1 TENGIZCHEVROIL TCO CHESM QUALIFICATION QUESTIONNAIRE GUIDANCE 1. Introduction The purpose of this guidance is to explain the main rules for completing TCO s CHESM Qualification Questionnaire. It is important that you strictly follow these rules, as an incorrectly filled questionnaire impairs our ability to complete CHESM evaluation correctly. 2. General rules - You are not allowed to modify the questionnaire in any way other than completing requested fields. Any manipulations with protected cells and/or formulas are not acceptable and will result in the denial of your questionnaire. - All fields containing explanations as shown below are mandatory and must be completed. Fields designed for evaluator comments and grading (Yes, NO, and N/A columns) must be left blank. 3. Recommendations - The information provided in your reply must be true and correct. - Your reply must be comprehensive, but concise and explicit at the same time. - Generally, a good answer refers to a document and/or provides examples. Answers like Yes are not considered comprehensive and will not be given credit. - All documents referenced in your reply as well as other types of supporting evidence must be provided together with the questionnaire. 4. Describe official control procedure of critical documents related to investigation results, regulatory and legal compliance checks etc. to secure such documents. Is there a written contractor safety management process? Do you evaluate the ability of subcontractors to comply with applicable HES requirements as part of your selection process? Provide example or criteria. If yes, provide documentation to demostrate. Describe your process of subcontractors selection in regards of potential subcontractor ability to meet HES requirements including its employees proficiency, used materials and resources. Provide HES requirements compliance appraisal of your company current subcontractors Hazard Identification, Communication and Control: Do you have a documented process to identify work-related hazards including task, work location, natural conditions and materials? Provide environmental-related process developed to identify such hazards as rotating equipment, hoisting machines, chemical corrosion environment, biological hazards, electricity, hot surfaces etc. which can impact workers during operation. Submit Job Risk Evaluation Procedure (JHA)? Are safe work practices and procedures developed based on the hazards identified to mitigate the risk to employees? Does your company perform Job Hazard Analysis (JHA) on its work operations? Provide examples. Provide safe work practices for all trades of your company, HES guidances. We are requesting Safety Instructions from them in the item 3.4 JHA is to be performed for a specific activity; it shows work stages, work hazards and hazards control actions. If Yes, provide example. Do you maintain updated and accessible MSDS for paints and chemicals? If company use chemicals and paint materials during operation it must have Safety data sheets base for these materials to be used by workers. Please confirm Incident Reporting, Investigation and Statistical Data: Are those incidents that qualify as recordables according to ROK Labour Code Section 5, Chapter 37 and TCO requirements investigated? Does your company have an approved written process to report and investigate incidents? Provide documentation to confirm. 4 Provide documentation to demostrate that all accidents are reported to management. If your company has approved procedure, provide documentation to confirm. Does your process provide a technique for root cause analysis? Do you have personnel trained and qualified to coordinate a formal investigation with ROK and TCO personnel? Provide documentation to demonstrate. Do recommendations directly address the root causes and are there clear action items that are completed by defined dates? Provide documentation to demonstrate. Describe process of root cause analysis of accidents related to management system deficiency, and process of clear action to prevent such accidents in the future. If Yes, provide certificates of personnel traning. Provide documentation to demonstrate current cause corrective actions. Page 3

Does your company have a process to share lessons learned on incidents and near misses? If Yes, provide documentation to demonstrate that intended actions are completed by appointees within the assigned time. How the assigned employees are supported to complete the actions? Describe your process of lessons learned sharing. It can be group safety meetings, prikazes, extra orientations, staff refreshing training etc. g) Does your company have a process to track man-hours and incidents to establish DAFW, total recordable and motor vehicle incident rates? Provide documentation to demonstrate. Describe process of information collection and analysis to improve labor conditions. It can be inspections which are registered in inspection results registry book. If yes, provide documentation to demonstrate Behavior - Based Safety: Do you have a behavior - based safety (BBS) process in place and is the program communicated to all employees? BBS objective is to observe workers behavior and to give comments which must be registered Specify if your BBS has been purchased from known international company or developped by your company specialists. If yes,provide documentaition Personal Protective Equipment (PPE): Do you have a written PPE program that includes: PPE selection 1 Submit official program that includes below items. Contractor must carefully learn conditions of all work areas and provide personnel with protective equipment accordingly. f) PPE use PPE care and maintenance Is the program communicated to all employees? Do you provide the required PPE for the jobs that you perform? Does your company have a process to identify PPE needs specific to a task or position? All PPE must be used during whole work shift in accordance with facilities requirements. All PPE and protective clothing must be maintained in accordance with current regulatory acts and manufacturer manuals. All above-mentioned items must be described in details in PPE program and must be communicated to all contractor personnel. List of awareness must be attached to PPE program. Contractor shall provide all empoyees and work group staff with PPE at company expenses. Provide documentation to demostrate. Specify allowance of protective clothing for specific works Transportation and Vehicle Safety Describe your company motor vehicle fleet (quantity and typ 6 Describe company's motor vehicle fleet. Describe the transportation services your company provides (e.g. transportation of cargo, people, etc.). Describe the transportation services. Does your company have a motor vehicle safety policy and process? Contractor shall have in place a policy to ensure motor vehicle safety. The policy includes: drivers training, journey planning, vehicle maintenance, pre-shift vehicle inspection, use of safety belts, ban on cell phone use while driving, medical examination etc. Provide documantation. Do you comply with all applicable traffic and vehicle safety regulations? Please describe. Do you have assigned personnel responsible for transportation safety in your company. Please describe. Describe the employment procedure for drivers and applicable requirements Contractor shall assign a lead mechanical engineer to be responsible for motor vehicle safety. The position to be approved by top manager. Do you have the practical driver training ground, so that drivers could demonstrate their driving skills to be tested by a committee? Provide documentation. Do you conduct a practical training and drivers evaluation to consolidate theoretical knowledge obtained? If yes, provide corresponding documents. Do you keep records of MVs mileage (dates, speedometer readings), MVCs involving your company vehicles etc? Provide documentation. j) Do you make any MV incident statistics analysis? And what type? Do you conduct any analysis (as per above-mentione to identify failures caused by insufficient maintenance, unskilled personnel, poor financing etc. Provide documentation. k) l) m) Have you implemented any MVS improvement recommendations (if any) for the last 12 months? Provide examples. Are there any plans forward to improve / sustain motor vehicles safety performance? Do you conduct technical inspection for your motor vehicle fleet. The inspection may be conducted in several stages: management, middle management and HSE engineers. Do you conduct self-inspections and audits and document audit findings and recommendations? Do you track corrective actions to verify completion within assigned time? Describe process. Are recommendations assigned to a position with the knowledge and authority to implement corrective actions? Provide schedule of site inspection works aimed at identification and insitu elimination of failures. Provide records of inspection results to be used for development of working conditions improvement activities. In respect to any deviations from HES requirements the remedial actions shall be developed, documented and approved by top manager. Recommendations shall specify the persons in charge and due date. The persons in charge are to be notified and get familiarized (supposedly, through the prikaz of top manager) Regulatory Compliance and Licenses: Do you know the HES - related government and local regulations pertaining to your work? If you do, Provide details on the various licenses your company currently holds for performing work in th ROK. Attach copies of your licenses for the specific type of work that you would like to perform in Tengiz. 4 Provide copies of general operating licenses and copies of licenses for the specific type of work to be performed. Describe the procedure for obtaining a state license Pollution Prevention: Does your company have a written procedure to identify hazardous and non - hazardous waste generated at your facility? 7 Provide the approved procedure which specifies types of waste your company generates in the course of work performance (luminescent lamps, tires, accumulator batteries, lubricants wastes, paints, medical wastes etc.) and methods of its disposal. Do you have any third-party contract on wastes recycling and disposal? (Provide Contracts) Do you have document the quantity and disposition of all hazardous and non hazardous waste you generate (waste management plan)? Provide Waste registration book. Do you have a site - specific spill prevention program? Provide spill prevention program with spill containment and prevention measures (lubricants, chemicals, paints) Provide spill response plan which specifies personnel actions in case of contaminants spill Emergency Preparedness and Response: Do you have a written emergency response plan? Note: Small companies may not have their own plan but still need to have site - specific emergency response plans. 3 Provide emergency response plan specific to the offices and other buildings of your company, which specifies master wardens, master points, emergency operators phone numbers, buildings evacuation plans and emergency exits (agreed with regional Emergency Department), etc. Do you conduct and document emergency response training and drills? Note: Small companies still need to document participation in site emergency response drills at owner's facility. Short Service Employee (SSE): employees new to your company or new in their work assignment (less than 6 months). Explain how your company manages SSEs Provide Short Service Employee progam, which shows the following: visible markings of new employees, maximum permissible period of stay in hazardous areas, mentoring Craft Training Does your formal training program include job/task specific training documents and competency evaluations? Provide documentation to demonstrate. In case you provide craft training and competency evaluation, submit relevent permitting documents (licenses). Provide state-approved training program. Have employees been trained in appropriate job skills? Provide certificates of trained personnel. Page 6

8 Is there an ongoing program for all employees to insure job knowledge and responsibilities are understood? Define program and frequency of training by position. Does your company have persons specifically assigned to provide training to your employees? If yes, are they certified? Is there a program for training Kazakhstan citizens? List the number of Kazakhstan citizens trained for each trade (welders, fitters, etc.). In case the training program, specified in item is available, provide craft training schedule. 10 4.5. Training Facilities and Records Does your company have facilities dedicated to classroom and field training? If yes, describe your facility and the equipment available for training (e.g. computer, audio, video, props, etc.). Provide pictures of premises, list of equipment, visual aids, audio and video materials. Are all employees formal educational and governmental certifications required by RoK regulations and industry standards on file? Provide documentation to demonstrate. Do you have HES and crafts training records for each individual employee that include employee identification, date of training and name of trainer? Are training records reviewed to insure all employees remain current on training and qualifications? Provide information of workers - personal files with diplomas and HES training certificates. Provide matrix tracking HES training of all staff. Who tracks training actuality in your Company? It can be Training Coordinator. Do you provide insurance to the employees for medical care and treatment? Equipment Does your company have written procedures for the operation, inspection and maintenance of machinery and equipment (e.g., cranes, forklifts) in your inventory? Be prepared to show your records. Provide manufacturers safety manuals for operation and maintenance of machinery and equipment along with awareness list singed by responsibles for operation and working conditions of machinery and equipment. Do you have a Safety Inspection Checklist for equipment used on business? Provide checklist of daily inspections of all used equipment (hoisting machines, vehicles). Checklist includes inspection of break system, marker lights, windshield and matching mirrors, tire pressure, first-aid kit, wipers, heating etc. Do you maintain operating equipment in compliance with regulatory requirements including certifications, calibration, maintenance system, etc.? Provide proof of conducting technical inspection for your equipment. Provide partial and complete hoisting machines partial and complete Technical Inspection.Provide certificates for hoisting machines showing good condition and Emergency department technical certification, calibration certificates of metrology instrumentation and SI system certification. 6.
