrant operations where there is an impact or risk to the following: Water quality The continuity of supply or actual Interruption to Supply To maintain water supply hygiene All staff either internal or external who are requesting access and raising work on the water distribution network are responsible for initially enabling site and checking all the valves / hydrants on site etc are locatable and are accessible for correctly completing the DOMS Impact Assessment Form for Water Operations and/or Leakage Delivery to Authorise. If any Contractor is unable to populate the DOMS IAF or complete any enabling work on site due to lack of information they must contact the relevant business function within SW for access to a GIS drawing. A DOMS IAF must be completed for the following scenarios where there is the potential for or actually any valve operations or hydrant operations etc are required for any planned or un-planned emergency work on the water distribution network: 1. All mains repairs or any mains intervention on the water distribution network that requires or has the potential for valves to be operated. This includes any mains repaired under pressure which still need valve(s) to be operated or hydrant / scours / washouts opened to reduce the pressure to fit a repair collar. 2. Service reservoirs that require compartments or tanks to be isolated for whatever reason. 3. Isolation of treated water storage points that require compartments or tanks to be isolated for whatever reason. 4. Mains and hydrant flushing as part of a planned / regular flushing program or as an emergency flushing program as part of an incident. Note Any Mains Flushing as part of any MST does NOT require an IAF but should be updated onto the Operation Log Pre and Post Flushing. 5. De-commissioning or re commissioning of mains 6. Tankering or bowsers being filled as part of an incident. 7. Non-Intrusive Pipe sampling or New Connection above 32mm to the live distribution mains. (Under Pressure branch or Bores) 8. Under Pressure bores / connection either 25mm or 32mm will only require a DOMS IAF if the work is to be undertaken on any main deemed a Complex shut or at SW discretion. All under pressure 25mm or 32mm connection will be completed as per the generic and/or any site specific Risk assessment or method statement. 9. All trial excavations must be checked, agreed and validated in advance with Water Operations staff. Any trial excavation on or near (SW discretion) any water main deemed to be a Complex shut. Requires a valid / completed DOMS Impact Assessment Form. Page 2 of 11

3 A DOMS IAF is required to be completed for all burst repairs including any communication pipes and service repairs. The only exception is where the individual supply can be isolated without carrying out any valve / hydrant operations on the distribution network. If there is any doubt regarding the isolation of any communication pipe or the service pipe supply a DOMS IAF must be completed. The IAF must have the necessary information supplied for the isolation of the main and the valves must be cleaned and marked out. Do you want to access our? Network Control forums have been set up to review all Planned COMPLEX shuts. SW has set up Network Control forums across Scotland to help us reduce the risk to our customers of carrying out work on the water network. At these forums, teams from across Scottish Water, including CID, Customer Service, Water Operations and Leakage, come together to agree priorities and programmes of work to help us move towards a more planned environment. These forums allow more visibility of work being carried out and ensure rigorous contingency plans are in place. We also review works to allow us to learn from our successes and failures and share best practice. Through these forums we will reduce the risk to water quality and interruptions to supply and also improve customer satisfaction. Therefore, if you want to access our network then it is vital that you get involved in your area s forum. 4 The Water Operational and/or the Leakage Delivery staff authorising and completing the DOMS IAF for a Complex shut are responsible for the communication and liaison between all the relevant business functions above for their information and any input required or authorisation. The final decision for authorising and allowing any Complex shuts to proceed lies with the Water Operational and/or the Leakage Delivery or Standby Staff. After authorisation and sign off by Water Operations and/or Leakage Delivery the DOMS IAF is valid for 7 days. The work on the water network must occur within 7 days of authorisation. Any network access after the agreed start / completion date will require a new IAF to be produced or the original IAF to be checked, amended and counter signed by the appropriate regional Water Operations contact to authorise the access extension. The person authorised for operating the valves is responsible for updating the Operations log from site on the day of the actual work being under taken on the water network Pre and Post valve operations with the relevant Interruption To Supply details (Water On / Off Times and Property numbers) and for updating and the completion of the DOMS IAF when the work has been completed. Complex Shut Criteria is any shut off required which meets any of the following criteria 1. Any Main 200mm or above in size? 2. Is a Strategic or Critical Main going to be affected? 3. Is it a multiple valve change job which requires NSO support (generally 5 Valves or more)? 4. Is Back-feeding required? 5. Is a District Metering Area (DMA) or Boundary Valve going to be operated / changed? 6. Are any Key Customers, Hospitals or Prisons going to be affected? Trunk Main Any main greater than or equal to 200mm diameter. Strategic Main Any main of importance to delivery of the water supply service into an area. This will include those previously referred to as trunk mains but are less than 300mm Diameter Critical Main Any main within the proximity of a geographic feature (e.g. Railway, River, Bridge, Buildings, etc) Which might cause difficulty of access to effect a speedy repair and the consequence of failure is therefore severe. Note Raw Water Mains are defined by their function, irrespective of diameter, as being any main carrying untreated water from a source to a treatment works or other delivery point. (These may also be termed Critical ) If the answer to any of the Questions above are yes The network intervention / shut off is deemed to be a Complex shut. Page 4 of 11

5 Complex shuts Complex shuts should be undertaken by Scottish Water NSO`s / NSI`s and/or Leakage Delivery LFTs or Stand By NSO. For all internal and external contractors requiring access to the SW water network which is deemed a Complex shut the Contractors staff will act under the direct instruction and/or supervision of the Water Operations NSO`s and/or Leakage Delivery LFTs or Stand By NSO to provide support and assistance where required for any Warning Bills / Valve & Hydrant Operations / Pressure Tests / Flushing / Water Sampling when and if required. Back feeding operation which are required for Complex shuts should be installed within a minimum of 48hs ahead of any planned shut down to make sure the back feed is feasible and suitable to mitigate any risk to both Water Quality and prevent any unnecessary Interruptions To Supply Downgraded Complex shuts Water Operation and/or Leakage Delivery may deem some Complex shuts to be LOW RISK and allow either Internal or External staff to undertake these types of Complex shuts. The Water Operation and/or Leakage Delivery staff member who authorise and deem the shut to be low risk will be responsible for ensuring that all Risk Assessments, Method Statements and Contingency Plans where applicable are checked too their satisfaction and in place ahead of the Complex shut being undertaken. Complex shuts update Burst Mains Repairs (2015) NSO Complex DOMS Handover Process for burst 250mm / 9" or larger Field Response NSO Investigate & locate burst. Raise request for Repair Can repair be planned for more than 48 Hours? Yes Field Response NSO Contact ICC OSRA, is there any network Impact Field Response NSO Take photo evidence and send to Water Ops North & East / West Ops south and West mailbox along with SR No. Follow up with call to Risk Technician requesting the Networks NSO to carry out DOMS IAF Risk Technician Allocate DOMS task with supporting documents immediately to Network NSO and phone call to Network Team Leader. No Field Response NSO Complete DOMS IAF ensuring correct level of Authorisation and plan for mains shut. Yes The above process details the hand over between Field Response NSO & Water Operations NSO for main repairs that are a diameter of 250mm / 9 or greater which can be planned over 48 hours. Any mains repairs that are a diameter of 250mm / 9 and are required to be repaired within 48 hours and all other Complex shuts which are less than 250mm / 9 in diameter must have a DOMS IAF completed by the Field Response NSO raising the work request. The Field Response NSO raising a planned work request contacts the (OMC) Risk Technician to schedule for a Water Operations NSO and for a Water Response repair squad after they have checked that the valves required to carry out the work are locatable and have been cleaned out and complete the DOMS IAF to the best of their Knowledge for issue to Water Operation staff for further ownership and authorisation. 6 The (OMC) Risk Technician should then assign a task to a Water Operations NSO ASAP / minimum 48hrs prior to any planned complex shut to complete any onsite valve checks and back feeds required as part of a complex shut. All Internal and external contractors who require a SW NSO resource to attend any site meeting for a DOMS IAF to be checked, authorised and signed off etc. MUST be done using Customer Services Delivery scheduling resource proforma for scheduling a NSO resource through Click. All requests must have a minimum 3 working days notice, preferably 5 days prior to any initially Planned site meeting and another 3 working days notice, preferably 5 days for any complex shut off with an Auto code / work order number provided for NSO cost and time capture. The same work order will be used in the event a Complex shut has been confirmed and requires either a Water Operations NSO or LFT to complete the complex shut on any planned and agreed date. All contractors should only contact the CSD Scheduling on to arrange for a NSO resource in EMERGENCY SITUATIONS ONLY. NSO resource Proforma s are to be e mailed to the relevant addresses as follows 'Ayr Work Requests@scottishwater.co.uk' 'Clyde Work Requests@scottishwater.co.uk' 'Don Work Requests@scottishwater.co.uk' 'Forth Work Requests@scottishwater.co.uk' 'Nith Work Requests@scottishwater.co.uk' 'Ness Work Requests@scottishwater.co.uk' 'Tay Work Requests@scottishwater.co.uk' 'Tweed Work Requests@scottishwater.co.uk' Non Complex Shuts If the answer to all 6 Complex shut criteria Questions is No the network intervention / shut off is deemed to be a Non-Complex shut. For Water Response Repair team s workloads the DOMS IAF will be completed and authorised by the Field Customer Service NSO raising the initial work request after they have checked that the valves required to carry out the work are locatable and have been cleaned out, are accessible, operable and are marked up for Water Response Repair team s. If 4 valves are required to be operated then all 4 should be cleaned out, checked and accessible. If there are reasons why any of the above criteria is not met this MUST be recorded in the comments box detailing why and what is required for the work to go ahead. The (OMC) Risk Technician does not require sending any NSO, as the Water Response repair Squad will complete the Valve Operations as part of a Non Complex shut. The Water Response Team Leader is responsible for communication / liaison with Water Operation and/or Leakage Delivery for any Complex / Non Complex shut off required on the water network with the planned start dates and times of shut offs to the network required. 7 Non Complex Authorisation update Metering Services and Customer Connection staff are allowed to complete and Authorise Non Complex DOMS IAF s for their work, with Water Operation and/or Leakage Delivery informed and the Operation Log update accordingly. Any planned Complex shuts required by Metering Services or Customer Connection must be authorised at the relevant Network Control forum by Water Operation and/or Leakage Delivery. For any internal or external contractors who require to access or work onto the Water distribution Network which is deemed Non Complex, the DOMS IAF will be checked and authorised by Water Operation and/or Leakage Delivery staff. The EUSR Skills number must be confirmed as valid by Water Operation and/or Leakage delivery staff for any external staff or contractors before being allowed to operate any valves on the water distribution network as part of any Non Complex shut or any Low Risk Complex shuts as deemed by any Water Operation and/or Leakage Delivery. The contractor s is responsible for any valve operations as part of a Non Complex or Low risk Complex shuts as per the instruction and detail on the agreed authorised and signed off DOMS IAF. The Water Operation and/or Leakage Delivery staff authorising the DOMS IAF for any internal or external contractor are responsible for the communication and liaison between all the relevant business functions and for escalating any Complex shut for the correct level of authorisation as per the type and category of Boundary valve and/or by the number of properties affected as defined in the table above. The Internal / External contractor is responsible for updating the Operation Log with the agreed / planned start dates and times and MUST also update the Ops Log with the actual Pre and Post valve operation from site with the correct ITS details Off / On times including any property numbers affected. The Contractor is responsible for updating the Ops Log with a copy of the Valid DOMS IAF for their Work Activity. The Water Operational and/or the Leakage Delivery staff authorising and completing the DOMS IAF for a Complex shut are responsible for the communication and liaison between all the relevant business functions above for their information and any input required. The final decision for authorising and allowing any Complex shuts to proceed lies with the Water Operational and/or the Leakage Delivery or Standby Staff. The Internal / External contractor is responsible for updating the Operation Log with the agreed / planned start dates and times and MUST also update the Ops Log with the actual Pre and Post valve operation from site with the correct ITS details Off / On times including any property numbers affected. The Contractor is responsible for updating the Ops Log with their planned where abouts and a copy of the Valid DOMS IAF for their Work Activity For all Non Complex shuts undertaken by the Internal / External contractor they are responsible for completing a manual Interruption to Supply ITS profoma and for updating into Scottish Waters CDR Corporate Data Repository. 8 After the work has been completed all completed DOMS Impact Assessment Forms are to be e mailed into. doms@scottishwater.co.uk All completed DOMS Impact Assessment Forms; E mailed into the above address must have either a Work Order number or Service Request or Project ID with the Location of the site and the date the work was undertaken on the water network and work type. Examples 27 Argyle St Glasgow 24 th May 2011 Burst Mains Repair Work Order Main Street Dundee 20 th May 2011 Mains Re-hab (Morrison s) Auto Code DOMS IAF`s especially any Complex shuts are to be reviewed by the Water Operations and/or Leakage Delivery person who authorised the network intervention to confirm the water network and all subsequent valves have been returned to the correct status with any issue from site are highlighted and captured for further investigation and resolution. I.e. Poor pressure, Valve missing, any updates to GIS, No water complaints as a result of the work being carried out etc. All staff both internal and external should update the Red section of the DOMS IAF and notify SW of any issues incurred during their work activity for SW to action and update it s corporate GIS. Arrange a SW Resource and site Visit All Internal and external contractors who require a SW NSO resource to attend any site meeting for a DOMS IAF to be checked, authorised and sign off etc MUST be done using Customer Services Delivery scheduling resource proforma for scheduling a NSO resource through Click. All requests must have a minimum 3 working days notice, preferably 5 days prior to any initially Planned site meeting and another 3 working days notice, preferably 5 days for any complex shut off with an Auto code / work order number provided for NSO cost and time capture. The same work order will be used in the event a Complex shut has been confirmed and requires either a Water Operations NSO or LFT to complete the complex shut on any planned and agreed date.
