The sealing operation will take approximately 3 shifts to complete inclusive of sealing, trafficking, sweeping and line marking. On the first night shift, the site will be sealed under left/right lane closures. Once sealed, the site will be set for trafficking. The following day shift will continue trafficking and monitoring. If the sites livens up during the day, or looks like it may be stripping, then trafficking intervals will be increased or a water cart may be utilised to mitigate damage. The following shifts will traffick until the sweeper arrives. The sweeping will generally be undertaken in the evening of the second night when traffic volumes are decreased. After the site is swept, road marking till follow before the site is packed up. If road marking is delayed due to the weather, the site will be monitored and centre and shoulder cones set as per attached TMD. Site access during sealing will be installed to suit activity. Generic TMDs to be used with the overview plan for relevant works. Day works are planned for the hours between and night works during A VMS sign will be added post seal as a trial basis with approved as per attached Plan F. Planned work programme Start date 12 th February 2017 Time 0000 End date 31st March 2017 Time 2359 Consider significant stages. No work will be carried out -during long holiday weekend or moratorium periods unless approved by the TMC/RCA, or -if weather conditions restrict site visibility distances to less than 200m Alternative dates if activity delayed Road aspects affected Pedestrians affected? No Proposed traffic management methods Installation (includes parking of plant and materials storage) On single direction carriageways, signs must be deployed on the left hand side of the road before the right hand side is deployed. On bi-directional carriageways, signs should be erected by travelling around the road network in a clockwise direction, taking in each side road as they are passed. By doing so, all turns in and out will be to the left, which is easier and safer. The first sign erected must be the advanced warning sign. Tapers and delineation devices must only be placed once all signs have been installed. Before any equipment or materials are brought into the work site, a drive through check must be made in all directions including all side roads. This check must confirm that the worksite is
2 TMP or generic plan reference Attended (day/night) Unattended (day/night) Detour route Removal -safe -to the minimum standard shown in the TMP -restriction to traffic flow is minimal -signs and delineation devices provide clear messages to road users, and -signs and delineation devices are securely erected and will remain in their correct position under expected traffic volumes and weather conditions. All plant vehicles are to be parked with the flow of traffic where possible, and clear from the carriageway. STMS responsible is to check that the TMP is appropriate to the worksite. Ensure traffic is monitored for queuing and delays, site checks are completed at least two-hourly, unless requested otherwise. STMS to also ensure that travelled distance from TSL to TSL is appropriate to work site. Sites may be unmanned after working hours, provided the site is left in a safe and tidy manner with appropriated signage installed as per attached diagram/s. Applied to RCA when required, or submitted retrospectively in the event of an emergency at TMC request. TTM removal to occur in the reverse of installation, starting with cones and then the signs. Remove any options which do not apply to your job Major Incident A major incident is described as: Fatality or notifiable injury - real or potential Significant property damage, or Emergency services (police, fire, etc) require access or control of the site. Actions The STMS must immediately conduct the following: stop all activity and traffic movement secure the site to prevent (further) injury or damage contact the appropriate emergency authorities render first aid if competent and able to do so notify the RCA representative and / or the engineer under the guidance of the officer in charge of the site, reduce effects of TTM on the road or remove the activity if safe to do so

Incident An incident is described as: excessive delays - real or potential minor or non-inquiry accident that has the potential to affect traffic flow structural failure of the road. re-establish TTM and traffic movements when advised by emergency authorities that it is safe to do so Comply with any obligation to notify WorkSafe. Actions The STMS must immediately conduct the following: stop all activity and traffic movement if required secure the site to prevent the prospect of injury or further damage notify the RCA representative and / or the engineer STMS to implement a plan to safely remove TTM and to establish normal traffic flow if safe to do so re-establish TTM and traffic movements when it is safe to do so and when traffic volumes have reduced. Note also the requirements for no interference at an accident scene: In the event of an accident involving serious harm the STMS must ensure that nothing, including TTM equipment, is removed or disturbed and any wreckage article or thing must not be disturbed or interfered with, except to: save a life of, prevent harm to or relieve the suffering of any person, or make the site safe or to minimise the risk of a further accident; or maintain the access of the general public to an essential service or utility, or prevent serious damage to or serious loss of property, or follow the direction of a constable acting in his or her duties or act with the permission of an inspector. Site induction briefing to occur at least daily, or when TTM conditions change, and acknowledgement of briefed hazards signed. All personnel to wear hi vis garments and any other applicable PPE as per site activity/tmp conditions. STMS to record and notify the RCA or engineer within 24 hours of all crashes at the worksite, or complaints of the TTM. Record and inform the RCA immediately of any significant modifications to TTM measures not included in the approved TMP. Where the site cannot be removed at the end of the shift, site checks will occur at least once in a 24hr period. Method for recording daily site TTM activity (eg CoPTTM on-site record) On site record, Generic TMP check list, STMS Check Sheet. Site safety measures Site hazard ID: The Fulton Hogan hazard ID procedure shall be used to identify site traffic and safety hazards, thus reducing any issues prior to work, and forming a basis for site induction/tailgate for site personnel. All site staff and visitors will be briefed as to safety hazards and traffic control arrangements, inclusive of entry/exit points. The site foreman is responsible for ensuring that adequate measures are in place. All signs/cones shall comply with the requirements of CoPPTM and will be set out as per diagrams in this submission. Approved Health and Safety, environmental quality plans and the temporary traffic management plan available on site for all contract staff at all times. The minimum safety equipment to be worn at all times while onsite is: Safety footwear (steel toe capped). Long sleeve and trousers worn High visibility vest. Safety glasses. Hard hat. Gloves available for manual handling. Hearing protection (as required) First aid kit. Fire Extinguisher. Any additional PPE applicable to job/company policies. Plant and Equipment All vehicles will have a roof-mounted beacon. Flashing beacons consist of a light encapsulated in an amber casing that may either flash (strobe) or appear to flash when circled by a rotating reflector. Hazard lights should be used where appropriate, but are not considered beacons.
