CA Re: Asbestos Survey Inspection Report Temple Israel-Weisz School Building and Parking Structure 7300 W. Hollywood Blvd. Los Angeles, CA Dear Mr. Merson: On September 12, 13,18 and 19, 2006, Air Lab conducted an asbestos-containing material (ACM) bulk survey at the above referenced location. The field investigation was limited to the above referenced project area and did not include any materials not shown on this report. ASBESTOS SURVEY The building was visually inspected for suspect ACM. The building materials were collected from random areas within the project area. Where possible, complete cross-sections were taken of the suspect materials. A total of Forty-Five (45) samples were collected and analyzed. The Bulk Samples were subsequently submitted for analysis to Patriot Environmental Laboratory Services, Inc. (Patriot). The National Institute of Standards and Technology (NIST) accredits Patriot through participation in the National Voluntary Laboratory Accreditation Program (NVLAP), lab code Sample analysis was performed by Polarized Light Microscopy (PLM) with dispersion staining using the method prescribed in EPA 600lR Laboratory report number is attached Seal Way, Unit D, P.O. Although Thermal System Insulation (TSI) was not identified in the survey, TSI may exist inside walls or above hard lid ceilings. Destructive testing must be performed prior to demolition or renovation. Multiple layers of floor tilelsheets and mastics were found. All base molding and mastics are assumed to contain asbestos in locations where the flooring tilelsheets or mastics contain asbestos Results of the Polarized Light Microscopy (PLM) test in Patriot Report No revealed the remaining materials to not contain asbestos. Sample locations [or the Weisz School Building and Parking Structure are attached. Please be advised, according to the State of California, Department of Industrial Relations, Division of Occupational Safety and Health (CalIOSHA), in Title 8, California Code of Regulations, Section 1529(b), an ACM is currently defined as any material containing more than one percent asbestos. However, the requirements of registration by CallOSHA for asbestosrelated work are currently any material containing greater than 0.1% asbestos. Employers performing construction activities containing any detectable asbestos must comply with all applicable provisions of 8CCR Prior to renovation or demolition, if affected, these materials should be removed and be disposed of as asbestos containing materials by a licensed and qualified asbestos abatement contractor in accordance with all federal, state and local laws, ordinances and regulations.
