4 Sign In Sign-in to the with the Username and Password you created when you set up your account. You will be navigated to the Workplace Health Achievement Index introduction tab upon successful sign in. Password Reset Process If you do not know your Password you may reset your Password via the account Username. STEPS SCREEN SHOTS 1. Click Forgot your password? Sign In Screen 2. 8 Workplace Health Achievement Index Introduction INTRODUCTION SUMMARY The Workplace Health Achievement Index ( Index ) is designed to help your organization assess the Quality, Comprehensiveness, and Effectiveness of your workplace health program. The Index consists of three sections: Section 1: Organization and Demographic Information collects company-level information that allows benchmarking reports comparing your results with the average results of your peer organizations based on company size and industry. Section 2: Structure and Process Measures use a 55-item questionnaire organized into seven sections that assesses how your organization s structure and processes support a workplace culture of health. This section will determine the effectiveness of your overall workplace or worksite health promotion and ability to promote a culture of health. Section 3: Performance Measures gathers employee level data based on AHA s Life Simple 7 ( LS7 ), a scientifically validated framework that measures ideal cardiovascular health. LS7 metrics include seven health behaviors and health factors critical to achieving cardiovascular health: not smoking, eating healthier, getting physically active, achieving and maintaining a healthy weight, managing blood pressure, controlling cholesterol, and reducing blood sugar. Data is submitted, stored and processed in a secure, HIPAA-compliant third-party database. If requirements are met for the employee health data submitted, then the organization will receive a heart health score. The heart health score is tracked year-to-year allows an organization to view progress on improving the health of its workforce. Your Total Index Score is comprised of points awarded in Section 2 and Section 3. Section 1 is not scored. For new companies taking the assessment for the first time, the maximum possible score is 217 points. For companies that take the assessment more than once, the maximum possible score is 250 points. To receive points for Section 3, companies must submit data on a minimum of 25% of employees. See the User Guide for more details. : 1/24/17 7

9 INTRODUCTION SUMMARY () This proprietary Index was created by the AHA and reviewed by the AHA s Workplace Health Steering Committee, a panel of fifteen volunteer members with expertise in various areas that make up employer groups, workplace health researchers, chronic disease prevention scientists, and a labor representative. The questions were adapted from the AHA s longstanding Fit-Friendly Worksite recognition program with additional relevant questions from the Centers for Disease Control and Prevention s Worksite Health Scorecard. 1 The questions are based on the most current science and evidence of workplace health promotion best practices. By participating in the Index, it is understood that the organization is responsible for obtaining all necessary and appropriate consents from participating employees. Scoring and weighting is reviewed annually by the AHA s Workplace Health Steering Committee and may be adjusted to ensure continuous quality improvement. MORE INFO Tip: We recommend that each Worksite apply separately for most accurate score. A Workplace constitutes the entirety of the Organization within the U.S. A Worksite is a geographically unique entity of the overall Workplace. For example: The Workplace Excellent Co. has three Worksites Columbus (headquarters), Boston, and Austin, so ideally there should be three applications. A Workplace will have the ability to roll- up results from individual Worksites to see their overall score. The user has successfully completed the survey when each module has a Completed check. Complete Survey No action is necessary. 57 INCOMPLETE SURVEY If the user has not completed the required questions in the Structure & Process Measures tab, the Index Dashboard will not be available to the user, and the Review module will alert the user to the number of questions requiring an answer within each section. 65 TOTAL INDEX SCORE This report shows the position of the organization s Total Index Score against the average Total Index Score of all companies that fall within the same Worksite Size Classification and the average Total Index Score of all companies that fall within the same Organization Classification.
