Overview The objective of this Expression of Interest is to invite service providers who are youth owned from the age of 18 to 35 and individuals living with disability (youth and adults)-designated groups in terms of amended B-BBEE Codes of Good Practices to express their interest to be considered as approved service providers, as and when required, on the University s Regional Supplier database. Local service providers from a specific region will be given preference in that region. 2. Proprietary and confidential information All material submitted in response to this tender shall become the property of Unisa. Any confidential information provided by a supplier in response to this Tender will be held in confidence and will only be used for the evaluation of this tender. 3. Dealing with the university of South Africa Service providers must not contact any member of Unisa with respect to queries they may have with this tender. The Tenderer shall not disclose any information or specification, whether explicit or implied, to any third party without the written consent from Unisa 4. Mandatory requirements Mandatory requirements will include the following and must be submitted in the same order: Annexure A1: Completed and signed Supplier List Application Form

6 (F25) Annexure A2: Current and valid original SARS Clearance Certificate or E-filing: Current and valid SARS Clearance Certificate Annexure A3: Copy of company registration documents indicating list of Shareholders / members from CIPC / CIPRO Annexure A4: A valid B-BBEE certificate or proof of exemption from an accredited SANAS /IRBA verification agency/auditor. An affidavit certifying their total annual income and level of black ownership will be sufficient for EMEs and QSEs. Failure to submit the above will result in a zero score for B-BBEE. Annexure A5: For youth ownership consideration, a copy of ID (RSA Identity Document) must be attached as proof of youth ownership status Annexure A6: For disability ownership status, a Medical Practitioner certificate must be attached as the proof of disability status Note: All documents submitted in support of this tender must be the documents of the tendering unit and may not pertain to different companies or units within a group. As an example, a tenderer cannot submit its own B-BBEE certificate, but the SARS certificate of its holding company. In the case of sole proprietor we will require the following documents: Annexure A1: Annexure A2: Annexure A3: Completed and signed Supplier List Application Form (F26) Current and valid original SARS Clearance Certificate or E-filing: Current and valid SARS Clearance Certificate A valid B-BBEE certificate or proof of exemption from an accredited SANAS /IRBA verification agency/auditor. 7 Annexure A4: Annexure A5: For youth ownership consideration, a copy of ID (RSA Identity Document) must be attached as proof of youth ownership status For disability ownership status, a Medical Practitioner certificate must be attached as the proof of disability status Note: All documents submitted in support of this tender must be the documents of the tendering unit and may not pertain to different companies or units within a group. As an example, a tenderer cannot submit its own B-BBEE certificate, but the SARS certificate of its holding company. 6. Closing date of documents submissions The required documents must be submitted in one sealed envelope marked with the Expression of Interest reference number EOI2017/19 The documents must be submitted into the official tender box at the regional office address mentioned above. Closing date: 26 September 2017 Documents submitted late will not be considered. 8. Expression of Interest Specifications Objective of the Document: The objective of this Expression of Interest is to invite service providers who are youth owned and people living with disability to express their interest to be considered as approved service providers, as and when required, on the University s Regional Supplier Database. Local service providers from a specific region will be given preference in that region. Goods and services are required in the following regions: Western Cape (Parow and George) Eastern Cape ( East London, Port Elizabeth and Mthatha) KwaZulu Natal (Durban, Pietermaritzburg, Richards Bay and Newcastle) Gauteng (Ekhuruleni, Florida, Johannesburg) Midlands (Rustenburg, Bloemfontein, Kimberley, Potchefstroom, Kroonstad and Mafikeng) Limpopo ( Polokwane, Makhado and Giyani)

Service providers are hereby invited to express their interest to be considered as approved service providers on the University s Regional Supplier database. Above all mentioned in the list of commodities below, a service provider will be given an opportunity to state the speciality/commodity of their own when submitting tender documents.
