Reg FF is a fanfold form of this certificate. 1.0 FORM DESCRIPTION One part continuous form, 8 inches x 12 inches, includes a ½ inch pin feed control strip on both sides, perforated for removal (7 inches x 12 inches torn out). In addition, a horizontal perforation 4 inches from the top of each form must be of strength not to tear when bursting occurs (this perforation will be detached by customer when the vehicle is sold). This perforation must be smooth when detached. The Notice of Release of Liability section of the form (REG 138A), found at the top of the certificate, must be exactly 4 inches from the top of the form to the horizontal perforation. The Certificate portion (REG 17.30FF) must measure exactly 8 inches from the horizontal perforation to the bottom of the form. There are 2,500 forms each per box. 2.0 REVISION DATE 06/2010r 3.0 ORDER VOLUME See 13.0 for product testing instructions. 50,000 (25,000 for each FY order plus an option of 25,000 for a third year) 500 samples of the final approved titles will have the word SAMPLE printed in ½ inch or larger letters, diagonally across the document in red non-solvent proof color. Samples are not to be numbered. 4.0 SERIAL NUMBERING To be provided when contract is awarded Each form will contain a serial number located on the front of the certificate in the bottom right corner. The CA prefix to the nine (9) serial numbers will be printed with Pantone 2935U blue ink. The nine (9) numeric digits will be printed in black ink. These inks will be non-scan and solvent proof color. The serial numbers (i.e. CA ) must be in consecutive sequence with no missing numbers (NOTE: Refer to section14.3 for information on web break requirements.) Date: 1/21/2010 Page 1
2 5.0 DELIVERY SCHEDULE Quantity 09/10 FY 10/11 FY 11/12 FY (Optional) 25,000 per year June 8, 2010 April 7, 2011 March 3, 2012 The delivery date is mandatory. The delivery must have a separate packing slip that matches the shipment precisely. The specified quantity must be delivered as a complete shipment within the specified delivery date(s). Do not split the delivery. The Department of Motor vehicles (DMV) is to be notified two (2) days in advance of the arrival date of each shipment. Call the warehouse at (916) and ask for receiving. Deliveries will only be accepted between 7:00 a.m. and 12:00 p.m. and 12:30 p.m. and 3:00 p.m. (NOTE: Refer to section 16.0 for shipment security measures and delivery address.) The (DMV) reserves the right to change copy prior to the production of these forms. 6.0 INK 6.1 Top portion (Notice of Release of Liability, Reg 138A) Front: Print entirely in solvent proof, red (Sinclair Valentine J6983) non-scan "drop out" ink. Alignment brackets to be printed in Pantone 2935U blue. 6.2 Title portion Front: Pantograph is to be printed in Pantone 196U pink with the border and text printed in Pantone 2935U blue. Red boxes in Sinclair Valentine J6983. Back: Print entirely in Pantone 2935U blue. 6.3 Title Background A pantograph supplied by the vendor and approved by the DMV will be printed in the interior of the bordered title and will display the word VOID repeatedly on the document even when copied with high resolution photo color copiers. The pantograph color will be no darker than specified in 6.2. NOTE: First time bidders must supply samples of multiple pantograph designs with their bid. 7.0 PAPER 7.1 Paper Stock White 24# OCR bond, non-optical brightening paper, U-V Dull, (a minimum of florescence added to the paper). 7.2 Repeating Watermark The paper shall contain a custom cylinder mould-made watermark. A Fourdrinier watermark will NOT be acceptable. The watermark design is to be placed continuously in a vertical position on the face of the paper. Artwork must be approved by DMV prior to creation of the die. The winning vendor must supply their own cylinder mould. Date: 1/21/2010 Page 2
3 7.3 Security Thread A continuous strand of polyester security thread shall be embedded into the paper during production.the thread shall be 1.0 mm in width and shall run vertically one and one half (1 ½) inch, plus or minus one sixteenth (1/16) from left edge of title form. The thread shall be printed with repeatedly in micro printed text. 7.4 Fluorescent Fibers Invisible and visible fluorescent fibers will be incorporated randomly during the paper making process. Quantity and length of fibers will be subject to approval by DMV. The visible fibers shall be red. The invisible fibers shall be fluorescent Yellow/Green and will fluorescence under a black light. 7.5 Chemical Sensitization This paper will react to the five (5) families of chemical falsifying products. The reagents are dispersed in the pulp so that the reaction will occur on both sides of the sheet regardless of which side is subject to chemical application. When one of the solvents is applied, a reaction will appear across the affected area and create a discoloration on the surface of the paper. The vendor will make all necessary arrangements with a reliable paper manufacturer to secure the sensitized paper. Reagents are to be incorporated into the paper that will exhibit chemical reactions (in the form of various stains) to thwart alteration of the document through the use of various chemical agents. Protection is to be afforded from those chemicals that are classified according to the following families: Polar Organic Solvents such as Alcohol s and Ketones A-Polar Organic Solvents, Acids Oxidizing Agents Alkaline Bases 7.6 Toner Retention To maximize retention of the toner images from the laser printer, both sides of the sheet will be fully coated enabling printed images to be anchored to the paper fibers so that attempts to remove the images via tape raising, scraping, erasing, etc., will result in fiber tear. The coating on the reverse is required to enhance sheet stability. The retention agent must not retard or impede the chemical sensitization contained in the paper and must be applied over the chemical sensitizing stains and fade-resistant text. The acceptability of the toner retention must be tested and approved by DMV prior to manufacturing. This testing will be completed upon the sample test forms. 8.0 PRINTING/PRESSWORK 8.1 Lithographic Border A high resolution, lithographic border will be printed on the front of the title (7 inches x 8 inches). Custom border engraving will be provided by the vendor upon successful award of the bid. Date: 1/21/2010 Page 3
4 8.2 Micro Printing The certificate will require one line of micro printing on both the front and reverse of the document. Under magnification, the line will be readable. Location and wording for this micro line will be provided to the selected vendor. A custom repeated pattern of micro-printed wording will be required in certain critical data fields. The pattern will be provided by DMV. 8.3 Warning Banner A line of reverse printing is to be placed in both side borders. It will read Void without bear watermark. Hold to light to view. 8.4 Alignment The printing and alignment on the face of the certificates is critical and must be uniform through out. All forms are to be kept within a tolerance of 1/64 inch. 8.5 State Seal Front: The State Seal will be provided by DMV and will be incorporated as a reverse image on the face of the document. See sample for size and location. Back: The design for the back will be the official seal of the approximately one inch (1 ) in diameter. The seal is to be printed in two columns on the right side, subject to DMV approval. 8.6 Screening The top portion of the form (Notice of Release of Liability, REG 138A) will contain a horizontal box 3 inches x 5/16 inch wide in the upper left corner as shown on the attached sample. Inside the box will be a 25% shaded overlay that will read "DMV MICROGRAPHICS USE ONLY". 9.0 PERFORATION The horizontal perforations shall be such that a full tie of ¼ inch +/- 1/16 inch (6.4 mm +/- 3.2 mm) length is left on each outer edge of the form. The cut and tie length, except for the outer edges, shall be 3.0 to 4.0 mm and 1.0 mm respectively. The fold perforation tear resistance shall be 10 +/- 1N cm using an instrument designed for this type measuring APPROVED PROCESSING METHODS The process of fanfold, interfold or similar method processing the product will be consider acceptable provided registration of all forms horizontally and vertically is kept within a tolerance of 1/64 inch. Satisfactory performance must be attained on the printer and bursting machine. Date: 1/21/2010 Page 4
5 11.0 PROCESSING EQUIPMENT Side perforations must be such that forms can be separated by the following bursting machines, Pitney Bowes series 8, Standard Register Model 2500 and Moore Detacher Model Printing tolerance on the form must be within 1/64 inch to line vertical rules with IBM imprint to be placed on the forms. The Certificate of Title forms are to be compatible for printing on a high speed OCE 6100 Variostream Laser Printer or a Sands 2000 Impact Printer at six lines per inch printing, or an OCEa 372 Pagestream Laser Printer PROOFS The (FAIS) will provide OSP with a mock-up copy. The vendor is to set up the form. Proofs will be provided to DMV for approval no later than fifteen (15) days after receipt of the mock-up copy. Three (3) clear acetate proofs will be included with each proof for checking tabulating copy and forms alignment. DMV reserves the right to request changes and additional proofs. DMV approval of proofs is required prior to vendor s test run Mail proofs to: Michael Mesaros Office of State Publishing 344 North Seventh Street Sacramento, CA Voice: (916) PRODUCT TESTING DMV will require 2,500 fanfold forms in advance of the initial production run for testing purposes. These forms will be stamped SAMPLE and shipped 2-day air. Sample forms are to be of exact construction as the final product. DMV must approve the test forms prior to the vendor s production run. Test run failure due to forms not meeting specifications will require additional production run samples at vendor s expense. Additional test run(s) may be required by DMV. Ship test forms to: 2730 Broadway Sacramento, CA Voice: (916) PLEASE NOTE: The testing for the 10/11 FY and the 11/12 FY may be waived if no problems are encountered with 09/10 FY product run. If a vendor is subcontracting product and changes subcontractors during the course of the contract, new test runs will be required from the new subcontractor SECURITY MEASURES FOR PRODUCTION The vendor, upon award of this bid, guarantees the following requirements are satisfied: 14.1 Negatives, proofs, computer discs, copies that contain format, or other copies of certificates, shall be stored in a locked security area at all times other than during preparation or printing. Following completion of production these items and the final color key on clear acetates are to be sent to DMV using a minimum 3-day delivery, traceable service. Date: 1/21/2010 Page 5
6 14.2 The vendor shall produce only one (1) copy of each serially numbered certificate. When more than one (1) serially numbered certificate is produced due to a printing or procedure error, the excess serially numbered copies shall be immediately destroyed at the press or collator site by the use of a shredding machine located near the press A break in the paper where no certificates are missing should be spliced with a minimum of 3/8 inch or a maximum of one inch perforated laser splicing tape. Splicing tape used is not to cause the printer or bursting machine to jam during printing or bursting All forms not part of the finished product, or certificates produced for inspection by the FAIS manager or a DMV designee shall immediately have the words NOT NEGOTIABLE or SAMPLE printed on them During production and storage by the vendor prior to delivery, all certificates not located at the press site shall be kept in secured storage. The product shall not be stockpiled at the press site or left unattended at any time except in the secured storage area The press shall not be left unattended during production of certificates Certificates are to be packed immediately after being printed or returned to the secured storage area if additional processing is required Cartons containing printed certificates are to be sealed immediately after being packed. This includes partially filled cartons During the production of certificates a FAIS manager or a DMV designee shall have the right to have one or more representatives present at the contractors' premises during the period(s) of production. The contractor agrees to make arrangements for such representative(s) to enter or leave the vendor s premises during the period(s) of production A FAIS manager or a DMV designee shall have the right to have one or more closed circuit television cameras installed on the vendor s premises The successful vendor shall immediately contact the OSP upon notification of receiving the award. 7 14.12 Vendor will provide DMV with a signed authorization (form will be supplied to successful vendor), which authorizes DMV to conduct a background search) and a completed fingerprint card (also supplied by DMV) for each plant employee who will be involved with the production, packing and shipping of the certificates. The vendor shall pay the fees for processing the fingerprints of assigned personnel as established by the Department of Justice. This information is to be submitted to DMV within 15 days of issuance of the purchase order. The vendor will remove any employee who refuses to submit to the above background check from the plant site on the days the certificates are being produced, packed and shipped SHIPPING PALLET REQUIREMENTS 15.1 The pallets are to be Size 2 (42 inches x 42 inches), Type 2 in accordance with the pallet specification A-01. The pallets will be non-returnable. The entire pallet shall be shrink-wrapped. The shrink-wrap must extend over the bottom and top of the stacked cartons or rolls in such a manner as to stabilize the cartons or rolls Cartons are to be designed as to withstand a test of 350 pounds and be capable of being stacked five (5) cartons high when prepared for shipment. Cartons are to be stacked in an interlocking pattern without overhanging the sides of the pallets in excess of two inches (2 ). Stacked cartons cannot be higher than sixty inches (60 ) INCLUDING the pallet The outside of each carton or roll will be identified with the following information in LARGE BOLD LETTERS: 1. P.O. # R FF Revision 06/ Quantity inside each carton 4. The manufacturer s job number 5. The date of production run 6. A chronological box number (for tracking purposes) 16.0 SHIPPING 16.1 Shipments of finished products are to be made from the vendor s printing facility to the DMV Warehouse, located at 4201 Sierra Point Drive, Sacramento, CA in closed and securely locked vehicles, which shall not be unlocked while in transit. In addition to the lock, provided by the vendor, a numbered seal, provided by OSP must be placed on the loading door. The key to the lock will be sent to the DMV Warehouse, not with the carrier. The number of the seal is to be shown on the packing slip These shipments shall not be interlined with other carriers. The drivers shall account for pickup and delivery of the itemized shipment in writing and a copy of such accounting shall be provided to FAIS management. The driver is to be bonded. 8 17.0 COMPLETION 17.1 If the vendor chooses to print more paper than is requested by DMV, DMV will not bear the costs Upon completion of the contract the vendor shall contact the DMV regarding disposal of unprinted stock.
