Has a Risk Assessment been completed for this project in the past? Has the community partner been contacted so a risk assessment of the planned project can be completed at the project site? Yes (Y) or Actions No (N) The Site Safety Leader/Officer will train the team of CIT members and employee volunteers using training formats used for other types of risk assessment trainings. The team will conduct the risk assessments. For smaller sites without a site Safety Leader, the business unit Safety Functional Excellence Leader will help initiate team, train or organize training, and help to identify roles and responsibilities of team. (*Note - a site HSE leader from another site, designated by the SFEL, could serve as a mentor in the process). If yes, document their name and information. If no, provide them with adequate training. A previously completed risk assessments is acceptable, provided it is up to date and includes any additional steps needed for the project, change in location, new tasks or different location The risk assessment should be conducted at the site of the planned project. The Community Partner shall be involved or consulted to make sure that all activities and steps, tools and equipment (including PPE) to be used, and types of people participating are considered in the risk assessment. A risk assessment provided by the Community Partner is acceptable, provided it has been reviewed by the Risk Assessment team. The team will review the risk assessment for accuracy (specific activity details), the location, equipment, people and potential hazards. Will the activity involve using tools, equipment, substances? Tools and equipment should not be provided (from home) by the participants. Either use company owned equipment (including PPE), because this will be in a planned preventive maintenance program; or the CIT Partner should provide it. If the latter, include visual checking in the work instruction or checklist. Do not use tools and equipment that appear to be damaged or worn. If chemicals such as cleaning fluids or paints are to be used ensure that the MSDS (Material Safety Data Sheet) is referenced for prohibited substances. 11 The MSDS shall be reviewed for use, handling, spills, and disposal of the chemicals. These shall be included in the risk assessment.
