2 Must Do s: 1. Have you completed the relevant site and/or company induction and are the site specific policies and procedures understood? 2. Are the relevant permits in place, understood and signed off by an authorised person? 3. Have you completed a SWMS/Risk Assessment for the work/task you are involved in and/or about to undertake? 4. Is your SWMS/Risk Assessment easily understood and signed off by an authorised person? 5. Has the mobile plant/piece of equipment you are about to use been inspected and/or is certified as per state regulatory requirements? 6. Have you completed a pre-start/operational inspection for the mobile plant/equipment you are about to use? 7. Have you read and are you familiar with any operators manuals/instructions for the piece of mobile plant/equipment you are about to use? 8. What is your rescue plan in the event of an emergency or injury? 9. Are you wearing the appropriate PPE for the work/task you are about to undertake? 10. Do you have the appropriate training and/or Licence for the work/task you are about to undertake?

4 Your Skills Passport This Skills Passport acts as a workplace consistent portable record of an individual s training and inductions. Purpose of this Passport Passports can or will be issued to anyone who: Is working under a PCBU (an employer) who requires their workers to hold and maintain a passport. Feels that the information and purpose of this Passport may assist them with any activity and/or work/task conducted in or at a workplace. Feels that this Passport may assist their business or organisation with the record keeping and compliance duties of its workers, contractors and/or sub-contractors. 1. Only one Passport shall be issued to any one person at any one time. 2. This Passport is issued with a unique number. 3. This Passport must be carried at all times and/or be in close proximity to any work/task being undertaken in or at a workplace. 4. This Passport is not intended to be legally binding but, if a PCBU (your employer) has accepted this into their business or organisation then carrying the Passport and having the information updated within this Passport may then become mandatory. Passport Entries A /Nominated person is a person who has been given authorisation within a business or organisation who has either conducted the training/assessment and/or has been authorised to sight, validate and then enter course records, certificates and/or Licence information into an Individual s Skills Passport. Who can make entries and/or complete information within a Skills Passport: A /Nominated person within a PCBU or organisation and/or Any Registered Training Organisation (RTO) and/or any selected Training Organisation working under a 3rd Party arrangement with an RTO. All entries must be handwritten, stamped and dated by the above described person. In the absence of a stamp, details can be handwritten with the name of /Nominated person/rto inserted into the stamp and date column of the Skills Passport. Where no Retraining date is listed on a training course record, certificate and/or Licence, then the relevant Australian Standard and/or a PCBU s policy and procedure must be referenced so as to ensure a valid entry can be made into the Retraining date column of The Skills Passport. Where the Retraining date is unknown or specified, the Retraining column of The Skills Passport column is to be left blank. Previous training course records/certificates and/or Licence information can be added into The Skills Passport but only after the original certificate/licence is provided, sighted/verified and can be validated by a /Nominated person and/or RTO (and/or as by persons described above). Passport Holder responsibilities The holder of The Skills Passport is responsible for: Ensuring that the information is correct and maintained. Must be carried at all times and/or be in close proximity to any work/task being undertaken. Presenting it to an authorised person on request (i.e. auditing/workplace inspection purposes). Providing relevant and authentic information when requested by a /RTO. Lost or found Passports Lost Passports are to be reported immediately to your PCBU (employer).
