1 E-Rate YEAR 16 ( ) REQUEST FOR PROPOSAL CICS Consortium Internal Connections Reference # CICS 2014 Wiring - Rockford Form 470 Application Number: February 5, 2013 Chicago International Charter School (CICS) is seeking proposals on behalf of the CICS Consortium to provide E-Rate eligible products and services for the funding year. This request for proposal is to upgrade the LAN wiring at one school building. Service Provider Criteria and Proposal Requirements: CICS expects each Service Provider to make themselves thoroughly familiar and compliant with the rules and regulations of the E-Rate program. Proposals must be received on or before March 5, Proposals received after this date will be disqualified. Proposals should be sent by to the following address: erate @chicagointl.org. Vendor must have a valid SLD Service Provider Identification Number [SPIN] and never have been suspended or debarred from the E-Rate program. Vendor must have a valid FCC Registration Number. Any potential bidder found to be in Red-Light Status will be disqualified from participation in the bidding process and will be considered non-responsive. Vendor must provide a certificate of liability insurance Vendor must be licensed to operate in the state of Illinois If applicable, vendor must be licensed to install low voltage electronic cabling systems in the state of Illinois Descriptions of products and services must provide sufficient line item detail, and be submitted in a suitable format, for use as Form 471, Item 21 attachments, without the need for subsequent modification by the applicant. Failure to document all of the above qualifications in the proposal will result in a disqualification of the proposal. In addition, a walkthrough will be required for all vendors prior to submission of the vendor s proposal. Walkthroughs can be scheduled by request by ing erate @chicagointl.org with the subject. E-Rate Walkthrough. Walkthroughs are by appointment only. It is the vendor s responsibility to arrange for the walkthrough. Failure to attend a walkthrough will result in disqualification of the proposal.
2 Additional Requirements All work must be completed by unionized labor. Vendors must bid on the entire scope of work. Vendors must segregate the bids for each school building. No subcontracting will be allowed without prior written approval of CICS. In the event of questions during the E-Rate application review process the successful vendor is expected to reply to questions associated with their proposal within three days. Pricing and Contracts Prices must be held firm for the duration of the E-Rate Year 16 fiscal year ending September 30, 2014 or until all work associated with the project is complete (including any SLD approved extensions). CICS shall have the option to extend the contract for two additional fifteen month periods under the same terms, conditions and prices contained in the contract. CICS will consider all allowable forms of contract including multi-year contracts and contracts with voluntary extensions All contracts entered into as a result of this RFP will be contingent upon the receipt of the funding award notification from the Universal Service Administrative Company ( USAC ), and/or the Schools and Libraries Division ( SLD ). The winning vendor should not start work or incur expenses until explicitly notified to do so in writing by CICS; notification of award is not in itself authorization to start work. No work can begin and no expenses can be incurred prior to July 1, CICS has a right to cancel or reduce the scope of any project for which E-Rate funding has been requested. CICS reserves the right to deny any or all proposals associated with this RFP, even with SLD funding approval. CICS may require that the work occur in multiple phases. Proposer must review and comply with the requirements detailed in: o AIA o AIA
3 Scope of Work Please see the attached documents for all details related to the scope of work: Appendix A: Technology Cabling System Requirements Appendix B: Construction Drawings Appendix C: AIA Appendix D: AIA The Vendor is responsible for coordinating efforts with the Client to ensure a well-managed installation, including but not be limited to scheduling and planning with the Client s team and working with local Client representatives. The Vendor is responsible for working in conjunction and coordinating with Client s General Contractor and all its sub-contractors, including but not limited to the electrical subcontractor. All work will be performed in accordance with the project timeline in a professional manner, compatible with the Client s normal or unique business requirements. All work will be performed in accordance with the project timeline as determined by the Client s General Contractor. It is assumed installation can be performed during normal workweek times. Vendors should include costs for any premium or off-hour efforts, as they deem necessary.
4 Proposal Instructions The proposed installation must include the following: Adherence to all notes contained in the structured cabling drawings as well as the project manual. A site survey/walkthrough will be required to be completed with a member of the school s technology staff in order to confirm locations prior to installation. Winning bidder must provide the schools with schematic layout of all work completed Vendor will warranty all parts and labor for 1 year from the date of final acceptance. Vendor must complete all work when school is not in session. All E-Rate work must be completed no later than September 30, Vendor will work with CICS to schedule this wiring. No work may commence without explicit notification from CICS. Work may be required to be done in several phases depending on the timing of the E-Rate notification and the school calendar. Vendor response must break down the bid s gross total using the unit cost method and the following parameters: o Bill of materials (manufacturer) o Part number o Quantity o Unit of measurement o Price per unit (material) o Price per unit (labor) o Total Suggested Proposals Format The Vendor s proposal should, at a minimum, include the following: 1. Provide your Vendor Name, Illinois Business License Number, FCC Registration Number, Address, certificate of insurance and SPIN 2. Provide the Date of Submittal to this RFP 3. Provide a description of similar E-Rate installations and/or service contracts 4. Provide a description of similar experience with other charter and/or public schools 5. Provide information relating to your company s manufacturer certifications 6. Provide information regarding your management capabilities 7. Provide information regarding personnel qualifications. 8. Provide a brief discussion on your experience with E-Rate including any support for Selective Reviews, PIA Challenges and Appeals involving Service Provider issues 9. Provide a minimum of three (3) client references 10. Provide a cost proposal and description for the products and/or services described above in the Scope of Work. This should be submitted in a suitable format for use as Form 471, Item 21 attachment 11. Separately itemize the cost of E-rate eligible and ineligible products and installation, if any. 12. Vendor shall provide documentation to certify that all assigned staff to this project has attended training courses corresponding to the type of cabling and equipment specified herein.
5 Bid Evaluation CICS will evaluate all complete proposals using the following factors and weights. Please note that in accordance with E-Rate cost-effective requirements, price is the most heavily weighted factor in the evaluation. Factor Weight Price 25% Completeness of Proposal (How well does the plan meet CICS needs) 20% Prior track record with E-Rate 10% Prior experience in K-12 setting providing similar services 20% References/Reputation 20% Management and Personnel Qualifications 5% 100% Additional Information and Addenda Questions or requests for clarification of this RFP should be sent to the bid submission address, Answers to questions of general interest from qualified vendors may be posted on the web site and should be considered to be formal addenda to this RFP. Technology contractor shall furnish, install and test all cabling and equipment necessary for a complete and functioning technology cabling system as specified and indicated on the drawings. Conduit, raceway and outlet boxes for the Information Outlets shall be provided as indicated in these project documents shall be provided by the electrical contractor on the construction project. The 4000 Wiremold and Wiremold for the Wall Telephones shall be provided by the technology contractor as part of this E Rate Technology project. B. Technology contractor shall insure the cable run from the technology closet to the information outlet does not exceed 90 meters (296 feet) C. Furnish and install all faceplates, patch panels, equipment racks, equipment trays and all other items necessary for a complete technology cabling system. D. The cabling system shall be comply to EIA/TIA 568B, TSB-67, and ISO/TEC IS PROJECT/SITE CONDITIONS A. Examine areas and conditions under which all items are to be installed, and notify the project manager in writing of conditions detrimental to proper completion of the work. Do not proceed with that portion of the work affected until unsatisfactory conditions have been corrected in a manner acceptable to installer SEQUENCING/COORDINATION A. Technology contractor shall coordinate the receipt of all materials being provided by the owner to ensure installation proceeds without delays. B. Technology contractor shall attend meetings as required by the project manager. C. Technology contractor shall review and be familiar with the construction project PATHWAYS FOR TECHNOLOGY SYSTEMS A. Electrical contractor shall provide the required conduit stubs and back boxes for all devices shown on the technology plans. In general requirements are a 4 square box with a single gang ring Provide a 1 min conduit with a threaded bushing to an accessible area. NOTE: Provide the specification sheets with the proposal response. NOTE: Provide THREE samples of EACH information outlet labeled with a 2' Minimum of each cable terminated shall be required for approval...shall be submitted with Initial Project Documentation 2. Complete itemized Bill of Material NOTE: Provide the itemized Bill of Material with the proposal response. 3. "Logical Flow Diagram" showing the following; (Computer generated using Visio Professional or equivalent) a. Local Exchange Carrier (LEC) Network Interface (if applicable) b. Main Technology Closet and other Technology Closets. c. Integration/Interfacing to other systems 4. Proposed layout of EACH equipment room. (Computer generated using Visio Professional or equivalent) a. Location of the equipment rack(s) in the room. b. Location of ALL rack mounted equipment in each equipment rack. 5. Complete listing of Charges as follows: a. Hourly rate for maintenance. b. Hourly rate for installation. 6. Documentation as follows: a. The technology contractor is an authorized dealer/distributor of the products and systems to be provided. b. The technicians assigned to this project have been properly trained on the installation/maintenance of the model/type of cabling system to be provided. Final Project Documentation: Technology contractor shall submit the following project documentation to the project manager for approval prior to Final Acceptance PROVIDE AS REQUIRED 1.08 QUALIFICATIONS 1. As built drawings shall include by not limited to the following: a. Any changes made to the system during installation. b. Location of all information outlets including jack number identification generated by AutoCAD text. 2. As built drawings shall be as follows: a. ONE copy in Full Size plan. b. ONE CD with AutoCAD files. c. ONE copy in REDUCED size - 11" X 17" (Included in the Technical Manual) 3. Technology contractor shall furnish ONE complete technical manual containing the following: a. Layout of EACH rack in EACH equipment room as installed. (Visio Professional or equivalent) b. Layout of EACH device in EACH equipment rack as installed. (Visio Professional or equivalent) c. All testing reports per Section 3.03 (Category SIX test reports MUST include a summary page(s) per 1.07D1c) d. REDUCED size - 11" X 17" as built drawing with the correct location of all information outlets & jack number generated by AutoCAD text. A. Technology contractor shall currently be licensed to install low voltage electronic cabling systems in the State if applicable. B. Technology contractor shall currently meet all manufacturers requirements for the provision and installation of all equipment specified herein. C. Technology contractor shall utilize and have operators trained in the utilization of the Category 6 and fiber optic test equipment. 1. Category 6 test equipment (FLUKE DSP 4000/DSP4100) a. Set Pass/Fail range for distance at 90 Meters (296 Feet) b. Test for applicable Category 6 standards. L&D # TECHNOLOGY CABLING SYSTEM

One copy of the Summary Page(s) is REQUIRED PRIOR to Substantial Completion. 2. Fiber Optic Cable test equipment (Lucent Technologies or equivalent) 1.09 MAINTENANCE SERVICE a. Test for loss 850 nm and 1300 nm for multimode 1310 nm and 1550 nm for single mode b. Provide length for each fiber optic cable. c. Results must be in electronic generated text stated in DB and generated from an electronic readout of the fiber test equipment. A. Provide warranty of cabling system for TWENTY years from date of final acceptance. PART 2 - PRODUCTS 2.01 GENERAL 1. Warranty: Warrant the cabling system against malfunction due to component failure or improper installation for a period of TWENTY years from the date of final acceptance. When notified of a malfunction, proceed to immediately correct the situation by replacement or repair without cost to the owner. Extend manufacturer's warranties when necessary to achieve the full duration. Clearly indicate provisions in the warranty manual. A. All products bid shall be the most current and up-to-date versions available, unless otherwise specified. B. Technology contractor shall provide written notification to the project manager, prior to submitting the bid, of any discrepancies in model or part numbers specified. C. Technology contractor shall provide written notification to the project manager, prior to bidding, of the announced discontinuation or replacement of any items specified herein. D. Technology contractor shall provide all necessary supplies, mounting hardware and accessories required to install the materials specified herein PRODUCT SUBSTITUTIONS A. All specified equipment detailed herein must, at a minimum, meet the following specifications or their equivalent and/or the equipment s replacement/updated version if appropriate MATERIALS A. Cabling 1. Horizontal Copper Cabling: Furnish and install 4-pair 24 awg twisted-pair Category 6 requirements. One dedicated continuous run from each jack to the patch panel. Riser Cable: Furnish and install the following: Main Technology Closet to the Technology Cabinet: Furnish and install a 12 Strand Multimode Fiber 50 Micron between the Main Technology Closet and the Technology Cabinet. Terminate all strands at both ends on a rack mounted patch panels in LC connectors. Fiber Cable and Connectors shall be OM3 Grade Commscope Uniprise or equivalent. Copper Riser Cable: Furnish one 50 Pair Category THREE from the MTC to the TC Terminate on a wall mounted 110 block adjacent to the network interface and terminate on a rack mounted 110 blocks at the MTC. T1 / PRI Cable: Furnish and install a quantity of TWO - 2 pair 24 awg individually shielded cable from the Main Technology Closet (MTC) to Local Exchange Carrier (LEC) network interface Coil TEN feet of cable at each end. (Belden, Berk-Tek, Commscope, or equivalent) 3. Faceplates: shall be Panduit or equivalent. Quantities as required.see details at the end of this section. 4. To be completed prior to Final Acceptance. D. All items must be complete as specified prior to Final Acceptance. It will be the responsibility of the technology contractor to ensure cabling meets all specifications and standards defined herein. E. It will be the responsibility of the technology contractor to insure that any cable run from the MTC/TC to the Information Outlet jack does NOT exceed 90 Meters (296 Feet) INSTALLATION A. Cabling - General: 1 Furnish and install cabling runs between the MTC/TC to each information outlet specified on drawings. A dedicated cabling run shall be utilized for each faceplate jack. 2 Conduit sleeves shall be provided by the electrical contractor on the construction project as identified by the technology contractor in the proposal response...fire stopping by the technology contractor as follows: The exterior perimeter of the NEW sleeve shall be fire stopped at installation with a paintable material. The interior ALL sleeves shall be fire stopped prior to substantial completion. 3. Furnish and install face plates and faceplate jacks in outlet boxes for all information outlets specified on drawings. 4. All jacks shall be terminated in accordance with cable pinout detail 568B 5. Excess cable behind face plate connections shall be pulled back into ceiling spaces and secured in such a manner as to prevent damage to cabling or connections. 6. A minimum 10 foot loop of extra cable shall be secured in accessible ceiling space for outlets installed in movable partition walls or power poles. 7. Furnish and install grommets and/or bushings in conduit ends PRIOR TO INSTALLING the cabling to prevent damage to insulation or conductors. 8. Use Velcro Bands to secure cable bundles in the MTC and TC locations. Cable Ties NOT used on this project.to the cable bundles Cable ties MUST be able to be turned freely around the bundle of cable 9. Tighten connectors and terminals, including screws and bolts, in accordance with equipment manufacturer's published torque tightening values for equipment connectors. 10. Allow sufficient slack in cable to prevent premature deterioration of cable system components and to assist in the maintenance and servicing of cable and/or other building systems and components. Avoid excessive and sharp bends. Ensure manufacturer's recommended pulling tensions are not exceeded. L&D # TECHNOLOGY CABLING SYSTEM
14 E RATE TECHNOLOGY CICS JACKSON SCHOOL ROCKFORD, ILLINOIS 4Feb All cabling shall be in J hooks deployed 4 or less and anchored to the building structure Additional J Hooks required if the cabling sags. 12. Fittings or connections are allowed only at the input and output of devices. Splicing shall not be accepted in any cable run. The entire cable run shall be replaced in all such instances. B. Grounding: Ground all equipment per manufacturers' instructions, NEC Guidelines, EIA/TIA 568, and EIA/TIA A Grounding Buss Bar to be provided by the electrical contractor on the construction project at the MTC, TC, and the Local Exchange Carrier (LEC) Network Interface. 2. Technology Contractor shall provide a # 6 Green Stranded Ground Wire from EACH equipment rack at the MTC/TC to the building structure and SECOND ground wire from the MTC/TC equipment rack to the Grounding Buss Bar. C. Labeling: 3.03 ADJUSTING/TESTING 1. Use the Information Outlet Coding System as shown on Sheet T Pan Net machine fed labels or prior approved equal self-adhesive typed labels shall be utilized as follows: a. Place labels on face plates adjacent to EACH jack. b. Label all patch panel terminations at EACH jack location. c. Label the EACH END of all cabling runs with a label at 3" to 5" from the termination at the patch panel and at the jack. NOTE: All labels must be of the same, size, font and manufacturer. Provide THREE samples of EACH information outlet labeled with a 2' Minimum of each cable terminated shall be required for approval shall be submitted with Initial Project Documentation A. Test all cabling and connections with specified equipment and certify as meeting all specifications. Provide hard copy test results for each cabling run in Technical Manual (Section D) B. Provide CD of the test reports with instructions and software for viewing SUBSTANTIAL COMPLETION AND FINAL ACCEPTANCE A.
