It lists the risk factors identified during the physical site evaluation and recommends appropriate measures to address these risks. Description of Identified Risk Factors Generally speaking, work spaces were found to have appropriate security in place throughout the campus. However, union representatives on the November 22, 2011 physical site evaluation identified the following: 1. In certain areas that serve the public, the cubicle configurations make it difficult for employees to see the public entering. 2. Not all faculty and staff know the campus emergency phone number. 3. Concerns about lighting in a few areas. 1

How the identified risk factors have been/are being addressed Identified areas of concerns will be addressed as follows: 1. Either provide mirrors or other reflective device to employees who face away from the entrance in spaces accessible to the public or change the configuration of the workspace to enable seeing the public enter. 2. Ensure all campus phones have a sticker with the emergency number for Public Safety. 3. Evaluate light levels and if needed, add lighting.
