Ironworker Safety Director Training Course Steve Rank Executive Director of Safety and Health

Providing Opportunities for Ironworkers and Company Safety Personnel to Learn New Skills-Sets and Manage Comprehensive Safety and Health Programs for our Contractors

2017 Zero Incident Campaign New Programs and Initiatives Targeting the Deadly Dozen Hazards

The Deadly Dozen Hazards 1) Falls through unprotected or inadequate floor opening covers 2) Collapse of unsecured open web steel joists 3) Lack of fall protection and inadequate use of equipment 4) Falls during installation of floor and roof decking 5) Material handling during steel erection & reinforcing steel 6) Column collapse due to anchor bolt failure

The Deadly Dozen Hazards 7) Structural collapse of reinforcing columns, walls, and decks 8) Struck-by injuries from falling objects, tools, and materials 9) Caught between injuries during hoisting and rigging 10) Impalement from reinforcing dowels or other projections 11) Electrical hazards from high-voltage power lines 12) Heat illness and toxic exposure to chemicals

The question is. What are we doing about it?

Ironworker Safety Director Training Course Fall Protection Systems (Active and Passive) and Basic of Lifeline Systems Calculations of Regulatory Compliance, Project Contracts and Safety Program Development Safety Training and Determining Competent and Qualified Persons Evaluating Workplace Health Issues and Exposures Steel Erection Hazards, Regulatory Requirements and Safe Practices

Ironworker Safety Director Training Course Development of Job Hazard Analysis, Site Safety Plans, Workplace Inspections Managing Recordable/Reportable Incidents, Incident Management and Investigations Crane Assembly and Disassembly Director Responsibilities Reinforcing Steel and Post-Tensioning Operations Using Construction Safety Apps to Monitor Workplace Safety

Review Contract Safety Requirements Project Verification of Federal OSHA, State OSHA, MSHA, Corps of Engineers Jurisdiction Learning to Implement Steel Erection Standards Utilizing Key Controlling Contractor Requirements Knowing Steel Fabrication Requirements for Columns and Beams Understanding and Referencing OSHA Compliance Directives and Clarification Letters Recognizing Steel Erection Standards Requiring Special Employee Training

Examples of Owner or General Contractor Safety Requirements Designation of Company Competent and Qualified Persons on Project Site specific Safety Plan and Job Hazard Analysis Fall Protection Height, Steel Toe Boots, Eye Protection, Work Gloves, High-Visibility Vests Documentation of Employee Training Schedule for Workplace Inspections Documentation of Representative Air-Sampling Tests for Welding Operations Note: Beware of Safety Clauses in Contracts that Allows Owners and Controlling Contractors the Final Word. Corps of Engineers Requirement - EM385-1 Department of Army, U.S. Army Corps of Engineers, 15 September 2008, Safety and Health Requirements. In case of conflicts between ACGIH and other standards or regulations referenced in this manual, the more stringent shall prevail.

Examples of Common Pit-Falls Ironworkers get Blind-Sided at Jobsite Lack of communication Between Employers to Field Ironworkers Lack of Proper Equipment No Designation of Competent or Qualified Persons on Project No Documentation of Employee Training No Schedule for Workplace Inspections No Documentation of Representative Air-Sampling Tests for Welding Operations

Project Verification of Jurisdiction Federal OSHA, State OSHA, MSHA, Army Corps of Engineers Federal OSHA Jurisdiction - Military Bases, Indian Reservations, Federally Funded Projects State Approved OSHA Plans - 27 States (Must Meet or Exceed Federal OSHA Standards) Mine Safety and Health Administration MSHA has Stand Alone Safety Standards Army Corps of Engineers Corps has Stand Alone Safety Standards Note: Some Projects Such as Bridges of Water Often Have Joint Jurisdiction Between Federal OSHA and State Approved OSHA Plans

Why is This Important and What is So Special About the OSHA Steel Erection Standards? Protect Your Company Safety Program by Referencing Specific OSHA Standards Negotiated By Our Industry for the Protection of Our Members and Contrators Always Include Certain Provisions in your Safety Project and Contract Submissions Make the Responsible Party Owner or Controlling Contractor Comply with Standards Lets Take a Look at a few..

Sample Letter Dear Mr. Jones: Please forward a copy of the concrete strength tests that confirm the compressive strength has obtain the minimum 75%. We will proceed column erection immediately upon receipt. Thank you, Archie Bradshaw CC: Ron Bruce

Dear Mr. Jones: Sample Letter Prior to the commencement of steel erection, please advise if any column anchor bolts have been altered or modified. We will proceed column erection immediately upon receipt. Thank you, Sir Bert Royer CC: Darrell LaBoucan

Sample Letter Dear Mr. Jones: Please provide adequate site conditions for mobilizing our cranes, equipment, and storage of materials. We will proceed with delivery of cranes and aerial lift equipment immediately after the site and conditions have been provided. Thank you, Pat McDermott CC: John Jay Hurley

Sample Letter Dear Mr. Jones: We have completed the erection on floors 7-8 and it is necessary for you to inspect, accept, and retain custody of the perimeter and interior safety cables. We will leave this fall protection equipment in place for other trades to use pending your acceptance. Thank you, Mark Thomas CC: Darlaine Taylor

(b)(3) Maintaining a planked and decked floor A fully planked or decked floor or nets shall be maintained within two stories or 30 feet, whichever is less, directly under any erection work being performed.

OSHA Compliance Directive Question 23: Section (b)(3) requires a "fully planked or decked floor or nets" within two stories or 30 feet, whichever is less. Can an employer's requirement that workers be protected by fall arrest equipment at all times above 15 feet (or less) take the place of nets and temporary floors? Answer: Yes. Where an employer establishes, communicates and enforces a requirement to be protected by fall arrest equipment at all times above 15 feet (or less), the failure to comply with (b)(3) is considered a de minimis violation and will not be cited.

Problems with OSHA Compliance Directive

IMPALEMENT ISSUE: Rebar Fabricated with 90 Degree Bends and Caned Ends Substituted for Approved Impalement Covers

Test-Drop Confirms Results

Impalement Clarification Letter Cal-OSHA does not accept the bending or hooking of rebar as an acceptable method of impalement protection. An employer that bends or hooks rebar for employee impalement protection instead of providing approved covers or troughs is in violation of Title 8, section Such an employer would be placing their employees at risk of serious injury.
