PROJECT NAME: Single Family Residence PROJECT SITE: XXXX Subdivision LOT # Block ADDRESS of SITE: OWNER NAME and ADDRESS: OWNER TELEPHONE NUMBER: SITE AREA: Approximately X ft X X ft (0.5 acre) See the plot plan PROJECT DESCRIPTION: A single-family house. XX bedrooms, XX bathrooms, Kitchen, Living Room, Game Room. Brick and rock outside. Light excavation to remove trees and grade the lot for the construction of house. After 80% of the house is complete the yard will be graded for driveway and landscaping at that time Silt Fence will be removed. RUNOFF COFFICIENT: SEQUENCE OF ACTIVITIES: Survey, Plot Plan, Building Permit, Storm Water Permit, Clear the Lot, Green Box (Toilet), Footing, Foundation, Filling, Framing, Bricking, Gas and Electricity pipeline, Grading the lot, Landscaping, and Final clean up
2 Page 2 NAME OF RECEIVING STREAM: Insert name of receiving stream. EXISTING SOIL INFORMATION Insert type of soil. ENDANGERED SPECIES This construction project is located in a developed subdivision; therefore, there are no endangered or threatened species affected by the project. EROSION AND SEDIMENT CONTROLS: Silt fence and Hay Bales will be installed as necessary to reduce or eliminate the runoff from the site to the water of the state or municipal storm sewer. (See included map for the direction of the flow and the erosion control measures used.) TIMING OF CONTROLS/MEASURES Silt Fences and/or hay bale walls will be installed in areas where needed prior to clearing the lot. When the site is stabilized (sod), silt fence and other erosion controls will be removed by the trash company. WASTE MATERIALS: Trash will be collected in the trash container on the site. The container will be emptied by the trash company. SANITARY WASTE: Portable Toilet from XXXX (Company Name) Rentals will be on the site after the clearing of the lot and remain until 90% of job is complete. Service of the portable toilet will be done by the XXXX Rentals. CONTROLS INSPECTION AND MAINTANCE PRACTIES: Erosion controls will be inspected weekly or after any storm event of 0.5 inches or greater to make sure the controls are in affect and do not require any maintenance. All erosion controls will be maintained in good working order. Any repair or replacement will be done as soon as possible but no later than 5 days after the inspection. NON-STORM WATER DISCHARGES: None
3 Page 3 MATERIAL MANAGEMENT PRACTIES: An effort will be made to store only enough products (Cements, sand, and dirt) to do the job. The project superintendent will inspect daily to ensure all materials are on the site and not in the street. COMPLETION OF JOB: After completion of the sewer line, water line, electricity line, and gas line the lot will be graded and ready for landscaping. All erosion controls will be removed and the street will be cleaned to make sure it is free of debris. Member of Company: Name of the people on the job site and their position. Each of the members are allowed to make any changes necessary to the erosion controls on the job site or this SWPPP. 303(d) list, TMDL, others: Keep one of the following paragraphs whichever is applicable. The storm water from the construction site discharges to XXXX (receiving stream). This water body is not on the list of impaired water bodies (i.e., 303(d) list) for siltation/turbidity. Since this water body is not impaired, a TMDL is not applicable to this construction project. or The storm water from the construction site discharges to XXXX (receiving stream). This water body is on the list of impaired water bodies (i.e., 303(d) list) for siltation/turbidity. Even though this project discharges to an impaired water body, a TMDL has not been written for the water body. Therefore, a TMDL is not applicable to this construction project. or The storm water from the construction site discharges to XXXX (receiving stream). This water body is on the list of impaired water bodies (i.e., 303(d) list) for siltation/turbidity. A TMDL has been written for the water body that is applicable to the construction project. The following information documents the construction projects compliance with the TMDL. Must include documentation that the storm water discharges from the site will meet the applicable TMDL guidelines.
