1 SECTION A GENERAL SPECIFICATIONS (GS) STANDARD CONTRACT DOCUMENTS FOR MUNICIPAL CONSTRUCTION PROJECTS 2018 EDITION

2 1. GENERAL The General Specifications pertain to items which are not generally specified in the Ontario Provincial Standards. 2. RECOVERED CONCRETE AS GRANULAR MATERIALS GRANULAR 'A', GRANULAR 'B'- SELECT, AND GRANULAR 'C' Granular materials shall be composed of clean, hard, durable un-coated particles obtained from deposits of gravel or sand, talus rock, quarried rock, iron blast furnace or blended nickel slag, clinkers, or recovered Portland cement concrete containing less than 20 per cent by mass of asphalt coated material as determined by petrographic examination, or other suitable granular materials. Granular aggregates from natural or slag sources shall comply with Tables 1 and 2 O.P.S.S as amended by Supplemental Specifications for Sewers. 3. RECOVERED BITUMINOUS PAVEMENT FOR BLENDING WITH GRANULAR FOR GRANULAR "A" The Contractor may blend the recovered bituminous pavement with granular material, such that the blended product contains no more than 30% recovered bituminous pavement by mass. If the granular material consists of crushed Portland cement concrete containing a percentage of bituminous pavement, this percentage must be included in the total amount of bituminous material in the blended product. The recovered bituminous pavement and granular material shall be uniformly blended to meet the Granular "A" gradation and crushed material requirements for the Granular fraction. The Contractor shall ensure that the recovered bituminous pavement does not include steel slag aggregate materials. 4. CONTRACT ADMINISTRATOR'S FIELD OFFICE The Contractor shall provide a field office in a location to be directed for the sole use of the Contract Administrator. The structure shall be weather-proof, insulated, lined with common plywood and painted on the interior and exterior. It shall have an exterior door fitted with a storm/screen door and adequate windows with screens. The overall floor space to be provided shall be 200 square feet and the building shall have an interior lined partition with flush door. 3 Means shall be provided for heating the entire building with forced hot air or similar approved heating. Each room shall be lighted electrically with fluorescent fixtures. The following furnishings and equipment shall be supplied. Used furnishings, in good condition, will be considered: 1 double pedestal desk with key 1 2-drawer filing cabinet 1 plan rack with plan holders 6 office chairs 1 drafting stool 1 sloping plywood plan table 2.4m X 0.9m 1 lock-up cabinet 1.8m X 0.75m X 0.6m deep 1 2.4m shelving 1 air conditioner Adequate road access and parking facilities shall be provided adjacent to the office. The Contractor shall take all reasonable precautions to protect the field office against fire, theft, or other damage and shall indemnify the Corporation and its agents against loss or injury to the building and its contents by fire, theft or any other cause. The Contractor shall provide daily janitor service to clean the field office. At the completion of the project, the Contractor shall remove the Contract Administrator's field office. When a mobile / cellular phone is supplied, the Contractor shall bear the cost of supplying and monthly user charges, except long distance calls. 5. PROJECT SIGN BOARDS Under the item Project Signs in the Form of Tender, the Contractor shall receive and transport two 1.22m x 2.44m (4' x 8') project sign boards from the City of London sign shop (Adelaide North Yard 1159 Adelaide St.N) erect, and maintain the project sign boards for the duration of the construction project, in locations directed by the Contract Administrator. Following construction, prior to release of holdback, the Contractor shall remove the project signs and return to the City of London sign shop, as directed by the Contract Administrator. Payment at the contract price for the above item PROJECT SIGN shall be full compensation for all labour, equipment and material (posts and hardware) to do the work. See project sign board detail. ADVANCED WARNING CONSTRUCTION SIGNS (ARTERIAL ROADS ONLY) The Contractor shall be responsible for supplying, placing and maintaining the advanced warning construction signs on Arterial Roads only. The new construction signs shall be erected prior to the limits of construction at locations determined by the Contract Administrator. The signs shall be installed at least (2) two weeks prior to the start of the project. The new signs shall meet the construction sign design specifications as specified herein: Size of sign 900 mm x 900 mm Font to be Helvetica Bold Sign to have Black Lettering on High Intensity Orange Scotchlite with Black Border Letters to be 88 mm (3 ½ ) unless stated otherwise. Street Name Closed (or Reduced Lanes) (140mm.(5 ½ ) Letters) From Street Name 900 mm To Street Name From Date to Date 900 mm The Contractor shall be responsible for supplying, placing and maintaining the TC-1 Construction Ahead signs according to the Ontario Traffic Manual Book 7. 6 The Contractor shall be responsible for attaching to the bottom of the TC-1 Construction Ahead signs an Expect Delay sign, measuring 600 mm. x 600 mm., using 176 mm. (7 ) Black lettering on High Intensity Orange Scotchlite with a Black Border. Payment at the contract price for this item ADVANCED WARNING CONSTRUCTION SIGNS shall be full compensation for all necessary, labour, material (post and hardware if necessary) and equipment to make the signs and place them at the specified locations as determined by the Contract Administrator. 7. RAISED MAINTENANCE COVERS AHEAD SIGNS The Contractor shall be responsible for supplying, placing and maintaining Raised Maintenance Covers Ahead signs attached to the bottom of the TC-1 Construction Ahead sign. The Raised Maintenance Covers Ahead signs will measure 600 mm x 600 mm using 176 mm. (7 ) Black Lettering on High Intensity Orange Scotchlite with a Black Border. 8. CONSTRUCTION OF SEGMENTAL RETAINING WALL The construction of new segmental retaining walls on the City of London road allowances or City projects by the Owner shall be constructed of units with a minimum weight of 160 kg/unit (typical), and a minimum product weight per square metre of 800 kg/m² wall face. New or reconstructed retaining walls built on the City s property where the elevation difference is greater than 600mm shall have handrails installed as per the Ontario Building Codes (OBC). The following is an overall list of segmental retaining walls approved for use on the City of London road allowances: Split Face Finish Walls (a.) Nava Stone Wallstone Grande (b.) 7 9. CONTRACTORS 24 HOUR CONTACT PERSON The Contractor must provide a contact name and number that can be reached after hours and on weekends for every project they are involved with. The contact person must answer after hour calls and respond to all concerns immediately. If for any reason the City of London dispatch cannot contact the General Contractors after hours designate and the City of London must send a City crew to the job site to address concerns or complaints, the General Contractor shall owe the City a three (3) hour minimum call in charge. The cost for the three (3) hour minimum call in charge will be $ per occurrence. Each additional hour after the first three (3) hours will be $ per hour. The City (Corporation) may deduct any amount due under this paragraph from any monies that may be due or payable to the Contractor on any account whatsoever. The charges are in addition to and without the prejudice of any remedy, action or other alternative that may be available to the Corporation. 10. HEALTH AND SAFETY DOCUMENTATION The City of London may complete a safety audit at the completion of the project. The Contractor shall provide to the City of London copies of the following documentation with every payment certificate: Traffic Control Checklist and Safety Plan (as required) Excavation and Confined Space Checklist report Supervisor s Weekly Report on Contractor Safety Meeting Weekly Contractor Health and Safety Meeting Report Contractor Weekly Safety Meeting Attendance Report The Contractor is required to submit to the City s Contract Administrator copies of all legislative orders and notices sent to MOL, MOE, TSSA, ESA and etc. Any responses or follow up actions should also be provided to the City for both the orders and notices. 8 11. ASBESTOS STANDARD CONTRACT CLAUSES FOR RIGHT AWAY PROJECT CONSTRUCTION The following clauses shall be included in contracts and tenders for right away road construction (i.e., sewer, water, paving etc.) where asbestos may be present. The City is responsible to notify bidders where asbestos may be present. The Notification of Designated Substances in Project form provides this information. Work involving asbestos materials is to be completed in accordance with the Ontario Regulation 278/05 respecting Designated Substances Asbestos on Construction Projects and in Buildings and Repair Operations. Where in the performance of work it is necessary for the successful bidder to disturb asbestos, or to conduct work in close proximity to asbestos materials, the successful bidder shall ensure that its personnel have received training and instruction in asbestos hazards and work methods, in accordance with requirements of the Ontario Regulation 278/05 respecting Designated Substances Asbestos on Construction Projects and in Buildings and Repair Operations. Constructors/Contractors must maintain records that all workers and supervisors involved in asbestos related work have received appropriate instruction and training. For Type 3 Operations the successful constructor/contractor shall provide proof of completion of the Asbestos Abatement Worker Training Program for workers and supervisors in accordance with the Ontario Regulation 278/05 respecting Designated Substances Asbestos on Construction Projects and in Buildings and Repair Operations. During the course of work the Project Manager or City contact shall be notified immediately if asbestos material is discovered that was not on the Notification of Designated Substances on Projects form. The successful constructor/contractor shall notify any subcontractors of the presence and location of asbestos, ensure subcontractors work in accordance with the Ontario Regulation respecting Designated Substances Asbestos on Construction Projects and in Buildings and Repair Operations and submit a written work plan, upon request, prior to the beginning of asbestos related work. GUIDELINES FOR EXCAVATING NEAR GAS LINES Please refer to the Technical Standards and Safety Authority Website, for the manual Guidelines for Excavations in the Vicinity of Gas Lines This manual is to be included with the job site safety documentation. 13. GUIDELINES FOR WORKING AROUND HYDRO ONE INFRASTRUCTURE Contractors must protect Hydro One infrastructure so that they may remain available for service and do not endanger personnel working in their vicinity. All costs associated with coordinating an approved onsite inspector, precautions and procedures for excavating, hydrovac excavation technologies/work, methods for temporarily supporting the U/G plant during excavation, the supply and compaction of special materials for use as bedding, shall be the responsibility of the Contractor. Contractors who will be performing excavation and construction activities in close proximity to Hydro One owned underground assets must obtain certain confidential Hydro One documents, information, and specifications solely for the purposes of performing said excavation and construction activities. These confidential documents outline the necessary precautions for protecting Hydro One s underground transmission cables exposed during construction work performed by outside organizations. Confidential Information includes (but is not limited to) the following Hydro One documents: Protecting High Voltage Power Cables Excavated During Construction Activities; Excavating in the Vicinity of Underground; Excavating with Hydrovac Technology in the Vicinity of Electrical Apparatus; and Line Procurement Specification Cable and Pipe Bedding and Special Thermal Backfill, all of which documents contain or otherwise disclose information which Hydro One treats as confidential or proprietary, whether transmitted orally, electronically or in written form, which the Recipient or its Representatives may receive from Hydro One together with any other documents developed or generated by the Recipient that includes all or any part of the Confidential Information. Hydro One wishes to maintain its proprietary rights to and the confidentiality of the information it discloses and to the recipient solely for the purpose and to ensure that the recipient retains the confidential information in confidence and in accordance with the terms and conditions as stated in Hydro One s CONFIDENTIALITY & PROPRIETARY RIGHTS AGREEMENT. 10 14. GUIDELINES FOR WORKING AROUND HYDRO DUCTS AND WIRES Boring Around Duct Structures Contractors who will be using boring equipment to tunnel under London Hydro s duct structure are advised that it is not possible to identify the exact location of concrete duct structures. Once London Hydro s Underground Plant Locator indicates the general location of the duct structure, as a minimum, the Contractors is required to dig down to expose the top of the structure to determine the depth of the top of the structure and its full width. If the Contractor can monitor the depth of their boring head through electronic means, additional digging is not required as long as the bore is kept at least 4 feet below the top of the exposed duct. If the Contractor does not have a method for monitoring the exact depth of the boring head, they will be required to dig down on one side of the duct structure to find the bottom of the structure and ensure that their bore is kept below this depth. The Contractor is reminded that for wide structures, the centre of the structure is often deeper than the sides. The Contractor must keep their bore sufficiently deep to ensure that the entire structure can be cleared by the boring head. Hydro Duct Structure Supports The following procedures are to be adhered to when a Contractors proposed excavation activity may result in the exposure of an existing London Hydro duct/maintenance hole structure. The Contractor shall assume that London Hydro s existing structure is not reinforced in any way and no load bearing or self-supporting characteristics unless specified by London Hydro Engineering personnel. The structure contains high voltage conductors that are vital to the reliability of the electrical distribution system. The Contractor shall: comply with all requirements of regulations, especially those pertaining to inspection and safety. clearly define proposed work area in relation to the existing underground structure. provide contact name and means of contact during full duration of work, including emergency after hours phone numbers. provide work schedule STANDARD CONTRACT DOCUMENTS FOR MUNICIPAL CONSTRUCTION PROJECTS 2018 EDITION 20

11 The Contractor shall submit: design of all support systems (traverse crossings) and bracing (parallel construction) of plant suitable for existing soils conditions. designs sealed by a registered Professional Engineer in Ontario define whether or not supports are permanent or temporary details of supports and bracings to be used and supplied by the Contractor provisions for not damaging existing plant designs that consider full external loading resulting from the excavation, construction, backfilling technique and backfilling material. for parallel works, the zone of influence of the excavation based on soil conditions provision for dealing with anomalous conditions good engineering and construction practice provision for handling poor weather conditions (rain/water, frost, wind) expected schedule / duration of procedures All submittals must be submitted to London Hydro for review at least one week prior to start of work. Construction shall not proceed until the submittals have been accepted by London Hydro. Acceptance of the above information submitted by the Contractor shall not relieve the Contractor from any responsibility for the adequacy or soundness of such work. All costs associated with preparing and submitting the above information shall be the responsibility of the Contractor. The Contractor is responsible for all damage to existing plant and associated costs resulting from their excavation. This includes any repairs made by London Hydro personnel. London Hydro may require an inspector to be on site during critical stages of the excavation. The Contractor is to contact London Hydro Engineering at (519) Ext to outline the extent of the proposed excavation on the existing structure upon award of the project. London Hydro requires advance payment for any work required by London Hydro personnel. Hydro Pole Supports The Contractor is to contact London Hydro at (519) Ext for hydro pole support assistance at least 3 working days before excavating within 1m (top of slope) of all hydro poles. London Hydro will provide all equipment required to support each pole (ie. Radial Boom Derricks, chains, etc.). London Hydro crews only are allowed to attach and remove supports from each pole. London Hydro requires either a purchase order from the City of London or advanced payment from the Contractor for all costs associated with the supporting of the poles. 12 London Hydro must approve all pole support methods. The method of support will be discussed at the pre-construction meeting. All poles must be firmly supported throughout the duration of the excavation. Line Clearing Around London Hydro s Electric System E.U.S.A.(Electric Utilities Safety Association) states that only competent personnel shall be assigned to perform line clearing operations within 3 meters (10 feet) of energized conductors or apparatus operating in excess of 750V. In order that London Hydro knows who is working around its electric system the following is required: Contractors shall provide London Hydro with the list of qualifications, documentation of training When Contractors need a hold-off, they must inform London Hydro of their work location and must provide London Hydro with a method of communication in the event the feeder trips or the hold-off needs to be revoked. The Contractor is to contact London Hydro Forestry at (519) Ext for assistance at least 3 working days before the start of the project. Overhead Hydro Clearances Occupational Health and safety Act, more specifically, Regulations for General Construction, Section 186 (Electrical Hazards-Proximity to Overhead Electrical Conductors) requires that no object can be within 3 metres (10 feet) to an energized conductor. Therefore, it s the Constructors responsibility to be aware, and to make all personnel on site aware that all equipment and personnel must not encroach the said clearance. 15. COLLECTION OF RESIDENTIAL SOLID WASTE AND RECYCLING When the street is not accessible on garbage collection days, the Contractor shall collect and drop off solid waste and recycling containers at an agreed location on site for pick-up by the City s collection forces. ACCESSIBILITY FOR ONTARIANS WITH DISABILITIES (AODA) The Contractor shall ensure that all its employees, agents, volunteers, or others for whom the Contractor is legally responsible receive training regarding the provision of the goods and services contemplated herein to persons with disabilities in accordance with Section 6 of Ontario Regulation 429/07 (the Regulation ) made under the Accessibility for Ontarians with Disabilities Act, 2005, as amended the Act ). The Contractor shall ensure that such training includes without limitation, a review of the purposes of the Act and the requirements of the Regulation, as well as instruction regarding all matters set out in Section 6 of the Regulation. The Contractor shall submit to the City, as required from time to time, documentation describing its customer service training policies, practices and procedures, and a summary of its training program, together with a record of the dates on which training was provided and a list of the employees, agents volunteers or others who received such training. The City reserves the right to require the Contractor to amend its training policies to meet the requirements of the Act and the Regulation.
