9 SAFE WORK PERMIT FLOW CHART Safe Work Permit Procedure Attachment #6 Work scheduled via SAP or other methods. Owning department determines types of work activity required and the associated permits needed. Process equipment is prepared for work crew as needed Note: A Safe Work Permit may be required for this preparation. Owning department completes applicable sections of Safe Work Permit. Work activity /Safe Work Permit reviewed with Work Crew representative at the job site. Required signatures are obtained and permit is issued. Work Crew tags or locks applied as needed. Work activity is performed until complete, end of shift or permit is revoked. Work Crew representative returns permit copy to owning department and section VII is completed. Owning department files completed Safe Work Permit.

JOB IN PROGRESS Is the permit signed by the appropriate operations and work crew personnel? Is all required Safety Equipment being used in the field? Have all requirements A-Z been met by the group assigned responsibility? Is the explosion meter calibration date current? Are combustible gas tests being taken at the required frequency? Based on interviews with the owning department and work crew personnel involved, do they understand: Red tag and yellow tag limitations compared to locks; the tags must never be ignored, or removed by unauthorized personnel, the tags must be legible; the tags must be properly afixed to their designated location; and the tags may evoke a false sense of security.
