1 REQUEST FOR QUOTES: # LE Navajo Division of Transportation Department of Airports Management ADOT GRANT #E5S2R - Independent Fee Estimate - Reconstruct Runway 15/33 (Approximately 2,000 x 75 ) The Navajo Division of Transportation (Navajo DOT) is requesting request for proposal from qualified firms to provide Independent Fee Estimate (IFE) services for Reconstruct Runway 15/33 (Approximately 2,000 x 75 ) project at the, Tuba City, Arizona. The IFE must be conducted in accordance with the Federal Aviation Administration s Airport Improvement Program (AIP) Handbook Order D, Sponsor Requirements for Cost Reasonableness. 2 TASK ORDER A ATTACHMENT TO PROFESSIONAL SERVICES AGREEMENT BETWEEN OWNER AND ENGINEER, DATED, 2015 FURTHER DESCRIPTION OF SERVICES OF ENGINEER 1. This Attachment is made a part of and incorporated by reference into the Professional Services Agreement made on between the NAVAJO NATION (Owner) and ARMSTRONG CONSULTANTS, INC., (Engineer) providing for professional engineering services. The Services of Engineer as described in Section 1 of the Agreement are amended or supplemented as indicated below and the time periods for the performance of certain services are stipulated as indicated below. 2. 3 SCOPE OF WORK TUBA CITY AIRPORT ADOT #E5S2R ELEMENT #1 RECONSTRUCT RUNWAY 15/33 (Approximately 4,600 x 75 ) Runway 15/33 is 6,230 feet long and is showing signs of failure in roughly the middle two thirds of the runway length. The runway ends appear to have been rehabilitated at some time in the past and are planned to be rehabilitated again by ADOT s APMS program with what is anticipated to be a one inch overlay. This scope of work provides for the design of the reconstruction of the remaining portion of the runway (approximately 4,600 feet) a bid package that reduces the length of reconstruction to fit within the ADOT E5S2R grant, and construction services for the reduced runway length project. The design will not include adjustment of the edge lights as it is not noted in the grant description, as a result, the finished slopes between the runway edge and the edge lighting may not be to FAA standard. This may need to be addressed in a future project. Element 1 includes the follow design considerations. 1.1 A geotechnical investigation will be conducted for the full length of the runway. 1.2 The design survey will topo the full length of the runway out to the outer edges of the safety area. 1.3 The drainage pattern and impervious areas will not change as part of this project and therefore no drainage design will be provided. 1.4 The pavement design will be calculated with FAARFIELD at 12,500 lbs in a single wheal gear configuration. Materials used will meet the FAA design standards. Based upon observations the existing pavement will likely not be able to be recycled for use in the pavement section but if the geotechnical investigation shows otherwise, it will be reused. 1.5 The proposed runway profile and transverse slopes will meet the FAA design criteria where possible within the limits of this project. 1.6 Erosion control measures may be installed in some areas adjacent to the new pavement if site conditions require and budget allows. 1.7 A centerline will be marked in accordance with Advisory Circular 150/5340-1L with Type I, grade A glass beads. 1.8 Shoulder grading will be included in the design from the edge of pavement to no further than the edge lights. This project is not intended to correct any safety area deficiencies, and therefore grading will not continue beyond the edge lights. Estimated Construction Cost for the 4,600 foot reconstruction is $2,800,000. The construction cost is dependent upon the availability of a suitable aggregate source. 2

4 PHASE 1 - PRELIMINARY DESIGN FOR ELEMENT 1 The design phase will assemble the design documents into the appropriate bid schedules and work elements to meet available funding and project phasing and sequencing requirements. Construction schedules will be coordinated to attempt synchronizing the least interference with airport operations, and appropriate timing relative to ADOT MPD Aeronautics Group funding schedules. The designer will complete its evaluation of alternatives and provide well-defined constriction requirements. Activities include: 1. Coordinate with the Owner, ADOT MPD Aeronautics Group and local users to minimize impacts to day-to-day operations. This will include various meetings during the design phase to review the progress of the design and discuss construction details, proposed time frame of the construction, special requirements of the project, and field information gathering. It is anticipated that there will be two meetings with the Owner and ADOT-MPD Aeronautics Group for this item. 2. Prepare project scope of work and contract. This includes establishing the scope of work through meeting with the Owner and ADOT MPD Aeronautics Group. This also includes drafting the contract for work to be done by the Engineer for the Owner. Prepare preliminary cost estimates and schematic design for each element of the project. 3. Preparing requirements for the design survey, establishing the limits of the work area and scheduling time for surveying to be completed are each part of this item. a. This task includes preparing the limits of work and time schedule for the topographical surveying for this project. b. The Scope will be prepared and submitted to subconsultants for determining proposed fees. 4. Analyze topographic and site survey data, including establishment of project control points. Prepare the data for use with computer modeling. Included are the following separate tasks: a. Input raw survey data into the computer program in order to sort data into company standard layers for efficient analysis. b. Sort all data points by layers and description for computer modeling. c. Prepare LDD network (surface model) of existing ground contours, pavement edges, electrical equipment, drainage features and other miscellaneous entities. d. Generate three-dimensional contour model from TIN. e. Prepare surface for pavement profiles, grading and/or paving cross sections and drainage features. 3

5 5. Prepare an overall construction phasing plan in order to maximize project constructability. 6. Determine fleet mix through coordination with Owner, State and FAA. 7. Inventory local material suppliers, sources and capabilities. 8. Complete a soils investigation, soils report, and provide recommendations including: a. Field Exploration i. Obtain thirty two (32) soil borings on the runway area. Laboratory testing shall be conducted on the borings. b. Laboratory Testing Test Number of Tests -200 sieve analysis 96 each In place moisture density 96 each Atterburgs 11 each Swell/Consolidation 32 each CBR & Proctors 11 each Sulfates 11 each.02mm gradations 11 each c. Special Investigation: During a field investigation of the runway, the runway surface appeared to lift up approximately three inches in an angular pattern across the runway. This was noted in two areas. An additional investigation will be conducted by the geotechnical sub consultant to investigate the issue. If necessary, a design to prevent future lifting in the problem area will be completed. 9. Analyze pavement and soils testing data. The Engineer will analyze the data, consisting of the following tasks: a. Generate geotechnical conclusions b. Prepare pavement data and soil information for incorporation within the contract documents. c. Prepare pavement data and soil information for incorporation within the contract documents. 10. Prepare 30% construction plans. Construction plans will be prepared depicting all work involved for Element 1. The following list of drawings will be used as a guideline. Drawings may be added or deleted during the design phase if required. a DESCRIPTION ELEMENT 1 Cover sheet- project title, project/grant number, funding agencies, index to drawings, project key and vicinity maps 1 Sheet 4

6 b c Survey Control Map, Summary of Approximate Quantities, General Notes and Legends, where applicable Construction Safety and Phasing Plan- This plan will identify all phasing and construction and safety notes for the project. It will identify to the Contractor the sequencing of work. Phasing will be required between Element 1 1 Sheet 2 Sheets d Grading and Drainage Plans 6 Sheet e Typical Sections and Details 2 Sheet f Pavement Marking Plans 1 Sheet g Pavement Marking Details 1 Sheet h Plan and Profile Sheets for Runway 6/24 depict the runway centerline profiles; ft. (8 STA) per sheet 6 Sheets i Connector Taxiway Tie-Ins 1 Sheet j Cross Section Sheets- (Runway) 6 Sheets TOTAL SHEET COUNT 27 Sheets 11. Prepare 30% contractor contract document packages. The Engineer will prepare contract documents including invitation for bids, instructions to bidders, proposal, equal employment opportunity clauses and applicable wage rates, construction contract agreement, performance bond, payment bond, general and special provisions. Preparation will include establishing the location for the bid opening and description of the work schedule. Contract documents will be prepared as early as possible during the design phase and submitted to the Owner and ADOT MPD Aeronautics Group. 12. The Engineer will assemble the 30% technical specifications necessary for the intended work. Standard FAA specifications will be utilized where possible. Additional specifications will be prepared to address work items or material that is not covered by the FAA specifications. The standard specifications to be utilized may include the following items: Item P-151 Item P-152 Item P-208 Item P-401 Item P-602 Item P-603 Item P-610 Item P-620 Item T-901/908 Clearing and Grubbing Excavation and Embankment Aggregate Base Course Plan Mix Bituminous Pavements Bituminous Prime Coat Bituminous Tack Coat Structural Portland Cement Concrete Runway and Taxiway Painting Seeding/Mulching The added technical specifications may include but not be limited to the following items: Item Special-1 Item Special-2 Mobilization Removals (Pavement, lights and signage) 5

7 Item Special-6 Watering 13. Prepare 30% engineer s design report packages. During the preparation of the construction plans and specifications, an engineer design report will be prepared. The report will include the summary of the project, pavement, drainage design, schedule and cost estimate for the completion of the project. The design report will follow the current FAA Airports guidance. 14. Solicit comments on preliminary design from ADOT-MPD Aeronautics Group including attending 30% review meeting. PHASE 2 FINAL DESIGN FOR ELEMENT 1 In the final design phase, the designer will provide well-defined construction requirements, with selected bid alternatives as appropriate to allow competitive construction bids. Construction schedules will be closely coordinated with weather conditions to minimize interference with airport operations. Activities include: 1. Incorporate engineering phase design comments and respond as necessary to requests for additional information. 2. Calculate Estimated Quantities. The Engineer will calculate all necessary quantities for the various work items in each Element. 3. Prepare Estimate of Probable Construction Cost for Element 1. Using the final quantities calculated following the completion of the plans and specifications, the Engineer will prepare the construction cost estimate. The estimate will be based on information obtained from previous projects, contractors, material suppliers and other databases available. 4. Prepare and submit final plans and specifications. Copies will be submitted to the ADOT MPD Aeronautics Group and Owner. A final set of plans, specifications and contract documents will be prepared which incorporates revisions, modifications and corrections determined during the ADOT MPD Aeronautics Group and Owner s review. 5. Prepare final engineer s design report. The final engineer s design report will include comments from the 30% review and incorporate the final design data. 6. Prepare and submit final plans and specifications. A final set of plans, specifications and contract documents will be prepared which incorporates revisions, modifications and corrections determined during the ADOT MPD Aeronautics Group and Owner s review. 7. An ADOT environmental form will be required to be completed and submitted to the ADOT MPD Aeronautics Group. 6

8 BIDDING SERVICES 1. Assist the Owner with the advertisement for bids, attend prebid conference at the airport, issue any addendums required and interpretation of the project requirements. 2. Provide technical assistance during bidding. 3. Attend and assist the Owner with the bid opening. 4. Prepare an abstract of bids and make recommendation for award. 5. Assist in award notification to successful bidder and notify and return bid bonds to the unsuccessful bidders. CONSTRUCTION SERVICES During the construction phase of the project, the designer will assist the Owner to monitor and document progress for quality and cost control. Review contractor payment requests, quality control and acceptance testing, establish necessary survey control, inform the Owner of project progress and problems, complete the quality assurance (QA) test summary, conduct the final project inspection and complete the final project report. Activities include: 1. Coordinate construction contract agreement, review bonds, insurance certificates, construction schedules, etc. 2. Review and accept the Contractor s Safety Plan Compliance Document prior to issuing the Notice to Proceed. 3. Coordinate a Construction Management Plan with the contractor prior to paving operations taking place. 4. No AGIS survey requirements are to be conducted as a part of this contract or project. 5. Conduct pre-construction conference. 6. Provide review of all submittals for materials to be used on the project. Review all shop drawings items as required during construction. 7. Provide technical assistance and recommendations to the airport during construction. 7

9 8. Provide a full-time resident inspector to monitor and document construction progress for Element 1, confirm conformance with schedules, plans and specifications, measure and document construction pay quantities, document significant conversations or situations, document input or visits by local authorities, etc. 9. Prepare change orders and supplemental agreements, if required. All coordination of change orders will be provided by the Engineer. 10. Prepare and submit weekly inspection reports. Reports will be submitted to the ADOT-MPD Aeronautics Group and Sponsor. 11. Prepare and confirm monthly payment requests. Payment requests will be reviewed for accuracy with contractor and project inspector. Engineer will prepare ADOT reimbursement requests for the Owner. 12. Prepare and submit the project QA test summary. 13. Conduct final project inspection with the Owner and ADOT-MPD Aeronautics Group and the contractor. Any punch list items will be noted and coordinated with the contractor for necessary action. 14. Prepare record drawings and a final project report. The final report will follow the current FAA AIP Final Report guidance. ADOT-MPD Aeronautics Group and the Sponsor will each receive a copy of the record drawings in the format they choose. 8
