This form must be completed and signed by the Farm Unit Manager. This form is available on the IID website at Master Farm Unit Name or Name on Water Account: Master Farm Unit ID# or Water Account # (only one): Bugs Bunny (The master farm unit name and master farm unit ID# are those provided on the Farm Unit Designation form completed for the Equitable Distribution Plan. If no Farm Unit has been designated by that form, the farm unit is comprised of a single water account and information for that water account must be provided.) If no field listed below provides a minimum acre-feet offer of water equal to or less than the farm unit fallowing target, IID may not accept any of the fields offered for fallowing and may not offer an agreement to the farm unit for participation in the program. IID will select and accept whole and/or partial fields resulting in an amount of acre-feet of fallowed water closest to the farm unit fallowing target.
